OBJECTIVE: Recent studies have shown that the mouth-gut-disc axis may play a key role in the process of disc structural failures (including intervertebral disc degeneration (IDD) and endplate change) in the cervical spine and neck pain. However, the potential mechanisms underlying the mouth-gut-disc axis remain elusive. Therefore, we explored whether periodontal disease is associated with disc structural failures in patients with cervical degeneration disorders and clinical outcomes.
METHODS: Adults (aged > 18 years) who met open surgery criteria for cervical spine were enrolled in this prosp

BACKGROUND: Arthroplasty with artificial disc replacement for surgical treatment of cervical spine degeneration was introduced with the notion that motion-preserving approaches would prevent development of adjacent segment disease. Though clinical outcomes favor arthroplasty over the commonly used anterior cervical discectomy with fusion approach, clinical studies confirming the biomechanical basis of these results are lacking. The aim of this study was to compare intervertebral kinematics between arthroplasty and fusion patients 6.5 years post-surgery during physiological motion of the neck.

BACKGROUND CONTEXT: Cervical total disc arthroplasty (TDA) is an alternative procedure to anterior cervical discectomy and fusion that facilitates neural decompression while both preserving motion of the spinal unit and decreasing the risk for degenerative changes at adjacent segments. However, due to its more recent introduction in clinical practice and low complication rates, the modes by which TDA may fail remain to be described.
PURPOSE: This study sought to identify the modes and frequencies of cervical TDA failure in order to propose a novel classification system.

STUDY DESIGN: Systematic review and meta-analysis.
OBJECTIVE: To compare return-to-work (RTW) outcomes between anterior cervical discectomy and fusion (ACDF) and anterior cervical disk replacement (CDR) in working-aged patients with degenerative cervical spine disease.
SUMMARY OF BACKGROUND: Degenerative cervical spine disease frequently affects individuals in their prime working years, causing physical and economic burdens. While both ACDF and CDR are effective surgical options, CDR may allow for faster recovery and earlier RTW. However, prior studies have reported inconsistent findings on

OBJECTIVE: Headache relief after anterior cervical spine surgery has been reported. No study, however, has followed patients out to 10 years to assess the durability of headache improvement. The authors analyzed a group of patients with a 10-year follow-up after one- or two-level cervical disc arthroplasty (CDA) from an FDA investigational device exemption (IDE) study.
METHODS: The authors performed a post hoc analysis of 189 patients treated with CDA from the 9 highest enrolling sites in a prospective multicenter randomized US FDA IDE clinical trial. Patients had one- or two-level CDA at con

INSTRUCTION: Anterior cervical discectomy and fusion (ACDF) is a well-accepted surgical management strategy for radiculopathy and/or myelopathy. However, complications sometimes required subsequent surgery at the operated and adjacent levels. Artificial cervical disc replacement (ACDR) has been accepted as the most extensive non-fusion procedure and is designed to preserve motion. However, some specific complications can result in subsequent surgery, thus, attracting the attention of investigators.
HYPOTHESIS: Compared with ACDF, ACDR had a lower subsequent surgery rates at both operated and

PURPOSE: The alignment at the cervical spine has been considered a determinant of degeneration at the adjacent disc, but this issue in cervical disc replacement surgery is poorly explored and discussed in this patient population. The aim of this systematic review is to compare anterior cervical fusion and total disc replacement (TDR) in terms of preservation of the overall cervical alignment and complications.
METHODS: A systematic review of the current literature was performed, together with the evaluation of the methodological quality of all the retrieved studies.

PURPOSE: While much evidence suggests that adjacent segment degeneration is merely a manifestation of the natural degenerative process unrelated to any spine fusion, a significant body of literature supports the notion that it is a process due in part to the altered biomechanics adjacent to fused spine segments. The purpose of this study was to review and critically analyze the published literature that investigated the in vivo kinematics of the adjacent segments and entire lumbar spine in patients receiving spinal fusion or motion-preserving devices.
METHODS: A systematic review of the PubMe

For more than five decades, anterior cervical discectomy and fusion has been the mainstay of surgical management of symptomatic cervical disc disease. Nonetheless, worries regarding adjacent segment degeneration (ASD) have prompted research into alternative techniques. Cervical disc arthroplasty (CDA), by preserving motion and restoring near-normal spinal biomechanics, seemed to be a promising solution. This review explores normal intervertebral disc (IVD) biomechanics and biomechanical changes following CDA. Evidence from current literature indicates that CDA generally maintains physiological

PURPOSE: Adjacent segment degeneration (ASDeg) after anterior cervical discectomy and fusion (ACDF) seriously affects the long-term efficacy of the operation. Therefore, our team has done a lot of research on allograft intervertebral disc transplantation (AIDT) to prove its feasibility and safety. This study will compare the efficacy between AIDT and ACDF in the treatment of cervical spondylosis.
METHODS: All patients who received ACDF or AIDT in our hospital from 2000 to 2016 and followed up for at least 5 years were recruited and divided into ACDF and AIDT groups. The clinical outcomes incl

Our objective was to compare the efficacy and safety of cervical disc replacement (CDR) versus anterior cervical discectomy and fusion using stand-alone cage (SAC) in single-level degenerative cervical spondylosis (DCS). We conducted a comprehensive search across four databases for articles comparing CDR with SAC in the management of DCS from inception until August 8, 2025. Meta-analysis was performed using Stata17. Seven studies were included in this analysis. In treating single-level DCS, CDR significantly demonstrated longer operative time [WMD = 26.94, 95% CI (17.88, 35.99), p < 0.01], inc

BACKGROUND CONTEXT: Various designs of total disc replacement (TDR) devices have been compared to anterior cervical discectomy and fusion (ACDF) with favorable outcomes in FDA-approved investigational device exemption trials. The design of M6-C with a compressible viscoelastic nuclear core and an annular structure is substantially different than prior designs and has previously demonstrated favorable kinematics and clinical outcomes in small case series.
PURPOSE: To evaluate the safety and effectiveness of the novel M6-C compressible artificial cervical disc compared with ACDF for subjects wi
