PURPOSE: To compare the biomechanical strength of a knotless suture anchor repair and the traditional outside-in repair of peripheral triangular fibrocartilage complex (TFCC) tears in a cadaveric model.
METHODS: We dissected the distal ulna and TFCC from 6 matched cadaveric wrist pairs and made iatrogenic complete peripheral TFCC tears in each wrist. In 6 wrists, the TFCC tears were repaired using the standard outside-in technique using 2 2-0 polydioxane sutures placed in a vertical mattress fashion. In the other 6 wrists, we repaired the TFCC tears using mini-pushlock suture anchors to the f

PURPOSE: To compare the biomechanical strength and safety of an all-arthroscopic triangular fibrocartilage (TFCC) repair technique with an outside-in technique in cadavers.
METHODS: Ten matched pairs of fresh-frozen cadaveric wrists were used for this study. The control group TFCC tears were treated with an outside-in technique using 2-0 polydioxane (PDS) sutures. The experimental group was treated with two FasT-Fix suture devices. I measured the location of the implants in relation to the neurovascular structures using a digital caliper. The strength of the repairs was then determined using

BACKGROUND: The triangular fibrocartilage complex (TFCC) plays an important role in distal radioulnar joint (DRUJ) stabilization, and is frequently torn. In particular, when conservative treatment fails, surgical treatment is needed.
METHODS: In this retrospective study, fourteen individuals with TFCC foveal tears and DRUJ instability were admitted to the department and treated with arthroscopic-assisted modified "double loop suture" transosseous repair between January 2021 and 2023. During surgery, an osseous tunnel was established, and two nickel-based alloy loops and a 2-0 polydioxanone II

BACKGROUND: Incidental triangular fibrocartilage complex (TFCC) abnormal findings could be detected on magnetic resonance imaging (MRI), whether these incidental findings were causes of symptoms can confuse clinicians. The purpose of this study was to demonstrate the MRI features of triangular fibrocartilage (TFC) disc in asymptomatic adults' wrists and explore the association with age, gender and ulnar variance.
METHODS: Healthy adult volunteers who reported no ulnar-sided wrist pain were recruited and underwent MRI examinations. The thickness and angle of TFC and ulnar variance were measure

Background: To restore distal radioulnar joint stability following injury to the Triangular Fibrocartilage Complex (TFCC), foveal repair surgery may be necessary. Post-surgery rehabilitation is prescribed to restore wrist and hand function; however, no universally accepted or definitive rehabilitation protocol currently exists. The aim of this study was to survey hand and wrist surgeons regarding their recommended postoperative rehabilitation protocols following TFCC foveal repair surgery. Methods: Australian hand and wrist surgeons were invited to complete a descriptive survey containing 10 q

BACKGROUND: We developed a new ligamentoplasty technique using the ulnotriquetral ligament (UTL) via a palmar approach for cases with dorsal instability of the ulna due to a foveal tear of the triangular fibrocartilage complex (TFCC). The study aimed to evaluate the clinical outcomes of this procedure.
METHODS: We retrospectively analyzed 19 cases with foveal tears of TFCC who underwent the ligamentoplasty using UTL. We harvested the UTL from the triquetrum as a pedicle graft, avoiding damage of the connection between the base of the UTL and palmar radioulnar ligament (PRUL) as possible. The

PURPOSE: Tears of the proximal component of the triangular fibrocartilage complex (pc-TFCC) lead to instability in the distal radioulnar joint. The aim of this study was to measure the load-bearing radioulnar distances (RaUls) and to evaluate its diagnostic suitability in patients with pc-TFCC tears.
METHODS: We retrospectively assessed and compared the lateral wrist radiographs of 61 adult patients with arthroscopically confirmed tears of pc-TFCC with those of a control group of 64 healthy participants. The RaUl was measured on lateral radiographs, and the difference in RaUl (D-value) betwee

BACKGROUND: Ulnar shortening osteotomy (USO), as its name implies, is used to shorten the ulna. It subsequently tightens the triangular fibrocartilage complex (TFCC) and ulnar wrist. TFCC foveal insertion is a primary stabilizer of the distal radioulnar joint. It is unclear whether USO is effective in TFCC foveal injuries. The purpose of this study was to review the clinical outcomes of ulnar shortening osteotomies with and without TFCC foveal injuries.
METHODS: We retrospectively reviewed patients with ulnar wrist pain treated with USO and wrist arthroscopy including the distal radioulnar jo

OBJECTIVE: This study was performed to determine the prevalence of triangular fibrocartilage complex (TFCC) injuries as shown by 3.0T magnetic resonance imaging (MRI) in patients with distal radius fractures.
METHODS: In total, 57 patients with distal radius fractures underwent 3.0T MRI examinations to observe the incidence of TFCC injuries after manual reduction and cast fixation. The fracture type was categorized by the AO classification, and the TFCC injury pattern was evaluated using the Palmer classification. The correlation between the location of the TFCC injury and the distal radius fra

UNLABELLED: A total of 20 patients scheduled for wrist arthroscopy, all with clinical signs of rupture to the triangular fibrocartilage complex and distal radioulnar joint instability, were tested pre-operatively by an independent observer for strength of forearm rotation. During surgery, the intra-articular pathology was documented by photography and also subsequently individually analysed by another independent hand surgeon. Arthroscopy revealed a type 1-B injury to the triangular fibrocartilage complex in 18 of 20 patients. Inter-rater reliability between the operating surgeon and the indep

The stabilising effects of various structures of the distal radioulnar joint (DRUJ) have been heavily debated. This biomechanical cadaveric study examined the effects of the volar and dorsal lips of the sigmoid notch and the volar and dorsal aspects of the triangular fibrocartilage complex (TFCC) on DRUJ stability. Sequential fractures of the distal radius and sectioning of the TFCC were performed followed by measurements of ulnar translation with the forearm in pronation, neutral and supination. A dorsal lunate facet fracture created instability in pronation. Lunate facet fractures alone did

BACKGROUND: Triangular fibrocartilage complex (TFCC) injuries are a common source of ulnar-sided wrist pain and functional impairment, particularly in athletes and individuals engaged in upper-limb-dominant or wrist-loading activities. Despite their prevalence, standardised, non-surgical treatment pathways remain underdeveloped.
HYPOTHESIS/PURPOSE: This narrative review aims to critically synthesise and discuss current evidence on the non-surgical management of TFCC injuries in physically active adults.
STUDY DESIGN: Narrative review.
METHODS: A structured literature search was performed in

BACKGROUND: Deep ulnar fossa tears of the triangular fibrocartilage complex (TFCC) frequently lead to instability of the radioulnar joint. Although positive Ulnar Variance (UV) is a recognised longitudinal bony factor associated with TFCC injury, many tears still occur in patients with neutral or negative UV; the influence of the morphological orientation of the ulnar styloid process in the coronal plane on ligament susceptibility remains unclear.
OBJECTIVE: To assess whether the ulnar styloid abduction angle (USAA) is an independent bony predictor of Palmer Type IB TFCC tears, and to investi

Background In distal radius fractures (DRFs) occurring in nonosteoporotic age groups, it is known that triangular fibrocartilage complex (TFCC) injuries are frequently combined, with some experiencing persistent symptomatic instability. However, a gold standard treatment for these associated lesions has not yet been established. Purpose Our purpose was to investigate the outcomes of primary arthroscopy-assisted transosseous foveal repair of the TFCC associated with DRFs in young adults. Materials and Methods Fifteen patients aged <65 years who underwent open reduction and internal fixation

CASE: A 34-year-old farmer and railroad worker injured his left wrist when working at a railroad. The resulting dorsal-ulnar wrist blow caused disabling pain. Splits and 2 subsequent surgeries failed, including an arthroscopic triangular fibrocartilage complex (TFCC) debridement and thermal ablation. Subsequently, magnetic resonance imaging documented a rare Palmer type 1C distal TFCC avulsion. Function was restored and return to farming permitted by reconstruction of the ulnotriquetral (UT) ligament using an extensor carpi ulnaris (ECU) slip.
CONCLUSION: Chronic Palmer type 1C TFCC injuries
