Unilateral sudden hearing loss: a rare symptom of Moyamoya disease.

A 38-year-old female patient experienced a sudden onset of unilateral sensorineural hearing loss due to Moyamoya disease. A detailed summary of audiological and neurological findings indicated that the sudden hearing loss might be due to Moyamoya disease resulting in occlusion of posterior and middle cerebral arteries. Intravenous prednisolone and trimetazidine dihydrochloride may improve hearing thresholds and speech understanding. To our knowledge, this is the first article in the literature reporting a case of sudden hearing loss as the first manifestation of Moyamoya disease in a young adu

[Painful left bundle branch block].

The author presents a case where the frequency-dependent left bundle branch block causes angina complaints with typical effort in normal coronary artery. The exercise ECG test triggered angina pectoris and a synchronous complete left bundle branch block appeared on the ECG. After the exercise, both the left bundle branch block and the angina pectoris disappeared. The angina pectoris and left bundle branch block with combined treatment with ivabradine and trimetazidine did not occur later. The possible pathomechanism of painful left bundle branch block and treatment options are discussed. Orv H

Medical and social issues of cardiovascular diseases and their solution based on the experimental study of myocardial fibrosis.

OBJECTIVE: Introduction: The prevalence and incidence of cardiovascular diseases have been attracting considerable attention in recent decades. This is partly due to the fact that myocardial fibrosis is the major consequence of the most nosological units of cardiovascular diseases. We believe that early pathogenic therapy of myocardial fibrosis should be taken into consideration as a solution to this issue. The change of the connective tissue metabolism in myocardium is the central chain in pathogenesis of diffuse ischemic necrotic cardiosclerosis (DINC) occurs after repeated epinephrine injur

Evaluating cardioprotective strategies for anthracycline-induced cardiotoxicity in breast cancer: insights from a systematic review and network meta-analysis.

INTRODUCTION: Anthracycline-induced cardiotoxicity is a significant concern for breast cancer patients undergoing treatment, often leading to chronic cardiovascular complications and reduced long-term survival. The study aimed to systematically evaluate the efficacy of nine classes of pharmacological agents in protecting against cardiotoxicity in breast cancer patients treated with anthracyclines. METHODS: A comprehensive search of databases was performed from January 2000 to October 2024 to identify randomized controlled trials (RCTs) investigating cardioprotective agents. The risk of bias i

High rate of potentially inappropriate medication use in older people: a case-control study.

Annually, 172 million fall events cause temporary or permanent impairment in older adults, and this number is increasing. Contributing factors that increase the risk for falls include age, polypharmacy, and malnutrition. This study evaluated medications mainly included in the EU(7)-PIM (potentially inappropriate medication) list. From March 21, 2022, to July 6, 2022, 945 patients who experienced a fall and visited the Department of Emergency Medicine at the Albert Szent-Györgyi Health Centre of the University of Szeged in Hungary. Data from 886 patients were collected (study group). The contro

Trimetazedine with hyperinsulinimea-euoglycemia, N-acetyl cysteine, and vitamin C: A new approach concept for management of aluminum phosphide poisoning.

Aluminum phosphide (AlP) is commonly used as a powerful suicidal tool. The exact mechanism of acute toxicity has not been well defined despite high mortality rates as well as its supportive treatment including rapid decontamination and institution of resuscitative measures. The current study aimed to investigate a new combination therapy using trimetazidine, N-acetyl cysteine, vitamin C, and hyperinsulinemia-euglycemia to manage acute AlP poisoning. Acute AlP-induced cardiotoxicity, hemodynamic changes, and hepatotoxicity were evaluated using electrocardiogram, creatinine kinase MB iso-enzyme,

Anti-ischemic and pleiotropic effects of ranolazine in chronic coronary syndromes.

The vast majority of antianginal drugs decrease heart rate and or blood pressure levels or the inotropic status of the left ventricle to decrease myocardial oxygen consumption (MVO2) and thus anginal symptoms. Ranolazine presents a completely different mechanism of action, which reduces the sodium-dependent calcium overload inhibiting the late sodium current. Current European Society of Cardiology (ESC) guidelines for the management of angina in patients with chronic coronary symptoms recommend the use of several drugs such as ranolazine, b-blockers, calcium channel blockers, long-acting nitra

Stable angina pectoris: current medical treatment.

Stable angina represents the main symptom of established coronary artery disease. In addition atherosclerosis is the common pathological substrate of chronic stable angina as well as acute coronary syndromes. The aim of stable angina management is the symptomatic relief and the secondary prevention. Lifestyle modification and pharmacological therapy are the cornerstones of chronic coronary artery disease management irrespectively of possible surgical or percutaneous revascularization. Optimal medical therapy is a combination of antianginal/antiischemic drugs and disease modifying agents, inclu

Stable Angina Pectoris: A Review of Pathophysiology, Diagnosis, and Its Management.

India is at the cusp of an impending epidemic of cardiovascular diseases (CVD). It has been triggered by rapid urbanization, industrialization, and globalization. Coronary artery disease (CAD) and ischemic heart disease (IHD) clinically present as angina pectoris (chest pain and discomfort). Despite advances in treatment options for CAD, a lack of awareness of risk factors and disease, access to healthcare, and affordability are the primary concerns in low- and middle-income countries. In India, CAD results in >7 million deaths annually. There is a need for active collaboration between patient

[Oxygen-dependent metabolism of neutrophils and monocytes in patients with unstable angina pectoris and its correction with preductal].

Spectrophotometric NBT-test and luminol-dependent chemiluminescence were used to study oxygen-dependent metabolism of neutrophils and monocytes; antioxidant defense was evaluated by activity of superoxide dismutase measured by spectrophotomety and peroxide chemiluminescence of blood plasma; heparin and trimetasidine (preductal) effects on the above metabolism were studied in 39 patients with unstable angina pectoris. All the patients were found to have a significant rise in functional activity of neutrophils and monocytes by NBT-test and insignificant one by chemiluminescence, unchanged activi

Nutrient-Driven Antioxidant Interventions for Prevention of Age-Related and Diabetic Cataracts.

Cataract formation remains a significant cause of global visual impairment. Increasing attention has been directed toward antioxidant-based interventions as potential non-surgical strategies to delay or prevent cataractogenesis, particularly in the age-related and diabetic contexts. This review summarizes recent preclinical evidence on nutritional antioxidants for the prevention of age-related and diabetic cataracts. Agents such as trimetazidine, Moringa oleifera stem extract, ginsenoside Rg1, lanosterol nanoparticles, β-casomorphin-7, and cerium oxide-based nanotherapies have been shown to mi

Old and New Drugs for Treatment of Stable Angina: New Anti-Anginal Drugs and Coronary Revascularization.

Since the first human catheterization performed by Forssman in 1929 angioplasty equipment and medical therapies have undergone considerable evolution and technical improvement allowing interventionalists to perform more complex procedures and solving most of the percutaneous limitations. While percutaneous coronary intervention (PCI) has dramatically changed the outcome in the Acute Coronary Syndrome (ACS) setting, its role in the treatment of chronic stable angina is still debated. Stable coronary artery disease (SCAD) is a major public health issue and its prevalence is still increasing in t

An intrinsic mechanism of metabolic tuning promotes cardiac resilience to stress.

Defining the molecular mechanisms underlying cardiac resilience is crucial to find effective approaches to protect the heart. A physiologic level of ROS is produced in the heart by fatty acid oxidation, but stressful events can boost ROS and cause mitochondrial dysfunction and cardiac functional impairment. Melusin is a muscle specific chaperone required for myocardial compensatory remodeling during stress. Here we report that Melusin localizes in mitochondria where it binds the mitochondrial trifunctional protein, a key enzyme in fatty acid oxidation, and decreases it activity. Studying both
