Effects of rivastigmine on the pathogenesis of the experimental trypanosomiasis.

Inflammation is important mechanism in Chagas disease (CD) pathogenesis. Considering that CD treatment presents a limited efficacy and anticholinesterase drugs may have antiinflamamtory properties, they could be used as a therapeutic option. We decided evaluate the action of rivastigmine on the anti-inflammatory cholinergic pathway during the course of CD. For this, 120 "Swiss" mice were divided into three groups: 30, 60 and 180 days, and subdivided into four subgroups: Uninfected/untreated (CTRL); uninfected/treated with rivastigmine (RIV); infected/treated (INF + RIV) and infected/untreated

The evolving spectrum of human African trypanosomiasis.

Human African trypanosomiasis (HAT), or sleeping sickness, continues to be a major threat to human health in 36 countries throughout sub-Saharan Africa with up to 60 million people at risk. Over the last decade, there have been several advances in this area, some of which are discussed in this overview. Due to the concerted efforts of several bodies, including better identification and treatment of cases and improved tsetse fly vector control, the number of cases of HAT has declined dramatically. The clinical heterogeneity of HAT has also been increasingly recognized, and the disease, while us

Pastoralists' Vulnerability to Trypanosomiasis in Maasai Steppe.

Trypanosomiasis is a neglected tropical disease of both livestock and humans. Although pastoral communities of the Maasai Steppe have been able to adapt to trypanosomiasis in the past, their traditional strategies are now constrained by changes in climate and land regimes that affect their ability to move with their herds and continually shape the communities' vulnerability to trypanosomiasis. Despite these constraints, information on communities' vulnerability and adaptive capacity to trypanosomiasis is limited. A cross-sectional study was therefore conducted in Simanjiro and Monduli district

Control and surveillance of human African trypanosomiasis.

In the 1960s, it appeared that human African trypanosomiasis (HAT) could be effectively controlled, but by the beginning of the twenty-first century several decades of neglect had led to alarming numbers of reported new cases, with an estimated 300 000 people infected. The World Health Organization (WHO) responded with a series of initiatives aimed at bringing HAT under control again. Since 2001, the pharmaceutical companies that produce drugs for HAT have committed themselves to providing them free of charge to WHO for distribution for the treatment of patients. In addition, funds have been p

The detection and treatment of human African trypanosomiasis.

Human African trypanosomiasis (HAT) is caused by the injection of Trypanosoma brucei (T. b.) gambiense or T. b. rhodesiense by Glossina, the tsetse fly. Three historical eras followed the exclusive clinical approach of the 19th century. At the turn of the century, the "initial research" era was initiated because of the dramatic spread of HAT throughout intertropical Africa, and scientists discovered the agent and its vector. Two entities, recurrent fever and sleeping sickness, were then considered a continuum between hemolymphatic stage 1 and meningoencephalitic stage 2. Treatments were develo

Trypanosomiasis, cardiomyopathy and the risk of ischemic stroke.

American (Chagas disease) and African (sleeping sickness) trypanosomiasis are neglected tropical diseases and are a heavy burden in Latin America and Africa, respectively. Chagas disease is an independent risk factor for stroke. Apical aneurysm, heart failure and cardiac arrhythmias are associated with ischemic stroke in chagasic cardiomyopathy. Not all chagasic patients who suffer an ischemic stroke have a severe cardiomyopathy, and stroke may be the first manifestation of Chagas disease. Cardioembolism affecting the middle cerebral artery is the most common stroke subtype. Risk of recurrence

Cardiac involvement in African and American trypanosomiasis.

American trypanosomiasis (Chagas disease) and human African trypanosomiasis (HAT; sleeping sickness) are both caused by single-celled flagellates that are transmitted by arthropods. Cardiac problems are the main cause of morbidity in chronic Chagas disease, but neurological problems dominate in HAT. Physicians need to be aware of Chagas disease and HAT in patients living in or returning from endemic regions, even if they left those regions long ago. Chagas heart disease has to be taken into account in the differential diagnosis of cardiomyopathy, primarily in patients with pathological electro

Problems for the chemotherapy of human African trypanosomiasis.

Problems associated with the current therapies of sleeping sickness include toxicity, resistance and a lack of a guaranteed supply. However, no new formulations are close to gaining a licence for clinical use and relatively few compounds have been shown to be effective in experimental systems. Many potentially good biochemical targets for drugs have been identified. Some of these have been validated and lead compounds have been developed. However, the biology of trypanosomes means that various pharmacological demands must be met in developing new trypanocides for clinical use. Foremost among t

Pharmacology of existing drugs for animal trypanosomiasis.

Lack of much interest by the pharmaceutical industry to venture into development of new antitrypanosomal drugs has been a major stimulus to an intensification of research into the few existing drugs. Those indicated for animal trypanosomiasis include: isometamidium, homidium and diminazene, used primarily against Trypanosoma congolense, T. vivax and T. brucei; and quinapyramine, mainly indicated for use against T. evansi infections. A great deal of research effort has focused on development of pharmacological and parasitological methodologies, which have considerably advanced our understanding

Pathophysiologic correlates of experimental trypanosomiasis in rabbits.

Rabbits inoculated subcutaneously with Trypanosoma brucei brucei developed parasitemia, fever, and reduced food and water intake within 4 to 6 days postinoculation. Subsequent alterations in clinicopathologic parameters included anemia and increased circulating nucleated red blood cells, fibrinogenemia, hypertriglyceridemia, and hyperproteinemia. Transient alterations in the numbers of neutrophils and lymphocytes were detected sporadically; however, leukocytosis was not a characteristic of this chronic infectious condition in rabbits.

Trypanosomiasis: a public health priority in Cameroon.

From a historical standpoint and the present distribution of the disease, human trypanosomiasis, in spite of its comparatively low incidence, still remains a public health priority in Cameroon, requiring both national and international support for control measures. The geographical position of the country within the Glossina belt, certain cultural practices, agro-industrial activities, and an improving transport system are current factors favouring the spread of the disease. Similarly the changing pattern of the disease, ignorance and the lack of valid diagnostic and safe chemotherapeutic tool

Camel trypanosomiasis and its vectors in Somalia.

Blood samples from 3000 Somali camels (Camelus dromedarius) were examined for trypanosome infection. Of these, 160 (5.33%) were infected with Trypanosoma evansi, one (0.03%) with T. congolense and one (0.03%) with T. brucei. Camel trypanosomiasis occurred in most areas of tabanid infestation throughout the country. The tabanids Philoliche zonata and P. magretti are incriminated as the major vectors of the disease.

Reproductive disorders in African trypanosomiasis: a review.

Reproductive disorders are frequently seen in human beings and in animals infected with tsetse-transmitted (African)trypanosomiasis. The disorders include irregular menstrual (or oestrus) cycle, infertility, abortion and impotence. Intrauterine infections occasionally occur, resulting in still birth or neonatal mortality. The changes are essentially reversible after treatment, although recovery may take several months.

Immunological approaches to the control of animal trypanosomiasis.

Control of African trypanosomiasis relies heavily on attempts to eliminate the tsetse fly vectors, and the use of trypanocidal drugs which are not entirely satisfactory. But the prospects for immunological control have a long history - since the last century, explorers and settlers in Africa have progressively exposed their horses and cattle to tsetse bites (a process known as 'salting') in an attempt to build up some degree of immunity to the disease. More recently, immunologists and biochemists have used modern techniques to explore the possibility of suitable vaccines. In this article, Vina

[Complications observed in the treatment of trypanosomiasis in the Congo].

The authors present the results obtained in the treatment of 453 sleeping sickness (T. gambiense) patients in the People's Republic of the Congo. They give the following conclusions: The mortality rate reaches 2,75% in the group of patients treated by melarsoprol and is nil in the group treated with pentamidin. The encephalitis rate is 2,1 per 100 series of melarsoprol. The treatment of encephalopathies with adrenaline according to the protocol of SINA gives very good results and should be generalized.

Thrombocytopenia: a uniform complication of African trypanosomiasis.

Because an increasing number of reports have indicated that thrombocytopenia may occur during the course of African trypanosomiasis, a comprehensive study was designed to analyse the variables influencing the incidence and severity of this complication. Thrombocytopenia occurred in all of 40 infected domestic livestock and wildlife that were studied. The magnitude of the platelet reduction was independent of the route of inoculation (intravenous or transmission by tsetse), the genus or breed of livestock, and the species or variable antigen type (VAT) of trypanosome (several VATs of Trypanosom
