[Internal fixation for the treatment of anteromedial facet fracture of the coronoid process of ulna].

OBJECTIVE: To investigate the effect of operative treatment for anteromedial facet fracture of the coronoid process of ulna, and to study its surgical exposures and fixation techniques. METHODS: From March 2005 to March 2010,18 patients with anteromedial facet fracture of the coronoid process of ulna were treated with open reduction and internal fixation. There were 12 males and 6 females with an average age of 37.8 years. A single midline posterior incision was used to expose the entire elbow joint. After elevating the full-thickness skin flaps, a lateral incision was made to expose and repai

Complex fracture-dislocations of the proximal ulna and radius in adults: a comprehensive classification.

BACKGROUND: Complex fracture-dislocations of the proximal ulna and radius include multiple anatomic lesions, the management of which is known to be demanding. Although several classifications have been proposed, none appear to be exhaustive, and most of them have neither therapeutic nor prognostic value. The purpose of this study was to design a comprehensive classification that may provide a guide for the operative management of these injuries. MATERIALS AND METHODS: The classification is aimed at identifying definite anatomic lesions, called the "main lesions," the presence of which can aff

Spontaneous bilateral distal ulna fracture: an unusual complication in a rheumatoid patient.

Bilateral ulna stress fractures are extremely rare. Patients with rheumatoid arthritis have osteopenic bone secondary to a variety of causes. We report a case of bilateral stress fractures of the ulna in an elderly patient with rheumatoid arthritis, and literature on this condition is reviewed. Prompt recognition and activity modification are essential to treat this rare injury. Recovery can take up to 12 weeks.

[Continuous skeletal traction in multifragmentary fractures of radius and ulna combined with cast].

INTRODUCTION: Fractures of the distal third of the radius are frequently encountered in emergency services. Until recently considered as benign, however, they represent a group of numerous therapeutic problems, as debated in anatomical, radiological and clinical studies. OBJECTIVE: To emphasize the importance of recovering the radial distance and the articular congruence in fractures of radius treated with a cast. HYPOTHESIS: To demonstrate that ligamentotaxis obtained with the application of the Cole-Obletz technique is a good method for treatment of distal radius fractures that have a comp

The surgical treatment of Kienböck's disease by radius and ulna metaphyseal core decompression.

We present a new surgical procedure, metaphyseal core decompression, to treat Kienböck's disease. Twenty-two patients were treated between 1976 and 1988 and were retrospectively reviewed. Sixteen were male and 6 female with an average age of 36 years (range, 18-64 years). The surgical technique involved curettage of the distal radius and ulna metaphyseal bone through small cortical windows. The average follow-up period was 10 years (range, 6-16 years). No postoperative complications were noted and no patient underwent additional surgical procedure. Sixteen patients were pain-free; 4 noted occa

Irreducible Galeazzi fracture-dislocation due to an avulsion fracture of the fovea of the ulna.

We report a rare case of Galeazzi fracture-dislocation with an irreducible distal radioulnar joint. The cause of the irreducibility was entrapment of a fragment avulsed from the fovea of the ulna. The patient was successfully treated with open reduction and internal fixation of the radius, ulnar styloid process and avulsed fracture at the fovea of the ulna.

Opening versus closing wedge osteotomy of the curved ulna in radial clubhand.

The use of an opening or closing wedge osteotomy to straighten the curved ulna in radial clubhands was simulated by using a specially designed computer program. Sixty-eight lateral x-ray films of curved ulnas in 39 children aged from a few weeks to early adolescence were examined. When lengthening is possible, closing wedge osteotomies yield, at best, only millimeters gain in length. Many will decrease bone length. There is no advantage in using multiple closing wedges. Opening wedges of reasonable angle will lengthen the ulna by 5 mms or more. Some multiple opening wedges can produce slightly

Matched distal ulna resection for posttraumatic disorders of the distal radioulnar joint.

In 1985 we published the results of the matched ulna resection in 44 patients, the majority of whom had rheumatoid arthritis. The matched ulna resection maintains the continuity of the distal ulna to the ulnar sling mechanism, including the triangular fibrocartilage complex (TFCC), and resects the distal ulna in a smooth, curved, convex fashion to match the contour of the radius throughout forearm rotation. This article presents the results of the procedure in patients with posttraumatic and mechanical disorders of the distal radioulnar joint. Good to excellent results were noted in 24 of 32 p

The treatment of chronic traumatic subluxation of the distal ulna by hemiresection interposition arthroplasty.

Dorsal and palmar subluxation of the distal ulna can be difficult to both diagnose and treat. Patients often present long after their initial injuries with posttraumatic arthritic changes in the distal radioulnar joint. Hemiresection interposition arthroplasty is a salvage procedure that is reliable for both pain relief and restoration of motion.

Osteochondral fractures of the glenoid fossa of the ulna in children: review of four cases.

Osteochondral fractures of the glenoid fossa of the ulna are exceptional injuries in children. These lesions can present diagnostic problems because the fragments are often radiolucent. We followed four children for 3, 6, and 24 months and 12 years, respectively. The elbow was definitely dislocated in two, and was probably dislocated in two other, children. The most reliable paraclinical examination was conventional sagittal tomography. Open revision is mandatory for reduction of the fracture and, when performed early, may lead to excellent functional results.

Pathologic fracture of the proximal ulna through a bone graft donor site.

Complications of harvesting autologous bone graft from the iliac crest and tibia are well known; exclusive use of upper extremity donor sites in hand surgery has therefore been advocated. Six weeks of postoperative elbow splinting has been recommended after harvest of large proximal ulnar grafts. We report two cases of pathologic fracture of the proximal ulna complicating local bone grafting and question whether the risk of fracture and the attendant patient inconvenience and potential joint stiffness incurred by postoperative immobilization are exceeded by the morbidity of carefully done ilia

Isolated fracture of the coronoid process of the ulna: a case report and review of the literature.

A rare case of a fracture involving almost the entire coronoid process of the ulna (type III) is described. Open reduction and internal fixation of this fragment restored elbow stability and resulted in a painless range of motion lacking only 5 degrees of extension at 7 months.

A new hereditary bone dysplasia with characteristic bowing and thickening of the distal ulna.

An autosomal dominant hereditary bone dysplasia was observed in three members of a Sardinian family living in Germany--the father and two sons. The guiding clinical symptom is thickening of the wrist proximal to the styloid process of the ulna. The radiologically recognizable changes--non-calcified cartilaginous islands protruding like cones into the metaphysis--begin with the onset of puberty and are not always associated with pain. Varying degrees of thickening and bowing of the relatively poorly mineralized metaphyseal region, particularly of the distal ulna, occur at the end of skeletal de

[Fractures of the upper end of radius and ulna and recent dislocations of the elbow in children].

In this report concerned with luxation of the elbow and fractures of the upper end of two forearm bones in children emphasis is placed on the importance of the initial treatment, since sequelae are not corrected during growth. Correct therapy requires firstly a precise diagnosis, this depending on the obtaining of images of very good quality, if necessary taken during sleep if the child has to be anesthetized. Therapy should be as atraumatic as possible, particularly in the region of the head of radius, to avoid its subsequent necrosis.

Posterior dislocation of the radial head without fracture of the ulna. A case report.

Posterior radial head dislocation without fracture of the ulna in an adult is extremely rare. A case is presented in which immediate closed reduction, a short period of immobilization, and early active range of motion resulted in a very good outcome. Early detection by thorough physical examination and careful radiographic analysis is crucial for a good final result.

Long-term results of resection of the distal ulna for post-traumatic conditions.

Sixty-two patients underwent resection of the distal ulna because of pain or limited motion after wrist trauma. The median length of resection was 23 mm. Injury to the distal radioulnar joint occurred primarily during fractures of the distal radius. Followup time averaged 87 months. The primary surgical indication was pain, but some procedures were performed in an effort to increase motion. All patients improved after surgery. Three patients had residual pain, 25 had mild pain, and 34 had no pain. Supination was greatly improved, with only modest improvement in other wrist motions. Four patien
