Outcomes after injury to the thumb ulnar collateral ligament--a systematic review.

OBJECTIVES: Rupture of the ulnar collateral ligament (UCL) is a frequent injury of the hand. When untreated, this injury may lead to decreased pinch strength, pain, instability, and osteoarthritis. There is currently no consensus on treatment of acute or chronic UCL injuries. Our primary purpose was to compare nonoperative treatment with surgical repair and surgical reconstruction of thumb UCL injuries. A secondary purpose was to compare graft choice and surgical technique for reconstruction. DATA SOURCES: A systematic review of multiple medical databases was performed using PRISMA (Preferred

Surgical Management of Chronic Ulnar Collateral Ligament Injury of the Thumb: A Systematic Review.

Chronic ulnar collateral ligament (UCL) injuries of the thumb differ from acute tears in tissue quality and surgical options, yet the relative merits of direct repair versus anatomic reconstruction remain uncertain. We conducted a Preferred Reporting Items for Systematic reviews and Meta-Analyses-aligned systematic review synthesising operative techniques, rehabilitation protocols, outcomes, and complications for chronic, closed thumb metacarpophalangeal (MCP) joint UCL injuries in adults. Two reviewers independently screened studies and extracted data; risk of bias was appraised with Risk Of

Reconstruction of the rheumatoid thumb.

Most rheumatoid patients will present with one or more thumb deformities at some stage of their clinical history. The goal of treatment is restoration and maintenance of stable and painless motion. Treatment is based on the type and stage of the deformity. The boutonniere thumb is the most common deformity. Metacarpophalangeal arthrodesis is preferred for isolated metacarpophalangeal involvement. For advanced cases in a low-demand patient, metacarpophalangeal arthroplasty with interphalangeal arthrodesis is performed. In the higher demand hand with an uninvolved carpometacarpal joint, arthrode

Predicting and Preventing Injury in Major League Baseball.

Major League Baseball (MLB) players are at significant risk for both chronic, repetitive overuse injuries as well as acute traumatic injuries. Pitchers have been shown to be at higher risk for sustaining injuries, especially upper extremity injuries, than position players. The past several MLB seasons have seen a dramatic rise in the number of ulnar collateral ligament reconstructions performed in MLB pitchers. Several recent prospective studies have identified risk factors for injuries to both the shoulder and elbow in MLB pitchers. These risk factors include a lack of external rotation, a la

A Review of Research on Throwing Biomechanics, Upper Extremity Injuries, and Treatment of Throwing Injuries in Professional Baseball and Football.

BACKGROUND: Throwing is a fundamental skill in many sports that requires intricate biomechanical coordination to achieve maximum performance and minimize injury. Baseball pitchers and football quarterbacks have distinct throwing motions that involve unique demands on the upper extremity, which ultimately contribute to injury, especially in cases of overuse. PURPOSE: To compare the throwing biomechanics, incidence, and types of injuries sustained and their respective treatments between baseball pitchers and football quarterbacks. STUDY DESIGN: Narrative review. METHODS: Comprehensive literat

How much valgus instability can be expected with ulnar collateral ligament (UCL) injuries? A review of 273 baseball players with UCL injuries.

BACKGROUND: The dynamic stress radiograph of the elbow was designed to help the clinician better define valgus laxity and instability in the throwing athlete. However, no large study has quantified the amount of laxity in athletes with ulnar collateral ligament (UCL) injuries. We hypothesized that valgus stress radiographs in patients with UCL injuries will demonstrate a significantly greater amount of valgus stress opening of the dominant elbows compared with the nondominant elbow. METHODS: Bilateral elbow static and stress radiographs that were taken as part of our standard preoperative wor

Incidence of Ulnar Collateral Ligament Surgery and Revision in Baseball Players: A Systematic Review and Meta-Analysis.

Ulnar collateral ligament (UCL) injuries are a major concern in baseball because of repetitive valgus stress from overhead throwing, often leading to surgical intervention. In recent years, UCL reconstruction has become increasingly frequent. Although this procedure has a high success rate, revision surgery is often required. This systematic review and meta-analysis examined the incidence of UCL surgery and revision procedures in baseball players, explored potential risk factors, and identified areas for future research. A comprehensive search of PubMed, Google Scholar, CINAHL, Embase, and Spo

Timing Matters: Challenges in Delayed Surgical Management of Pediatric Humeral Medial Epicondyle Fractures.

BACKGROUND: Humeral medial epicondyle fractures comprise 12% of pediatric elbow injuries. Delay in surgical intervention may be more involved than acute surgery due to soft tissue shortening, fragment-induced joint surface damage, elbow stiffness, and ulnar nerve displacement/scarring. There are limited prior case reports on delayed surgical management. The purpose of this study is to provide a descriptive analysis of a series of medial epicondyle fracture patients treated with delayed surgical management. METHODS: Delayed surgical management was defined as any operative repair of the medial

Repair and augmentation of the lateral collateral ligament complex using internal bracing in dislocations and fracture dislocations of the elbow restores stability and allows early rehabilitation.

PURPOSE: Most elbow dislocations can be treated conservatively, with surgery indicated in special circumstances. Surgical options, apart from fracture fixation, range from repair or reconstruction of the damaged ligaments to static external fixation, usually entailing either a long period of immobilization followed by carefully monitored initiation of movement or dynamic external fixation. In general, no consensus regarding surgical treatment has been reached. A new method of open ligament repair and augmentation of the lateral ulnar collateral ligament using a non-absorbable suture tape in ca
