Policy integration of pediatric oral health: aligning medicine and dentistry for child well-being.

Oral health remains a critical yet overlooked aspect of pediatric health, despite strong evidence linking untreated dental disease with systemic complications, impaired growth, and reduced quality of life. Globally, early childhood caries and related oral conditions affect 60%-90% of children, disproportionately impacting disadvantaged groups. However, pediatric oral health is often siloed from mainstream child health services. Pediatricians, frequently the first point of contact for families, have limited oral health training, while pediatric dentists often see children only when conditions h

Health insurance, perceived threat, and posttraumatic stress after suspected acute coronary syndrome.

OBJECTIVE: Threat perceptions during evaluation for acute coronary syndrome (ACS) in the emergency department (ED) predict posttraumatic stress symptoms (PSS). It is unknown how health insurance status affects threat perceptions. We tested whether lacking health insurance is associated with higher threat perceptions and PSS in patients with suspected ACS in the ED and whether threat perceptions mediate associations between lack of health insurance and subsequent PSS. METHOD: Patients in the Columbia University Irving Medical Center ED with suspected ACS enrolled in an observational cohort stu

Health Insurance Coverage Among Adolescent and Young Adult Cancer Survivors Receiving Health Care before and During the COVID-19 Pandemic.

We investigated insurance coverage among adolescents and young adults (AYA) with cancer before and during the COVID-19 pandemic. AYAs diagnosed with cancer 15-39 years of age were identified using Utah Cancer Registry records and linked with University of Utah electronic health records. Poisson models calculated incidence rate ratios (IRRs) of health insurance coverage during pre-pandemic (11/4/2017-3/5/2020; n = 2,140) and pandemic (3/6/2020-7/6/2022; n = 1,894) periods. Prior to the pandemic, insurance gaps were higher (pre-pandemic = 16.40%, pandemic = 13.73%; IRR = 0.84, 95%CI = 0.71-0.98)

Bridging the gap between health insurance eligibility and uptake: A multistate quantitative analysis of entitlement-based verification patterns under India's PM-JAY Scheme.

Publicly-funded health insurance schemes, wherein governments subsidize insurance contributions for disadvantaged groups, are a key strategy under universal health coverage. India's Pradhan Mantri Jan Arogya Yojana (PM-JAY) uses an entitlement-based approach whereby approximately 700 million beneficiaries are automatically eligible for scheme benefits, subject to a simplified identity verification procedure instead of enrolment. This study investigates the determinants of undergoing verification among PM-JAY's target population to identify underserved groups and inform strategies to expand cov

Abnormally Low Hemoglobin A1c as Harbinger of Hemoglobinopathy.

The umbrella of employment-based health benefits is growing increasingly threadbare. As a result, health benefits are once again a major arena of labor-management strife, and once again calls for universal health care by many labor leaders mask important differences between them over health care reform. Some labor leaders advocate a bottom-up mobilization in support of a single-payer solution that would dismantle the system of job-based benefits rooted in private insurance. Others stake their health care strategy on wooing key business leaders to be constructive partners in some kind of unspec

Influence of insurance status and vaccine cost on physicians' administration of pneumococcal conjugate vaccine.

OBJECTIVE: In 2000, heptavalent pneumococcal conjugate vaccine (PCV7) was recommended for children younger than 2 years, but its high cost relative to other universally recommended childhood immunizations and variability in insurance coverage for the vaccine raised concerns. We investigated the influence of PCV7 cost and insurance coverage on physician recommendation of PCV7 to their patients and administration of PCV7 in their practices. METHODS: We conducted a mail survey from April to July 2001 of a random sample of 833 pediatricians and 788 family physicians in 24 states with different va

Correlation Between Technology and Improved Outcomes in Youth With Type 1 Diabetes Mellitus: Prospective Study Examining Outcomes for Patients With Depression and Those With Public Insurance.

BACKGROUND: Adherence to type 1 diabetes mellitus (T1DM) treatment regimens decreases during adolescence. While comorbid depression and health insurance disparities are individually known to potentiate this risk, technological devices for T1DM appear to be protective. OBJECTIVE: We examined whether technology use impacted the association between depression and poorer health outcomes in T1DM. Given established insurance-based disparities based on technology access, we also studied whether the protective effects of T1DM technology differed among publicly and privately insured youth. METHODS: D

A roadmap for kidney health for South Africa in the context of universal health coverage.

Chronic kidney disease (CKD) in South Africa (SA) is a growing public health crisis, driven by the intersecting burdens of HIV, tuberculosis, hypertension, diabetes and obesity. In Black African populations, high-risk apolipoprotein L1 (APOL1) variants add a genetic predisposition to non-diabetic kidney diseases, compounding risk. Global recognition of CKD has recently advanced, marked by the 2025 World Health Organization (WHO) non-communicable disease resolution, which urges the integration of kidney care into national health strategies. This resolution emphasises prevention, early detection

Financial Burden in Adults With Chronic Illness in Switzerland: A Secondary Analysis of Qualitative Interviews Using Natural Language Processing and Topic Modeling.

BACKGROUND: Chronic illness may cause a financial burden that affects patients, their caregivers, and families. While international research, mostly from the United States, has largely focused on cancer-related financial hardship, less is known about whether financial distress due to other chronic illnesses exists, specifically in countries that have universal health insurance coverage, such as Switzerland. OBJECTIVE: This study aims to provide insights into how financial burden is discussed by individuals living with chronic illness in Switzerland. METHODS: Based on a natural language proce

National Variation of Insurance Coverage for Gender-Affirming Facial Feminization Surgery.

Background: Gender-affirming surgery may be pursued by individuals experiencing gender dysphoria. Although genital and chest procedures are classified as medically necessary, facial feminization surgeries (FFSs) are often considered cosmetic. Insurance companies may limit coverage of these procedures, especially in states less supportive of transgender individuals. Objectives: To determine insurance coverage and ease of finding policy information for FFSs, and to analyze differences based on state advocacy. Methods: Insurance policies for the top three commercial health plans per state were re

Decolonizing global health in an age of fragmentation: reimagining equity for universal health coverage.

The global health landscape is undergoing a significant transformation as traditional frameworks of cooperation face fragmentation amid geopolitical tensions. Declining support from Western nations, exemplified by US withdrawal from the World Health Organization and cuts to programs like the President's Emergency Plan for AIDS Relief, has exposed the profound instability of an aid architecture built on colonial dependencies. The COVID-19 pandemic, marked by vaccine nationalism, was a stark litmus test of this systemic failure for low- and middle-income countries (LMICs). This commentary argues

Care-seeking and health insurance among pregnancy-related deaths: A population-based study in Jember District, East Java Province, Indonesia.

BACKGROUND: Despite the increased access to facility-based delivery in Indonesia, the country's maternal mortality remains unacceptably high. Reducing maternal mortality requires a good understanding of the care-seeking pathways for maternal complications, especially with the government moving toward universal health coverage. This study examined care-seeking practices and health insurance in instances of pregnancy-related deaths in Jember District, East Java, Indonesia. Case studies of health economic analyses informing pharmaceutical health technology assessments for essential medicine selection and public-sector guidelines in South Africa.

BACKGROUND: Constrained resources under universal health coverage (UHC) necessitate a balance between medication costs and essential health system requirements. Policymakers practice priority-setting, as either implicit or explicit rationing, embedded in evidence-informed decision-making processes to guide funding decisions. Health technology assessment (HTA) is a method that may assist explicit evidence-informed priority setting. South Africa developed an official HTA methods guide in 2022, however before this, commissioning and performing economic evaluations was not standardized. METHODS:
