Vitamin B12 levels in children and adolescents on plant-based diets: a systematic review and meta-analysis.

CONTEXT: The popularity of plant-based diets, characterized by a partial or complete exclusion of animal products, has increased significantly over the last 10 years. The exclusion of animal products removes the most common sources of vitamin B12, which can lead to vitamin B12 deficiency and result in irreversible damage, such as growth stunting. OBJECTIVE: This aim of this systematic review and meta-analysis was to qualitatively evaluate all studies on this subject and to quantify the potential difference in vitamin B12 levels in healthy children and adolescents aged 5 to 18 years. DATA SOU

Phytosterol-deficient and high-phytosterol diets developed for controlled feeding studies.

Phytosterols reduce cholesterol absorption and low-density lipoprotein cholesterol concentrations, but the quantity and physiological significance of phytosterols in common diets are generally unknown because nutrient databases do not contain comprehensive phytosterol data. The primary aim of this study was to design prototype phytosterol-deficient and high-phytosterol diets for use in controlled feeding studies of the influence of phytosterols on health. A second aim was to quantify the phytosterol content of these prototype diets and three other diets consumed in the United States. This stud

Risks of alternative nutrition in infancy: a case report of severe iodine and carnitine deficiency.

A 7.5-month-old infant with failure to thrive, developmental delay, muscular hypotonia, a visible goitre and severe osteopenia is described. Laboratory examination revealed a markedly increased serum TSH with low free T4, severe iodine and carnitine deficiency. The infant was breastfed until the age of 2.5 months and was then given a mixture of almond extract in water. The mother is a strict vegan and the father a lactovegetarian. The nutritional intake of the child was severely depleted in calories (-46%), calcium (-73%) and iodine (-88%). The restrictive alternative nutrition was responsible

[Vitamin B12 deficiency in the elderly].

The prevalence of vitamin B12 deficiency increases with age. Patients with dementia and spouses of patients with dementia are at special risk for the development of vitamin B12 deficiency. In a normal diet this vitamin is present only in animal source foods; therefore, vegans frequently develop vitamin B12 deficiency if not using supplements or foods fortified with cobalamin. Apart from dementia, most of these manifestations are completely reversible under correct therapy; therefore it is crucial to identify and to treat even atypical presentations of vitamin B12 deficiency as early as possibl

Impact of healthy and sustainable diets on the mortality burden from cardiometabolic diseases and colorectal cancer in Mexican adults: a modeling study.

BACKGROUND: Healthy and sustainable diets (HSDs) have been associated with reduced mortality from chronic diseases, particularly in high-income countries. However, evidence from Mexico is limited, and no study has assessed the potential impact on mortality of adopting different HSD scenarios. This study aimed to assess the impact of various HSD scenarios on the mortality burden from cardiometabolic diseases and colorectal cancer (CRC) in Mexican adults. METHODS: Using a comparative risk assessment model, we examined six HSD scenarios: Mexican Healthy and Sustainable Dietary Guidelines 2023 (MH

The Psychosocial Aspects of Vegetarian Diets: A Cross-Sectional Study of the Motivations, Risks, and Limitations in Daily Life.

INTRODUCTION: The popularity of vegetarian and vegan diets is linked to various motivations, such as health, ethics, ecology, and social and religious influence. India has the highest proportion of vegetarians and vegans. The practise of these diets is linked to moral and health reasons and environmental concerns. Vegetarianism may also be associated with eating disorders such as orthorexia (ON). AIM: The main aim of this study was to determine the psychosocial aspects of vegetarian diets. Understanding these aspects is crucial for identifying potential risks and developing effective interven

Creatine supplementation research fails to support the theoretical basis for an effect on cognition: Evidence from a systematic review.

Creatine supplementation has been put forward as a possible aid to cognition, particularly for vegans, vegetarians, the elderly, sleep deprived and hypoxic individuals. However, previous narrative reviews have only provided limited support for these claims. This is despite the fact that research has shown that creatine supplementation can induce increased brain concentrations of creatine, albeit to a limited extent. We carried out a systematic review to examine the current state of affairs. The review supported claims that creatine supplementation can increases brain creatine content but also

Vitamin B12: A Comprehensive Review of Natural vs Synthetic Forms of Consumption and Supplementation.

Vitamin B12, also known as cyanocobalamin, methylcobalamin, and adenosylcobalamin, is an essential vitamin that can be primarily obtained through the human diet. Although animal-based foods supply adequate B12 for most people, deficiency remains common in older adults, vegetarians and vegans, and in those with gastrointestinal disease or malabsorption. Deficiency can lead to megaloblastic anemia, neurologic symptoms, and pregnancy complications. In healthy individuals, supplementation raises serum B12 levels to a similar extent as dietary intake; however, in disease states such as megaloblasti

Unmasking Cerebral Venous Sinus Thrombosis Induced by Vitamin B12 Deficiency.

This is the case of a healthy, 45-year-old right-handed man with no past medical issues who presented with mild drowsiness, language difficulty, right-sided weakness, and headache. Vital signs were unremarkable, and on examination, he was drowsy and found to have expressive aphasia and right upper and lower extremity flaccid paralysis. The patient was found to have cerebral venous thrombosis on imaging, as a result of his vitamin B12 deficiency from a vegan diet. The purpose of this clinical reasoning case is to instruct readers on the localization of symptoms, key imaging findings, approach t

Plant-based diets, cognitive function, and Alzheimer's disease.

Background: Alzheimer's disease (AD) represents 60-70% of global dementia cases, with up to 45% attributed to modifiable risk factors. Plant-based diets (PBDs) are increasingly recognised for their potential benefits on the gut microbiome, cardiometabolic risk factors and prevention of neurodegenerative disorders. This narrative review synthesises PBDs association with cognitive decline and AD risk. Methods: This review aims to summarise evidence from observational and interventional studies examining vegan, vegetarian, pesco-vegetarian, semi-vegetarian dietary patterns, and PBD indices in rel

Is vegetarianism healthy for children?

According to the Academy of Nutrition and Dietetics' influential position statement on vegetarianism, meat and seafood can be replaced with milk, soy/legumes, and eggs without any negative effects in children. The United States Department of Agriculture endorses a similar view. The present paper argues that the Academy of Nutrition and Dietetics ignores or gives short shrift to direct and indirect evidence that vegetarianism may be associated with serious risks for brain and body development in fetuses and children. Regular supplementation with iron, zinc, and B12 will not mitigate all of thes

High frequency of low serum levels of vitamin 12 among patients attending Jordan University Hospital.

This study aimed to estimate the status of serum vitamin B12 level in patients attending Jordan University Hospital in Amman, and to examine the relationship with demographic data, chronic illness, dietary habits, haematological parameters and symptoms related to vitamin B12 levels. A total of 838 patients completed a questionnaire and gave blood samples; 44.6% were vitamin B12 deficient (< 180 pg/mL) and 34.2% had hypovitaminosis (180-300 pg/mL). Vitamin B12 deficiency was associated with memory impairment, low meat intake and strict vegetarian (vegan) diets. The high frequency of low vitamin

Multiple nutritional deficiencies in infants from a strict vegetarian community.

Severe nutritional deficiencies developed in four infants from a new vegan religious community. They had received breast milk until the age of 3 months; thereafter, breast milk was supplemented with or replaced by extremely low caloric-density preparations. All of the infants had profound protein-caloric malnutrition, severe rickets, osteoporosis, and vitamin B12 and other deficiencies. One infant died, while the three others had an uneventful recovery. After discharge of the infants from the hospital, the community responded well to a modification of the infants' diet, which did not violate t

Central retinal vein occlusion secondary to severe iron-deficiency anaemia resulting from a plant-based diet and menorrhagia: a case presentation.

In this case presentation, we present a young vegan patient who developed a CRVO secondary to severe iron-deficiency anaemia (IDA) attributable to menstrual losses and limited iron intake. CRVO is a rare complication of IDA.With rising calls for sustainable diets and rising evidence for a plant-based diet, there has been a rise in popularity of such diet forms. While there are ocular benefits from this diet trend, the potential for nutritional deficiencies including iron needs to be monitored especially in susceptible individuals. Iron is essential for retina metabolism and function; however,

Plant-Based Dietary Patterns and Cardiovascular Disease Risk in Australians: Protocol for a Cross-Sectional Study.

Plant-based diets (PBDs) emphasise higher intakes of plant foods and lower intakes of animal foods, and they have been associated with reduced cardiovascular morbidity/mortality and lower cardiovascular disease (CVD) risk factors. Evidence is limited regarding the dietary profile, diet quality, and nutritional adequacy of PBDs, including their impact on CVD risk compared with traditional meat-eating diets in Australians. The PBD Study (PBDS) is a cross-sectional study that will recruit 240 adults from the Hunter region (NSW) without known CVD who are habitually consuming vegan (no animal flesh

Peak bone mass, which is reached by the end of the second decade of life, is mainly determined by genetic factors. Among other factors influencing bone capital, dietary intakes, particularly calcium and protein, play a significant role in peak bone mass attainment. Both nutrients are provided in dairy products, which accounts for 50-60% and 20-30% of the daily calcium and protein intakes, respectively. Children avoiding dairy products are at higher

Liver Paradoxical Reaction Presenting as a Solitary Hepatic Mass During Antituberculosis Therapy: A Case Report.

A vegan Indian woman in her late 20s was diagnosed with pulmonary tuberculosis. Imaging revealed a solitary hepatic mass five months after the initiation of antituberculosis therapy. Contrast-enhanced ultrasonography cannot distinguish between inflammation and malignancy. A biopsy revealed epithelioid cell granulomas without caseous necrosis, while a smear, culture, and PCR were negative. The therapy was continued without modification, and the hepatic lesion regressed within 7 months, consistent with a hepatic paradoxical reaction. This case illustrates that a new solitary hepatic mass emergin
