BACKGROUND: Major trauma patients are at significant risk of developing pulmonary embolism (PE). We aimed to (i) analyse the current incidence and timing of PE after injury; (ii) identify risk factors that predispose major trauma patients to the occurrence of PE; and (iii) describe the use of vena cava filters (VCFs) following major trauma.
METHODS: Prospectively collected data from The Alfred Hospital's Trauma Registry were used to identify all major trauma patients admitted over a 7-year period. A multivariable logistic regression model was used to identify factors associated with the occur

BACKGROUND: Use of inferior vena cava filters (IVCFs) has become more prevalent for the prevention of venous thromboembolism in part due to their ease of deployment and retrieval. Nonthrombotic complications of IVCFs are unusual but have been described. This study characterizes this cohort of patients and elucidates their clinical outcome.
METHODS: Between January 1, 2006 and December 31, 2011, six patients were identified with nonthrombotic symptoms attributed to their IVCF. Symptoms included abdominal/back pain, hypertension from renal artery compression, and hydroureter from ureteral compr

Inferior vena cava (IVC) filters are frequently placed to prevent pulmonary embolism in patients in whom anticoagulation is contraindicated or ineffective. Delayed erosion of the filter into adjacent vital structures is a rare complication. We report 3 complications of IVC filters managed with both surgical and endovascular therapies. A review of the available literature addresses incidence of delayed IVC filter complications, the approach to these problems, and the role of retrievable IVC filters.

OBJECTIVES AND BACKGROUND: The build-up of thromboses in vena cava filters after deployment presents serious problem to the patients. We proposed a novel way to overcome this problem in a belief that intentionally induced swirling flow can optimize blood flow patterns in vena cava filters, enhance the stirring motion of flow, in turn accelerate the dissolution of blood clots captured in the filter and facilitate blood to flow pass through the filters.
METHODS: In this study, we experimentally compared the work efficiency of a vena cava filter under swirling flow condition with that of the sam

BACKGROUND: Several authors have showed that bedside insertion of inferior vena cava filters (IVCF) is feasible and cost effective, with the additional benefit of not having to transport a critically ill patient to the operating room or radiology department. The objective of this study was to examine our experience of 158 IVCF insertions at the bedside in the intensive care unit.
STUDY DESIGN: A prospective, observational study of bedside IVCF insertion performed by the authors from February 1996 through August 2000 was undertaken.
RESULTS: One hundred fifty-eight patients underwent bedside

We describe the case of a young woman who developed fatal pulmonary embolism during thrombolytic therapy of a deep pelvic and leg vein thrombosis, despite the insertion of a temporary vena cava filter. So the opinion that the insertion of inferior vena cava filters always prevents lethal pulmonary embolism caused by thrombi of the deep vein system must be revised.

BACKGROUND: Injured patients are at significant risk for venous thromboembolic complications. Multiple studies have reported a benefit of prophylactic inferior vena cava filter (IVCF) insertion in selected high-risk trauma patients. Often, these high-risk patients reside in the intensive care unit (ICU) and require mechanical ventilation, intracranial pressure monitoring, multiple intravenous infusions, and other invasive monitoring modalities. This puts these patients at risk for transport from the ICU.
METHODS: We prospectively studied a series of consecutive patients undergoing bedside pre

The objective of this study was to determine the feasibility, cost-effectiveness, and complications of bedside placement of inferior vena cava (IVC) filters in the intensive care unit (ICU) in the trauma patient. A prospective trial involving 25 trauma patients admitted to Memorial Regional Hospital (Hollywood, Florida), a Level I trauma center, from April 1997 to April 1998, meeting the criteria for insertion of a prophylactic IVC filter according to Eastern Association for the Surgery of Trauma trauma practice guidelines was conducted. IVC filters were placed in the ICU with the use of a dig

Diagnostic and therapeutic indications have increased the need for ready access to the inferior vena cava. Multiple anomalies of the inferior vena cava have been described. Correct placement of a Greenfield filter in a left-sided vena cava requires recognition of this anomaly. Placement through a right internal jugular vein is preferable; however, if this is not possible, the left femoral vein should be used. Because of the incidence of anomalous inferior vena cava, routine inferior venacavography is recommended when an intracaval device is to be placed.

The purpose of this study was to report the prevalence of thrombus within a retrievable vena cava filter inserted prophylactically in major trauma patients referred for filter extraction. Between November 2002 and August 2005, 80 retrievable inferior vena cava filters (68 Optease and 12 Gunther-Tulip) were inserted into critically injured trauma patients (mean injury severity score 33.5). The filters were inserted within 1 to 6 (mean 2) days of injury. Thirty-seven patients were referred for filter removal (32 with Optease and 5 with Gunther-Tulip). The indwelling time was 7 to 22 (mean 13) da

Catheter ablation with pulmonary vein isolation is a well-established treatment for symptomatic atrial fibrillation, particularly in patients intolerant to antiarrhythmic therapy. However, the presence of an inferior vena cava (IVC) filter represents a technical challenge for standard transfemoral access. We report the case of a 65-year-old woman with a history of recurrent deep vein thrombosis and a permanent Greenfield IVC filter implanted approximately 15 years prior, who presented with symptomatic paroxysmal atrial fibrillation and intolerance to multiple antiarrhythmic medications. Pulmon

Inferior vena cava filters are medical devices used as an alternative method in the prevention of pulmonary thromboembolism in patients with deep vein thrombosis. Though generally regarded as safe alternatives, these devices do carry a risk of associated complications. Presented is a case of a 32-year-old female who developed a duodenal perforation after undergoing the removal of a retrievable inferior vena cava (IVC) filter. She required placement of the IVC filter during her second pregnancy due to multiple thrombotic events. The patient underwent nonoperative conservative therapy and ultima

BACKGROUND: This article describes a rare case of inferior vena cava (IVC) filter perforation into the duodenum in a patient presenting with abdominal pain.
CASE REPORT: A 55 year old woman presented with abdominal pain four years after an IVC filter placement. Workup demonstrated an IVC filter strut perforating the duodenum. The filter was removed via laparotomy, the duodenum was closed primarily, and the IVC was repaired. The patient was discharged home on post-operative day five and is doing well.
CONCLUSIONS: Most extraluminal perforations of IVC filter struts are asymptomatic. Rare filt
