Sudden balance disorders with violent vegetative symptoms (nausea and vomiting) pose a diagnostic and therapeutic problem. In children vertigo/dizziness with symptoms of vestibular dysfunction is rare, but as vascular etiology is unlikely in children such symptoms arouse concern. This article presents two cases of this type of vertigo. The patients were two boys (6 and 9 years old). They came down with similar symptoms: sudden dizziness, disabled walking, nausea and vomiting, spontaneous nystagmus, and a positive Romberg test. The onset of the balance disorder was preceded by respiratory infec

BACKGROUND: Measurement of ocular counterrolling (OCR) is well known as a simple method to evaluate the peripheral vestibular organ, especially the otolith organ. But it has rarely been used because of the difficulty in differentiating between pathologically reduced OCR and the wide physiological variation of this parameter.
PATIENTS: In this study, the OCR of 55 patients with unclarified vertigo (n = 20), vestibular neuronitis (n = 15), Menière's disease (n = 10), and benign paroxysmal positional nystagmus (n = 10) were evaluated and compared to a control group (n = 30) with the intention of

Histologic sections of the human temporal bone display snapshots of the entire lifetime integrated into the moment the bone enters fixative. The bulk of the literature on vestibular histopathology is anecdotal and descriptive in nature, rather than quantitative. This is because the means of describing and measuring patients' vestibular symptoms are poorly developed, and the complex geometry of the vestibular labyrinth complicates efforts to study it in serial histologic sections. Histopathologic findings in the common peripheral vestibulopathies, including Meniere's syndrome, benign paroxysmal

The aim of this study was to assess the relationship between the results of the objective assessment of the vestibular organ in patients with peripheral vertigo with Hashimoto's thyroiditis and thyroid hormone metabolism. Twenty eight women with Hashimoto's thyroiditis and chronic vertigo were enrolled in the study. Patients underwent audiological assessment of hearing, Dix-Hallpike test, videonystagmography with caloric test, head impulse test (HIT) and cervical vestibular-evoked myogenic potentials (cVEMPs). Levels of thyroid hormones and anti-thyroid antibodies were determined. Relationship

Otolaryngologists typically perform diagnoses and offer medical treatment for vestibular dysfunction. This vestibular dysfunction manifests as benign paroxysmal positional vertigo (BPPV), Ménière's disease, vestibular neuronitis, and so on. The etiology of BPPV is still not clear, so in this article we discuss inner-ear function, etiology, and factors related to BPPV. We examined by pure-tone audiometry and hot and cold caloric tests patients whom we identified as having diagnosed posterior canal-type BPPV. We observed canal paresis at a high rate on the affected side (p < .01). The term of re

The symptom of vertigo is usually managed in primary care without further referral. This review examines the evidence on which general practitioners can base clinical diagnosis and management of this relatively common complaint. Research in this area has in the main been from secondary and tertiary centres and has been of variable quality. Indications are that the conditions that present in general practice are most likely to be benign positional vertigo, acute vestibular neuronitis, and Ménière's disease; however, vascular incidents and neurological causes, such as multiple sclerosis, must be

Based on two cases and a review of the recent literature, we would like to draw attention to the fact that a cerebellar infarction, localised to the territory of the PICA, can present with just a single clinical feature-acute rotary vertigo. Cerebellar vascular accidents remain a little known cause of acute vertigo, often masquerading as a peripheral vestibular disturbance. The main differential diagnosis is vestibular neuronitis. The prognosis is usually good, but the risk of a late recurrence or of a further cerebro-vascular episode is difficult to predict. This uncommon aetiology should not

PURPOSE OF REVIEW: To review the latest developments in the surgical management of common peripheral vestibular disorders.
RECENT FINDINGS: Although surgery is indicated mainly for patients with Meniere's disease, other less common disorders such as benign paroxysmal positional vertigo, semicircular canal dehiscence, perilymphatic fistulae, fistulae of semicircular canals, VIII nerve vascular compression, chronic vestibular neuronitis, and dizziness following sudden sensorineural hearing loss may require surgical intervention when conservative management has failed.

Fixation nystagmus suppression in caloric tests was used in examination of 56 patients suffering from chronic remittent labyrinthopathy, acute labyrinthopathy, vestibular neuronitis, otosclerosis, adhesive otitis media, vertebrobasilar insufficiency, motor disease, chronic remittent vestibulopathy and neurinoma of the VIII nerve. The bithermal test (BT) with electronystagmography was made to calculate the fixation suppression index (FSI). Complete suppression of the caloric nystagmus in at least one BT test appeared to be the most frequent variant of the response (24 cases). This was not depen

Although vertigo is commonly encountered in many clinical disciplines, meticulous analysis of the onset and nature of the vertigo is essential not to overlook the vertigo of brainstem or cerebral origin without any other neurological signs and symptoms. Three central vestibular projection pathways and their possible functional roles in relation to vertigo were briefly discussed. To illustrate the characteristic clinical features of vertigo of various origins, 16 typical cases were presented and discussed. Their etiology include Ménière's disease, vertigo of neurovascular compression, sudden de

Benign paroxysmal positional vertigo, vestibular neuronitis and Menière's disease cause most cases of acute vertigo. However, doctors must consider central neurological reasons to vertigo. If it is determined that a patient has oto-neurological vertigo, the next task is to determine whether the patient has a peripheral or a central cause of vertigo, if the condition is potentially lethal and if there is a need for acute radiological imaging and/or medical intervention. This review highlights the oto-neurological approach to the dizzy patient with particular focus on the patient's history, clin

High-resolution CT of the petrous bone was carried out in 259 patients with clinically diagnosed abnormalities of the inner ear; in some the lesions were bilateral. In 62 patients (approximately 20%), the scans showed findings which related to the signs and symptoms. Patients with hearing loss and vestibular neuronitis showed no abnormalities. Amongst the remaining patients, the most common findings were acoustic neuromas (17 cases), cochlear otospongiosis (14 cases), malformations (13 cases) and tumours or cholesteatomas of the labyrinth (13 cases) as well as single rare isolated lesions.

In patients with various otoneurological diseases like hearing loss, neuronitis vestibularis, Ménière's disease and Bell's palsy, analyses concerning the immunoregulation and immunogenetics were done. For analysing the immunoregulation the T-helper (CD4) T-suppressor (CD8) ratio was determined. In contrast to patients with hearing loss caused by otobasal fractures and a healty control group, this ratio was elevated in 50% of the patients suffering from hearing loss. The elevation of the CD4/CD8 ratio was mainly caused by a reduction of CD8 positive cytotoxic-suppressor T-lymphocytes. The CD4/C

It is well known that diseases of the vestibular system can be compensated by increased spontaneous activity of other systems engaged in maintaining equilibrium, i.e. proprioceptive and visual systems. A complex approach using multisensory stimulation is the optimal way to achieve vestibular compensation. The aim of our study was to determine the effect of vestibular rehabilitation therapy as measured by posturography in a group of 72 patients suffering from vestibular disorders: Ménière's disease (n = 31), neuritis vestibularis (n = 21) and vertebrobasilar insufficiency (n = 20). OBJECTIVE: In a previous publication, the authors documented the prevalence and characteristics of head-shaking nystagmus (HSN) in over 1300 consecutive dizzy patients during formal electronystagmography (ENG) at the combined Otologic Function Unit (OFU) of the Toronto and Mount Sinai Hospitals. This companion study was done to prospectively analyze a subgroup of 300 patients with known clinical diagnoses belonging to the senior author (J.R.).
METHODS: The prevalence of HSN in this series of patients with a known diagnosis is prospectively compared to the presence of HSN in a series of normal

Vestibular compensation, or neuronal plasticity in the central vestibular system, is quite an important process in patients with acute unilateral peripheral vestibular disease, allowing them to lead a comfortable daily life when medical treatments fail to cure the peripheral vestibular function. Is the residual unilateral vestibular input from damaged vestibular endo-organs a positive or negative factor for the development of dynamic vestibular compensation in the central nervous system? To elucidate the true mechanism of vestibular compensation, we examined the ENG findings and dizziness hand

In this retrospective study it was found that 549 out of 1175 patients seen for vestibular assessment within an 8-year period within a district hospital service showed lateralised canal paresis as determined by caloric testing. It was found that there was a tendency for canal paresis, indicative of vestibular hypofunction, to be on the side of handedness. This bias showed statistical significance, (chi-squared, p < .005), for the patients overall, as well as for the 65 left-handers considered separately, (p < .05). Surprisingly, this form of bias does not appear to have been reported previousl

Benign paroxysmal positional vertigo (BPPV) has received renewed clinical interest. At the Johns Hopkins Otological Vestibular Laboratory, 781 patients were tested on more than one occasion between September 1976 and November 1992. Of these patients, 187 (24%) were found to have positional nystagmus consistent with a diagnosis of BPPV. A retrospective analysis of these patients' records, including vestibular test and eye movement reports, audiograms, questionnaires, and hospital charts was performed. After this review, the nystagmus, initially diagnosed as due to BPPV, was believed to be a man

OBJECTIVE: To identify etiology, clinical symptoms of bilateral vestibulopathy (BV) depending on etiology.
RESULTS: Idiopathic BV was identifies in one third of patients. The most common identified reasons are Meniere's disease, meningitis and ototoxicity. BV develops more often without incidental hearing loss in case of idiopathic, autoimmune etiology, ototoxicity and bilateral vestibular neuronitis. BV after neuroinfection is characterized by prompt symptoms development, bilateral severe hearing loss, Gain reduction in vHIT lower 0.4 and loss of more than 8 lines in DVA test.

The diagnosis of vestibulocochlear nerve and statoacoustic organ lesions is based on the symptoms vertigo, hearing loss and tinnitus. The most common diseases are vestibular neuronitis, Ménière's disease, benign positional vertigo and sudden hearing loss. 6 patients are presented, complaining of at least one of the symptoms vertigo, hearing loss or tinnitus. They were suspected of suffering from a peripheral audiovestibular disease when admitted to the clinic, but all of them revealed other pathologies.
