Localized 1H NMR measurements of 2-pyrrolidinone in human brain in vivo.

Localized 1H NMR homonuclear J editing spectroscopy was used to measure the concentration of 2-pyrrolidinone (PRDN) in the human occipital lobe of five normal and six epileptic subjects taking vigabatrin. PRDN is a lactam cyclization product of gamma-aminobutyric acid (GABA). From a localized volume of 13.5 cm3 in the occipital cortex, the concentration of PRDN ranged from 0.2 to 0.3 micromol/g in normal subjects, whereas in epileptic subjects on vigabatrin PRDN was elevated to 0.6 +/- 0.1 micromol/g. The elevated PRDN in patients on vigabatrin was in accord with raised GABA levels compared wi

Treatment of infantile spasms in tuberous sclerosis complex: dismal outcomes but future hope?

This commentary discusses a retrospective study by Muzykewicz et al. that details EEG results from children with infantile spasms in the setting of tuberous sclerosis complex (TSC). In this study, several factors were identified as being predictive of poor cognitive outcome, including worsening degree of hypsarrhythmia, abnormalities in EEG background activity, and incomplete response to vigabatrin. Notably, the majority of patients had a poor outcome, experiencing cognitive impairment and intractable epilepsy irrespective of whether they were treated with adrenocorticotropic hormone (corticot

Seizure-induced apnea causes sudden death independent of cortical spike wave discharge in mouse models of Tuberous Sclerosis Complex.

Sudden unexpected death in epilepsy (SUDEP) is the leading cause of seizure-related mortality for people with epilepsy (PWE). Current evidence suggests that many cases of SUDEP are due to seizures that produce respiratory arrest, aka seizure-induced apnea. Patients with tuberous sclerosis complex (TSC) are at a significant risk of SUDEP, and mouse models of TSC have early mortality. Previous reports in TSC mouse models demonstrate that although Vigabatrin suppresses seizures, it does not improve survival. In other mouse models, our team has demonstrated that seizure-induced apnea occurs during

Epidemiology and outcome of infantile spasms in Denmark in 1996-2019.

PURPOSE: To investigate the treatment of infantile epileptic spasm syndrome (IESS) in Denmark. METHODS: National retrospective cohort study of all patients born 1996-2019 who had a diagnosis of IESS in the National Patient Registry. Medical records were reviewed to evaluate the diagnosis. Patients were included if semiology was compatible with IESS, or if unclear semiology if there was an abnormal EEG or EEG with hypsarrhythmia. RESULTS: Number of cases with a register based IESS diagnosis was 538. Medical records were unavailable in 48 and 164 did not fulfil the inclusion criteria. Thereby

Treatment efficacy for infantile epileptic spasms syndrome in children with trisomy 21.

BACKGROUND: Infantile Epileptic Spasms Syndrome (IESS) is the most common epilepsy syndrome in children with trisomy 21. First-line standard treatments for IESS include adrenocorticotropic hormone (ACTH), oral corticosteroids, and vigabatrin. Among children with trisomy 21 and IESS, treatment with ACTH or oral corticosteroids may yield higher response rates compared with vigabatrin. However, supporting data are largely from single-center, retrospective cohort studies. METHODS: Leveraging the multi-center, prospective National Infantile Spasms Consortium (NISC) database, we evaluated the effic

[Proposals for the treatment of children with West's syndrome].

OBJECTIVE: The justification of guidelines for the treatment of children with infantile spasms and West's syndrome. DEVELOPMENT: We review, by means of prospective studies of suitable methodology, the efficacy and side-effects of adrenocorticotropin, sodium valproate, vigabatrin and other drugs used in the treatment of children with West's syndrome. CONCLUSIONS: In view of the information obtained from these studies, we suggest that in children with idiopathic or cryptogenic West's syndrome, treatment should be started with oral prednisone up to a dose of 8 mg/kg/day which should be changed

[Concentric changes in the visual field associated with GABA-mimetic antiepileptic agents].

Bilateral visual field constriction has been recently reported in patients treated with vigabatrin. It has been considered that vigabatrin, a GABA agonist antiepileptic drug, was specifically responsible for this visual field defect. We present four observations sharing the same characteristics of chronic tunnel vision. Three patients had had vigabatrin but the fourth one received other antiepileptic drugs, progabide, an agonist of post-synaptic GABA receptors, and phenobarbital which interferes with GABA-A receptors. It is thus possible to hypothesize a retinal toxicity triggered by chronical

Epilepsy in children with tuberous sclerosis complex: Chance of remission and response to antiepileptic drugs.

OBJECTIVE: To describe treatment and outcome of epilepsy in children with tuberous sclerosis complex (TSC). METHODS: Seventy-one children with TSC and epilepsy treated at the ENCORE TSC Expertise Center between 1988 and 2014 were included. Patient characteristics and duration and effectiveness of antiepileptic treatments were extracted from our clinical database. Correlations were made between recurrence of seizures after response to treatment, and several patient characteristics. RESULTS: Median age at time of inclusion was 9.4 years (range 0.9-18.0). Seizure history showed that 55 children

Rationalized polytherapy for epilepsy.

In the past, epilepsy was usually treated with polytherapy, but with little knowledge as to the interactions and side-effects of the combinations of the anti-epileptic drugs used. Adverse events and sparse clinical knowledge led to monotherapy becoming the treatment regime of choice. A new generation of drugs, which are well-tolerated and have few or predictable interactions, have enabled the reassessment of polytherapy for the treatment of epilepsy. Extensive clinical trials of these drugs are allowing the emergence of a new, rationalized approach to polytherapy. In our study, 19 patients wit

Autistic regression associated with seizure onset in an infant with tuberous sclerosis.

We report here on a male diagnosed with tuberous sclerosis at 6 months of age. The child was treated with vigabatrin at age 6 months after an abnormal electroencephalogram but before onset of seizures. Vigabatrin was discontinued at age 13 months to avoid possible visual field defects. At 21 months, the child developed partial seizures with secondary generalization and infantile spasms. Standardized developmental assessments were performed at 12, 18, 24, 30, and 36 months of age. Cognitive and social development were normal until age 21 months and the onset of seizures. When assessed at 24 mon

Retinal defect in children with infantile spasms of varying etiologies: An observational study.

OBJECTIVE: To determine the prevalence of retinal defect in children with infantile spasms (IS) unrelated to treatment with vigabatrin and clarify if specific primary etiologies for IS are associated with retinal defect more than others. METHODS: This was an observational cohort study including 312 patients (176 male, 136 female) with IS who were vigabatrin-naive. Participants ranged from 1.7 to 34.7 months of age (mean 8.8 months). Electroretinograms (ERGs) were performed according to the International Society for Clinical Electrophysiology of Vision. Retinal defect was identified as abnorma

The latest on infantile spasms.

PURPOSE OF REVIEW: This article reviews the most significant advances in the field of infantile spasm during the past year, with emphasis on best practise for treatment, and on some new etiological genetic and metabolic causes for the spasms, and new advances in the knowledge of tuberous sclerosis. RECENT FINDINGS: Up-to-date information comparing corticotrophin, oral steroids and vigabatrin shows that hormonal treatment is the most effective therapy in the short term. In a recent randomized trial, large doses of prednisolone were as effective as corticotrophin. There are insufficient data to

Medication selection, health services outcomes, and cost trajectories for Medicaid beneficiaries with infantile spasms.

OBJECTIVE: There are three recommended first-line treatments for infantile spasms, adrenocorticotropic hormone (ACTH), oral corticosteroids, and vigabatrin, though non-standard treatments such as topiramate are sometimes selected. Is it uncertain how treatment selection influences health services outcomes. METHODS: We conducted a retrospective cohort study of Medicaid beneficiaries newly diagnosed with infantile spasms from 2009-2010. We included infants with a new diagnosis of infantile spasms between age 2-9 months who filled ACTH (reference), prednisolone, vigabatrin, or topiramate prescri

Long-term neurological outcome in children with early-onset epilepsy associated with tuberous sclerosis.

In tuberous sclerosis complex, early seizure onset is associated with high risk of intractable epilepsy and cognitive/behavioral impairment. We retrospectively evaluated the long-term outcome of 44 infants presenting with seizures in the first 12 months who received vigabatrin, and were followed up for at least 3.5 years. At the final evaluation 55% of patients were still having seizures, 80% had intellectual disability, and 30% had autism. Sixty-five percent of children who had been treated earlier with vigabatrin after seizure onset achieved seizure freedom, compared with 24% of subjects who

The new antiepileptic drugs: pharmacological and clinical aspects.

In recent years several new drugs (oxcarbazepine, lamotrigine, topiramate, gabapentin, zonisamide, tiagabine, fosphenytoin, vigabatrin and felbamate) have been added to the therapeutic armamentarium against epilepsy. Some of these represent structural modifications of pre-existing compounds, others were developed with the specific objective of modifying neurotransmitter function, and many more were found to be clinically useful even though their mode of action is unclear or differs from that originally planned. The pharmacokinetics of these drugs differ widely from one agent to another. Some (

[Visual field defect in a patient given sodium valporate then carbamazepine: possible effect of aminotransferase inhibition].

We report the case of a 25-years old woman with anti-epileptic drugs who presents a visual field defect similar to those described with vigabatrin even though she was successfully treated with valproic acid then carbamazepine without vigabatrin. The association with trichorrhexis nodosa, a hair disease sometimes associated with inherited perturbation of metabolism of urea cycle in which visual loss can occur, could suggest an aspecific inhibition of several aminotransferases which could explain different adverse effects of some anti-epileptic drugs (visual abnormalities, alopecia) perhaps in g

[West syndrome--new therapeutic approach].

West syndrome (WS) is an age dependent epileptic syndrome caused by various brain disorders. WS has been frequently classified in two groups, cryptogenic and symptomatic. As symptomatic WS consists of patients with numerous types of brain lesions, the prognosis and evolutional changes may be different among the types of brain lesions. WS is resistant to treatment to most conventional antiepileptic drugs, and only valproic acid, benzodiazepines, adrenocorticotropic hormone (ACTH), corticosteroids and vigabatrin have been found efficacious. Benzodiazepine, notably nitrazepam, and less clonazepam
