Management strategies for burning mouth syndrome: a comprehensive review.

Burning Mouth Syndrome (BMS) is a complex chronic neuropathic orofacial pain disorder characterized by a persistent burning or dysesthetic sensation in the oral cavity without an identifiable organic cause. The management of BMS has evolved beyond symptom relief to focus on achieving full functional recovery (FFR), which encompasses restoring patients to their usual activities without restrictions, addressing both physical and psychological dimensions. Key pharmacological treatments such as clonazepam and capsaicin are explored in detail, alongside the potential of newer agents like various cl

Comparative efficacy and acceptability of anxiolytic drugs for the treatment of anxiety disorders: a systematic review and network meta-analysis.

BACKGROUND: Anti-anxiety medications' side effects and dependency risks necessitate thorough consideration of their efficacy and acceptability when making treatment decisions. METHODS: A systematic review and Network Meta-Analysis (NMA) was conducted using three databases from 1980 to 2020, focusing on adults diagnosed with generalized anxiety disorder (GAD) or any relevant diagnostic classification of anxiety disorder or those for whom Hamilton Anxiety (HAM-A) data is available, and including any comparator (placebo and/or active comparator treatments). The primary outcomes were efficacy (me

SSRIs in the course of COVID-19 pneumonia: Evidence of effectiveness of antidepressants on acute inflammation. A retrospective study.

INTRODUCTION: Relationships between inflammation and mood have been observed in terms of pro-inflammatory effects induced by depressive conditions and, in parallel, by an antidepressant-induced favorable effect on the recovery of inflammatory states. Selective serotonin reuptake inhibitor (SSRI) drugs were hypothesized to improve the prognosis of COVID-19 pneumonia, a typical acute inflammation, in terms of decreased mortality rate and pro-inflammatory cytokine serum levels. METHODS: The medical records of COVID-19 pneumonia inpatients at Careggi University Hospital (Florence) were analyzed f

Long-Term Pharmacological Treatments of Anxiety Disorders: An Updated Systematic Review.

Many aspects of long-term pharmacological treatments for anxiety disorders (AnxDs) are still debated. We undertook an updated systematic review of long-term pharmacological studies on panic disorder (PD), generalized anxiety disorder (GAD), and social anxiety disorder (SAD). Relevant studies dating from January 1, 2012 to August 31, 2015 were identified using the PubMed database and a review of bibliographies. Of 372 records identified in the search, five studies on PD and 15 on GAD were included in the review. No studies on SAD were found. Our review confirms the usefulness of long-term pharm

The Safety, Tolerability and Risks Associated with the Use of Newer Generation Antidepressant Drugs: A Critical Review of the Literature.

Newer generation antidepressant drugs (ADs) are widely used as the first line of treatment for major depressive disorders and are considered to be safer than tricyclic agents. In this critical review, we evaluated the literature on adverse events, tolerability and safety of selective serotonin reuptake inhibitors, serotonin noradrenaline reuptake inhibitors, bupropion, mirtazapine, trazodone, agomelatine, vilazodone, levomilnacipran and vortioxetine. Several side effects are transient and may disappear after a few weeks following treatment initiation, but potentially serious adverse events may

A comprehensive review and meta-analysis of neurological side effects related to second-generation antidepressants in individuals with major depressive disorder.

Second-generation antidepressants (SGADs) often cause neurological side effects (SEs). This meta-analysis seeks to quantify the short-term rates of neurological SEs related to routinely used second-generation antidepressants used to treat major depressive disorder (MDD). A search of the PubMed, EMBASE,Cochrane Library databases and Web of Science was done to uncover double-blind, randomized, placebo-controlled studies evaluating the effectiveness of frequently used SGADs medicines in people with MDD. Qualifying studies were required to concentrate on the use of SGADs routinely used in MDD and

Sigma-1-targeting multimodal compound HBK-15 reverses memory deficits and restores hippocampal plasticity under NMDA hypofunction.

Memory impairment is among the most disabling features of depression and schizophrenia, yet remains largely untreated by available pharmacotherapies. NMDA receptor hypofunction is strongly implicated in these deficits, while sigma-1 receptors, by stabilizing calcium signaling and supporting glutamatergic plasticity, have emerged as a promising therapeutic target. HBK-15, a methoxyphenylpiperazine derivative with a multimodal receptor profile, had previously shown preliminary anti-amnesic activity in rodents, prompting us to test its efficacy under NMDA receptor hypofunction. We therefore inves

Fine-tuning 5-HT7 receptor selectivity, inverse agonism, and metabolic stability of new (aryloxy)ethyl-piperidines toward antidepressant and pro-cognitive properties.

Affective disorders, the leading causes of disability and premature death worldwide, require new and effective treatment strategies. Clinically used antidepressants and second-generation antipsychotic drugs, including vortioxetine and lurasidone, act as potent 5-HT7 receptor antagonists and improve cognitive functions in the patients with mood disorders. Additionally, 5-HT7 receptor-mediated activation of matrix metalloproteinase 9 (MMP-9) induces depressive-like behavior in mice. We designed and synthesized a series of 27 arylsulfonamide derivatives of 2-[(2-aryl/2-heteroaryl)phenoxy]ethyl-pi

Update Lessons from PET Imaging Part II: A Systematic Critical Review on Therapeutic Plasma Concentrations of Antidepressants.

BACKGROUND: Compared with antipsychotics, the relationship between antidepressant blood (plasma or serum) concentrations and target engagement is less well-established. METHODS: We have discussed the literature on the relationship between plasma concentrations of antidepressant drugs and their target occupancy. Antidepressants reviewed in this work are citalopram, escitalopram, fluoxetine, fluvoxamine, paroxetine, sertraline, venlafaxine, duloxetine, milnacipran, tricyclic antidepressants (amitriptyline, nortriptyline, and clomipramine), bupropion, tranylcypromine, moclobemide, and vortioxeti

Psychological and Psychopharmacological Interventions in Psychocardiology.

Patients with mental disorders have an increased risk to develop cardiovascular disease (CVD), and CVD are frequently comorbid with especially adjustment, anxiety and depressive disorders. Therefore, clinicians need to be aware of effective and safe psychological and pharmacological treatment strategies for patients with comorbid CVD and mental disorders. Cognitive behavioral therapy and third-wave of cognitive-behavioral therapy are effective for patients with CVD and mental disorders. Internet-based psychological treatments may also be considered. In more severe cases, psychopharmacological

Drug-drug Interactions between COVID-19 Treatments and Antidepressants, Mood Stabilizers/Anticonvulsants, and Benzodiazepines: Integrated Evidence from 3 Databases.

INTRODUCTION: The SARS-CoV-2 pandemic with psychiatric comorbidities leads to a scenario in which the use of psychotropic drugs may be required. This requires the support of evidence-based medicine to take into account possible interactions between antidepressants, mood stabilizers, benzodiazepines, and coronavirus infection treatments. METHODS: Three databases were consulted: (a) Lexicomp Drug Interactions, (b) Micromedex Solutions Drugs Interactions, (c)Liverpool Drug Interaction Group for COVID-19 therapies. Treating co-occurring alcohol use disorder and major depressive disorder: A systematic review of combination and multimodal pharmacotherapy.

BACKGROUND AND OBJECTIVES: Major depressive disorder (MDD) and alcohol use disorder (AUD) frequently co-occur and are associated with greater symptom severity and elevated suicide risk relative to either disorder alone, yet no pharmacological regimen is approved for the combined indication. This review systematically catalogs trials of combined and multimodal pharmacotherapy for comorbid AUD and MDD. METHODS: We searched PubMed/MEDLINE, Web of Science, and Google Scholar from inception through May 2024 for randomized or prospective open-label trials of combined pharmacotherapy, or single agen

Comparative efficacy of interventions for transdiagnostic anhedonia: a network meta-analysis.

Anhedonia is a transdiagnostic disturbance of reward processing, yet the comparative efficacy and certainty of available interventions remain unclear. We searched PubMed, Embase, Web of Science, and MEDLINE for controlled trials reporting quantitative anhedonia outcomes. The protocol was registered in PROSPERO under CRD42025630784. Random-effects frequentist network meta-analyses were conducted overall and by population, including major depressive disorder, schizophrenia, and healthy individuals with trait anhedonia. Randomized trials were assessed with the Cochrane Risk of Bias 2 (RoB 2) tool

Sequenced treatment alternatives to relieve adolescent depression: A pragmatic clinical trial.

BACKGROUND: Adolescent major depressive disorder (MDD) is a widespread mental health condition for which treatment outcomes remain suboptimal. For adolescents, it remains unclear whether fluoxetine monotherapy or fluoxetine combined with cognitive-behavioral therapy (CBT) is more effective as an initial treatment. Additionally, no research has compared augmentation versus switching strategies after initial fluoxetine failure. METHOD: We conducted a multistep, multicenter, pragmatical clinical trial with a partially randomized design. In step 1, patients had the opportunity to choose treatment

Memantine as an Adjunctive Treatment for Residual Anxiety and Agitation in Early Parkinson's Disease With Depression: A Case Report.

BACKGROUND: Non-motor psychiatric symptoms, including anxiety, agitation, and irritability, are common in Parkinson's disease (PD) and may persist despite antidepressant treatment. While monoaminergic dysfunction has traditionally been emphasized, accumulating evidence suggests that glutamatergic dysregulation, particularly involving N-methyl-D-aspartate (NMDA) receptors, may contribute to these symptoms. However, clinical evidence supporting NMDA receptor modulation in early-stage PD without dementia remains limited. CASE PRESENTATION: We report the case of a 65-year-old woman with early-sta

A review of the TGF-β1 pathway in Alzheimer's disease and depression: Possible restoration potential of antidepressants.

Alzheimer's Disease (AD), the most common type of dementia, is increasing rapidly in prevalence, while Major Depressive Disorder (MDD), the most frequent psychiatric condition, continues to pose significant global healthcare challenges. Recent hypotheses suggest that AD and MDD might be interrelated, potentially sharing common pathophysiological mechanisms. One possible central link is the Transforming Growth Factor-beta 1 (TGF-β1) pathway. Reduced TGF-β1 levels, a cytokine involved in inflammatory responses, have been observed in both conditions. In AD, diminished TGF-β1 might contribute to n

DReAmocracy: A Method to Capitalise on Prior Drug Discovery Efforts to Highlight Candidate Drugs for Repurposing.

In the area of drug research, several computational drug repurposing studies have highlighted candidate repurposed drugs, as well as clinical trial studies that have tested/are testing drugs in different phases. To the best of our knowledge, the aggregation of the proposed lists of drugs by previous studies has not been extensively exploited towards generating a dynamic reference matrix with enhanced resolution. To fill this knowledge gap, we performed weight-modulated majority voting of the modes of action, initial indications and targeted pathways of the drugs in a well-known repository, nam

Sexsomnia in a Young Man: A Non-REM Parasomnia With Diagnostic and Therapeutic Challenges.

BACKGROUND Sexsomnia, a subtype of non-REM (NREM) parasomnia, consists of atypical sexual behaviors during sleep and presents with diagnostic and therapeutic challenges due to its under-recognition and overlap with other sleep and psychiatric disorders. Thus, its early diagnosis is quite challenging and important. CASE REPORT A 29-year-old man presented with a 2-year history of abnormal nocturnal sexual behaviors including pelvic thrusting, masturbation, and attempted intercourse during sleep reported by his partner, consistent with sexsomnia. All episodes occurred with complete amnesia. The s

The Cardiovascular Effects of Newer Antidepressants in Older Adults and Those With or At High Risk for Cardiovascular Diseases.

Depression is common in older adults and those with cardiovascular disease. Although selective serotonin reuptake inhibitors generally have been shown to be safe to treat depression in these patients, it is important to identify additional antidepressants when selective serotonin reuptake inhibitors are not effective. This qualitative narrative review summarizes what is known about the cardiovascular side effects of some of the newer antidepressants. Twelve novel non-selective serotonin reuptake inhibitor antidepressants were identified from the literature: venlafaxine, desvenlafaxine, duloxet
