Global burden and epidemiology of Barrett oesophagus and oesophageal cancer.

Oesophageal cancer is a global health problem; in 2018 there were more than 572,000 people newly diagnosed with oesophageal cancer worldwide. There are two main histological subtypes of oesophageal cancer, oesophageal adenocarcinoma (EAC) and oesophageal squamous cell carcinoma (ESCC), and there has been a dramatic shift in its epidemiology. While the incidence of EAC and its precursor lesion, Barrett oesophagus, has increased in Western populations over the past four decades, the incidence of ESCC has declined in most parts of the world over the same period. ESCC still accounts for the vast m

Overview of hepatitis C and skin.

Hepatitis C (HCV) is the most common cause of chronic liver disease and hepatocellular carcinoma, as well as the leading indication for liver transplantation in the Western world. For many patients, cutaneous manifestations may be the only, the earliest, or the most apparent sign of the underlying infection. The dermatologic manifestations of HCV infection are reviewed.

Acalculous clonorchiasis obstructing the common 3ile duct: a case report and review of the literature.

Clonorchis sinensis has a minor incidence in Western countries and most commonly manifests as cholangiohepatitis with secondary bacterial infection. This parasite rarely obstructs the common bile duct without associated biliary stricture, stones or tumor. This paper documents the fourth reported case of acalculous C, sinensis biliary tract obstruction and reviews the world literature.

Australian Trachoma Surveillance Report 2024.

Trachoma is the world's leading infectious cause of preventable blindness and is linked to poor living conditions. In Australia, trachoma has remained a public health problem in remote Indigenous communities in northern, central, and western regions. To seek formal validation by the World Health Organization (WHO) of the elimination of trachoma as a public health problem, Australia has needed to maintain levels of overall trachoma prevalence below 5% in children for at least two years in each formerly endemic state or territory. Australia must also demonstrate that the prevalence of previously

Possible eradication of wild poliovirus type 3--worldwide, 2012.

In 1988, the World Health Assembly resolved to eradicate polio worldwide. Since then, four of the six World Health Organization (WHO) regions have been certified as polio-free: the Americas in 1994, the Western Pacific Region in 2000, the European Region in 2002, and the South-East Asia Region in 2014. Currently, nearly 80% of the world's population lives in areas certified as polio-free. Certification may be considered when ≥3 years have passed since the last isolation of wild poliovirus (WPV) in the presence of sensitive, certification-standard surveillance. Although regional eradication has

Are the Major "Western" Findings on Attitudes to Immigration Supported by Global Evidence? Mostly Yes, Actually.

The causes of variation in attitudes to immigration have been the subject of extensive scholarship in recent decades, with some factors being repeatedly evidenced. However, these findings are overwhelmingly based on studies from a small selection of "Western" countries. This article makes use of recent World Values Survey data across 66 countries globally to test how generalizable such findings on the determinants of attitudes to immigration are globally. It finds that the most typically identified sociodemographic, economic, and contextual determinants of immigration policy preferences, preju

Monkeypox Mysteries of the New Outbreak in Non-Endemic Areas.

Monkeypox virus (MPXV) was declared by the World Health Organization (WHO) in mid-2022 to be a public health emergency of international concern, following its spread around the world after circulating in Western and Central Africa. This new outbreak is concentrated in men who have sex with men (MSM). Moreover, beyond the epidemiological change, compared with endemic countries, differences in clinical features and many other aspects have also been detected. These and other characteristics are unusual and still unclear. Based on the available data, the authors try to help to clarify some of the

Use and underuse of oral anticoagulation for stroke prevention in atrial fibrillation: old and new paradigms.

Atrial fibrillation (AF) is the most common sustained arrhythmia of the Western world. The increased ischemic stroke risk of the AF patient is one of the most important clinical issues to manage. Despite the well-known benefit of oral anticoagulation in high-risk AF patients, these drugs are widely underused in daily practice all over the world. We describe old and new paradigms of the use of oral anticoagulation. In the future, increased comprehensibility of stroke risk scores, development of a validated clinical bleed risk score, and new patient and physician user-friendly antithrombotic med

Certification of poliomyelitis eradication--European Region, June 2002.

On June 21, 2002, the Regional Commission for the Certification of Poliomyelitis Eradication (the Commission) certified that the European Region (EUR) of the World Health Organization (WHO) is free of indigenous wild poliovirus transmission. The last known case in EUR of polio caused by indigenous wild poliovirus transmission occurred in southeast Turkey in November 1998. EUR comprises 51 countries with an estimated population of 873 million and is the third of the six WHO regions to be certified as polio-free, following the Americas Region in 1994 and the Western Pacific Region in 2000. The past, present and future of global malaria and neglected tropical diseases: a disease burden assessment from 1990 to 2030.

INTRODUCTION: The World Health Organization (WHO) plans to control the epidemics of malaria and neglected tropical diseases (NTDs) by 2030. The aim of this study was to evaluate the realizability of achieving the WHO targets by assessing the past, present, and future global disease burden of malaria and NTDs. METHODOLOGY: Joinpoint regression, Spearman's correlation, and the autoregressive integrated moving average model (ARIMA) were used to estimate the trends of malaria and NTDs from 1990 to 2030 based on the Global Burden of Disease (GBD) Study 2019. RESULTS: Western Sub-Saharan Africa ha

A prospective discussion of past international nutrition catastrophes--indications for the future.

It is of interest that, aside from starvation, the nutrition catastrophes of the past, including scurvy (vitamin C deficiency) resulting from lack of fresh vegetables and fruit and beriberi (vitamin B1 deficiency) from consumption of polished rice, are forgotten and only of interest as history. The problems of vitamins were largely considered settled by the 1950s. With the appearance on the market of multiple-vitamin-and-mineral tablets, the public was also satisfied and considered the problem of deficiencies solved. Now we are faced with unexpected nutrition problems primarily in the industri

Stable Huh-7 cell lines expressing non-structural proteins of genotype 1a of hepatitis C virus.

Hepatitis C virus (HCV) infection has infected approximately 3% of the world population. HCV genotype 1a is distributed throughout the world, and along with genotype 1b, is relatively resistant to current standards of therapy compared to other HCV genotypes. The present study was designed to produce stable Huh-7 cell lines expressing non-structural proteins of HCV genotype la, representing an in vitro system to facilitate the development of new antiviral drugs against chronic HCV infection. The non-structural genes of HCV genotype 1a were amplified and cloned in a mammalian expression vector p

Anthrax: a disease in waiting?

Anthrax was a relatively unknown disease in the Western world until 2001, when spores were maliciously mailed in the US, causing five deaths. The mortality of the disease, the stability of its spores and the subsequent lack of person-to-person spread make anthrax an attractive biological weapon for terrorists with a desire for targeted mass destruction.

Cutaneous manifestations of metabolic diseases: uncommon presentations.

Metabolic diseases are common diseases in the Western world. Many of these diseases, including diabetes mellitus, hyperlipoproteinemia, gout, calcinosis, and hemochromatosis, are associated with skin diseases or often present with specific cutaneous signs. A knowledge of cutaneous manifestations helps to identify patients at risk, establish the internal diagnosis, and monitor the adverse effects of therapy.

Hereditary hypophosphataemic rickets: report of a family from the Indian subcontinent.

Nutritional deficiency continues to be an important cause of rickets in the underdeveloped and developing parts of the world. In the western hemisphere, predominantly non-nutritional forms of rickets and osteomalacia are now seen. In this report we discuss a family with X-linked hypophosphataemic rickets from the Kashmir region of the Indian subcontinent (which is an uncommon entity here).

Identification and Cloning of a New Western Epstein-Barr Virus Strain That Efficiently Replicates in Primary B Cells.

The Epstein-Barr virus (EBV) causes human cancers, and epidemiological studies have shown that lytic replication is a risk factor for some of these tumors. This fits with the observation that EBV M81, which was isolated from a Chinese patient with nasopharyngeal carcinoma, induces potent virus production and increases the risk of genetic instability in infected B cells. To find out whether this property extends to viruses found in other parts of the world, we investigated 22 viruses isolated from Western patients. While one-third of the viruses hardly replicated, the remaining viruses showed v

Use of oral polio vaccine for polio eradication.

The experience in three of the world's six WHO Regions (the Americas, the European and the Western Pacific Regions) shows that OPV is the right choice for stopping wild poliovirus transmission. Polio can be eradicated by carrying out SIAs to supplement routine vaccination and by placing added emphasis on reaching every child during every polio round. Monovalent OPV will be key tool in helping us to achieve the goal of polio eradication. The task of global eradication of poliomyelitis is uphill but well within our reach. A strong will and political commitment by the Government of India and effe

12-month prevalence of panic disorder with or without agoraphobia in the Swedish general population.

OBJECTIVE: The present study examined the prevalence of panic disorder with or without agoraphobia according to DSM-IV criteria in the Swedish general population. METHOD: Data were obtained by means of a postal survey administrated to 1000 randomly selected adults. The panic disorder module of the World Health Organization's Composite International Diagnostic Interview (CIDI) was included in the survey. RESULTS: 12-month prevalence was estimated at 2.2 % (CI 95 % 1.02 % - 3.38 %). There was a significant sex difference, with a greater prevalence for women (5.6 %) compared to men (1 %). CONC

[Jejuno-ileal volvulus, a rare cause of intestinal obstruction].

Volvulus of the small bowel without predisposing anatomical defects is a rare cause of acute intestinal obstruction in the Western world, and its incidence is decreasing in Europe and America, although it is common in others parts of the world. The causes of primary volvulus of the small bowel and the mechanism of rotation of the entire root of the mesentery remain unexplained. Several anatomical and dietary factors have been implicated. This theory requires strong peristalsis, tense, firm abdominal muscles and feeding pattern of very bulky meals taken at long intervals. In the present case, a

[Ascaris lumbricoides--a cause of acute abdomen].

Ascaris lumbricoides infestation is the most common helminthic disease in many developing countries and may cause severe surgical complications, especially in children. We present a 3-year-old Ethiopian immigrant brought to hospital directly from the airport because of signs of acute abdomen. It was found to be caused by volvulus and necrosis of a loop of bowel impacted with worms. The necrotic bowel loop was resected and a temporary ileostomy was formed; recovery was uneventful. This disease and its severe complications are rare in Israel and the western world. Awareness of its occurrence is
