Performance, ability to stay awake, and tendency to fall asleep during the night after a diurnal sleep with temazepam or placebo.

Sleep loss and increased sleepiness on the job are among the most prevalent problems encountered by people involved in night shift work, especially in cases of abrupt shift of the wake-sleep cycle. In such conditions, detrimental effects on performance are well documented. In these situations, to avoid decrements of performance at night, one possibility is to use hypnotics for improving the quality and quantity of daytime sleep. In this study, we evaluated the effects of 20 mg of temazepam on daytime sleep, the subsequent levels of nocturnal alertness/sleepiness, and performance in a laborator

The Association Between Resident Physician Work-Hour Regulations and Physician Safety and Health.

BACKGROUND: In 2011, the Accreditation Council for Graduate Medical Education (ACGME) instituted a 16-h limit on consecutive hours for first-year resident physicians. We sought to examine the effect of these work-hour regulations on physician safety. METHODS: All medical students matched to a US residency program from 2002 to 2007 and 2014 to 2017 were invited to participate in prospective cohort studies. Each month participants reported hours of work, extended duration shifts, and adverse safety outcomes, including motor vehicle crashes, percutaneous injuries, and attentional failures. The i

Sleep deprivation in junior doctors--house officers in Singapore.

House officers are known to endure marked levels of sleep deprivation in administration of their duties. We aim to establish sleep patterns of local house officers while on the job and the impact it might have on their mood and sleepiness state. We also studied their sleep during their final year of medical school and pre-university for identification of any prior sleep deprivation. Questionnaires were used to assess sleep and mood change. Sleepiness levels on the day after call were assessed using the Stamford Sleepiness Scale. Subjects were found to sleep a median of only 1.0 (+/- 2.0) h per

Psychological well-being of surgery residents before the 80-hour work week: a multiinstitutional study.

BACKGROUND: Accreditation Council on Graduate Medical Education work-hour restrictions are aimed at improving patient safety and resident well-being. Although surgical trainees will be dramatically affected by these changes, no comprehensive assessment of their well-being has been recently attempted. STUDY DESIGN: A multicenter study of psychological well-being of surgical residents (n = 108) across four US training programs before implementation of the 80-hour work week was performed using two validated surveys (Symptom Checklist-90-R [SCL-90-R] and Perceived Stress Scale [PSS]) during acade

Psychosocial health risks among nursing staff working in shifts.

BACKGROUND: Nurses are one of the professional groups most exposed to psychosocial risks. Excessive responsibilities, time pressure, the need to make quick decisions, shift work, lack of adequate rest, contact with death and human suffering - all these factors can significantly affect the mental state and result in the development of health problems among nursing staff. MATERIALS AND METHODS: The study was conducted among 193 nurses working at the University Clinical Hospital No. 2 of the Pomeranian Medical University in Szczecin. Participation in the study was anonymous and voluntary. The st

Impacts of shift work on sleep and circadian rhythms.

Shift work comprises work schedules that extend beyond the typical "nine-to-five" workday, wherein schedules often comprise early work start, compressed work weeks with 12-hour shifts, and night work. According to recent American and European surveys, between 15 and 30% of adult workers are engaged in some type of shift work, with 19% of the European population reportedly working at least 2 hours between 22:00 and 05:00. The 2005 International Classification of Sleep Disorders estimates that a shift work sleep disorder can be found in 2-5% of workers. This disorder is characterized by excessiv

Working the night shift: a necessary time for training or a risk to health and safety?

The European Working Time Directive (EWTD) limits excessive night shifts and restricts the working week to no more than 48 hours. The underlying rationale is to minimise the health risks to all workers. Here we debate the impact of night rotas for doctors-in-training on patient safety and medical education; when the EWTD was agreed these topics may not have been considered, either systematically or objectively. The impacts of diurnal rhythms on human functions affect all night workers, but the nature of rostered medical and surgical work has little precedent in other industries or even in the

Night-shift work and breast cancer risk in a cohort of Chinese women.

Shift work involving disruption of circadian rhythms has been classified as a probable cause of human cancer by the International Agency for Research on Cancer, based on limited epidemiologic evidence and abundant experimental evidence. The authors investigated this association in a population-based prospective cohort study of Chinese women. At baseline (1996-2000), information on lifetime occupational history was obtained from 73,049 women. Lifetime night-shift exposure indices were created using a job exposure matrix. During 2002-2004, self-reported data on frequency and duration of night-sh

Working hours indirectly affect anxiety symptoms through sleep and stress in dentists, physicians, and psychotherapists.

The aim of this study was to examine the effects of working hours on anxiety symptoms in highly educated helping professionals, especially through sleep duration, perceived stress and sleep quality. We analysed the results of 172 helping professionals (dentists, physicians, and psychotherapists, 57 % women, average age 25-63 years) who had participated in a larger study examining the effects of working conditions and constitutional factors on the onset of hand eczema. The participants answered a battery of questionnaires, including Cohen's Perceived Stress Scale, Zung Self-Rating Anxiety Scale

Measuring Light at Night and Melatonin Levels in Shift Workers: A Review of the Literature.

Shift work, especially that involving rotating and night shifts, is associated with an increased risk of diseases, including cancer. Attempts to explain the association between shift work and cancer in particular have focused on the processes of melatonin production and suppression. One hypothesis postulates that exposure to light at night (LAN) suppresses melatonin, whose production is known to slow the development of cancerous cells, while another proposes that circadian disruption associated with shift work, and not just LAN, increases health risks. This review focuses on six studies that e

There was evaluated the risk to health in females employed in shift work, including night shifts. According to the data of periodical medical examinations health indices of 403 females employed in shift work, including night shifts, were compared with indices of 205 females--workers of administrative units of the same enterprise. Overall relative risk (RR) for the health disorder associated with the night shift was 1.2 (95%; confidence interval (CI): 1.09-1.28). A statistically significant increase in risk was observed in relation to uterine fibroids (OR 1.3; 95% CI: 1.06-1.54), mastopathy (OR

Relationship of chronotype to sleep, light exposure, and work-related fatigue in student workers.

Students who work during the school year face the potential of sleep deprivation and its effects, since they have to juggle between school and work responsibilities along with social life. This may leave them with less time left for sleep than their nonworking counterparts. Chronotype is a factor that may exert an influence on the sleep of student workers. Also, light and social zeitgebers may have an impact on the sleep-related problems of this population. This study aimed to document sleep, light exposure patterns, social rhythms, and work-related fatigue of student workers aged 19-21 yrs an

Disability after labyrinthectomy.

A labyrinthectomy is known to relieve vertigo successfully in the majority of patients who suffer from Menière's disease and have non-serviceable hearing in the affected ear. It is assumed that the procedure reduces disability, helps the patient to return to work and improves the quality of life. Eighteen patients who underwent a transmastoid drill-out labyrinthectomy between 1980 and 1990 were interviewed and an attempt was made to evaluate the success of the operation in accordance with the guidelines set out by the AAO-HNS 1985. In the present series it was noted that although vertigo was r

Psychosocial factors influencing outcome in patients with low-back pain.

A psychosocial analysis of 58 patients with low-back pain was conducted. Three groups of patients with different outcomes were delineated with distinctly different psychosocial profiles. Patients who recovered uneventfully had a better income, reported more satisfaction with hospitalization or medical personnel, and had prolonged pain-free periods. Patients with the worst outcomes in terms of poor work role adjustment and severe pain complaints were more likely to be immigrants from mainland China, to be engaged in heavy manual labor, to be involved in compensation claims for work-related inju

Night shift work and cardiovascular disease biomarkers in female nurses.

BACKGROUND: Night shift work is associated with cardiovascular disease, but its associations with cardiovascular disease biomarkers are unclear. We investigated these associations in a study of female nurses. METHODS: We used data from the Nurses' Health Study II for total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein (HDL) cholesterol, triglycerides, C-reactive protein (CRP), and fibrinogen. The sample sizes for our analysis ranged from 458 (fibrinogen) to 3574 (total cholesterol). From questionnaires, we determined the number of night shifts worked in the 2 week

Improving sailor's amount and efficiency of sleep in two-section watchkeeping schedules: A cluster randomized trial study.

STUDY OBJECTIVES: The present study aimed at comparing sailors' amount and efficiency of sleep in two two-section watchkeeping schedules, i.e. the traditional 6/6 and the 7-5-5-7. Intention for the latter schedule was to align the major sleep episodes with biologically driven nocturnal sleep patterns. METHODS: Using a parallel crossover design with cluster randomized trials, forty-eight crew members from two vessels of the German Navy participated in a field trial with two 1-week underway periods varying the watchkeeping schedule in randomized order. Sleep minutes and sleep efficiency were as

Night today, day tomorrow: how irregular work shifts interfere with our psychological health.

The 24/7 economic activity has led to a growing demand for shift workers who now make up about 21% of the working population in Europe. Shift work causes desynchronization of biological and social rhythms and therefore increases the risk for physiological and psychological health issues. Night shifts, but in particular irregular shifts are considered to have numerous harmful effects on health and well-being. In this investigation, 185 shift workers employed at an Austrian railway company, filled in online questionnaires, which included the PSQI, the ESS, and items assessing sleep issues, their

Promoting alertness with a short nap during a night shift.

The use of a short (< 1 h) nap in improving alertness during the early morning hours in the first night shift was examined under laboratory conditions. The study contained four experimental, non-consecutive night shifts with a nap of either 50 or 30 min at 01.00 or 04.00 hours. An experimental night shift without a nap served as a control condition. Each experimental shift was followed by daytime sleep. Fourteen experienced male shift workers went through all of the experimental conditions. The results showed that the naps improved the ability to respond to visual signals during the second hal
