[Chest physiotherapy techniques - can they reduce hyperinflation].

The authors review the literature concerning the possibilities of modifying the mechanical characteristics of the respiratory system with breathing retraining and other chest physiotherapy intervention. Breathing retraining techniques with prolonged and active expiration induce increased work of breathing and do not help diaphragmatic function. Only pursed lips breathing seems to produce some mechanical advantages. The seated and forward position with fixed arms may reduce dyspnea but this does not appear to be caused by a decrease in operating lung volumes. By contrast, the dorsal decubitus p

Increased respiratory effort during sleep is non-invasively detected with movement sensor.

INTRODUCTION: Measuring breathing effort during sleep with an oesophageal pressure sensor remains technically challenging and has not become routine practice. The aim of the present work was to investigate whether increased thoracic pressure during sleep can be detected with the Emfit movement sensor. Experimental data suggest that increased respiratory efforts with the intrathoracic pressure variation induce high-frequency spikes in the Emfit signal, but this has not been systematically examined. METHODS: Polysomnography, oesophageal pressure and Emfit signal were recorded in 32 patients wit

Vagal control of the breathing pattern and respiratory mechanics in the adult and newborn rabbit.

In an attempt to define the role of SARs on the breathing pattern and respiratory mechanics, we have studied adult rabbits anaesthetised, supine and tracheostomised during air breathing (A), after SO2 breathing (200 ppm for a period long enough to abolish the apnea following lung inflation, 10-30 min) (S), and after bilateral vagotomy (V). Tidal volume and inspiratory time increased, while breathing frequency decreased from A to V. The effects of S depended upon the type of anaesthesia used, with a definite increase in volume and a drop in frequency in the urethane anaesthetized animals and a

CO2 sensing by connexin26 and its role in the control of breathing.

Breathing is essential to provide the O2 required for metabolism and to remove its inevitable CO2 by-product. The rate and depth of breathing is controlled to regulate the excretion of CO2 to maintain the pH of arterial blood at physiological values. A widespread consensus is that chemosensory cells in the carotid body and brainstem measure blood and tissue pH and adjust the rate of breathing to ensure its homeostatic regulation. In this review, I shall consider the evidence that underlies this consensus and highlight historical data indicating that direct sensing of CO2 also plays a significa

[Mechanism of the fast neurogenic component of the respiratory response to muscular work].

Dynamics of pulmonary ventilation, electric activity of the intercostal muscles and of the alveolar gas composition was studied in 12 healthy men during dosaged muscular work; these men were given different gas mixtures to breathe. The respiratory response at the initial period of work in inhalation of the hypoxic-hypercapnic gas mixture was greater than that in persons who breathed room air. This response practically disappeared after oxygen hyperventilation. Apparently the rapid component of the ventilation response to the muscular work was largely due to increased sensitivity of the respira

Free-breathing qRF-MRF with pilot tone respiratory motion navigator for T1, T2, T2*, and off-resonance mapping of the human body at 3 T.

Standard quantitative abdominal MRI techniques are time consuming, require breath-holds, and are susceptible to patient motion artifacts. Magnetic resonance fingerprinting (MRF) is naturally multi-parametric and quantifies multiple tissue properties, including T1 and T2. This work includes T2* and off-resonance mapping into a free-breathing MRF framework utilizing a pilot tone navigator. The new acquisition and reconstruction are compared to current clinical standards. Prospective. Ten volunteers. 3 T scanner, Quadratic-RF MRF, Balanced SSFP, Inversion recovery spin-echo, LiverLab. [Breath alcohol test in construction sites in the Province of Belluno: a campaign in favour of health promotion and surveillance and against drinking].

BACKGROUND AND OBJECTIVES: Health impairment due to alcohol use and abuse is well known, in terms of relationship with traffic accidents and work accidents. In Italy almost 10 per cent of accidents at work involve intoxicated people injuring themselves and innocent victims. Alcohol abuse is a factor involved in determining severe accidents in the construction industry and epidemiological studies demonstrated a relationship between an elevated alcohol use and severity of accidents. Since in the Province of Belluno alcohol consumption may be elevated also at work, the two Occupational Health Uni

[Obstruction of the nose in newborns].

Five newborns, 4 girls and 1 boy, presented with breathing and feeding difficulties caused by obstruction of the nose. The causes were choanal atresia, apertura pyriformis stenosis, teratoma, glioma and haemangioma. Following surgical treatment the children were symptom-free. Rapid diagnosis and treatment of these conditions are essential. An alternative airway should be created if a newborn suffers from breathing problems. Diagnostic work-up includes flexible laryngo-endoscopy followed by MRI- or CT-scan. Treatment is usually surgical.

Recurrent cutaneous decompression sickness in a hyperbaric chamber attendant with a large persistent foramen ovale.

A 41-year-old female nurse had cutaneous decompression sickness on two occasions after acting as an inside chamber attendant for patients receiving hyperbaric oxygen. She breathed air during the treatments at pressures equivalent to 14 and 18 metres of seawater, but each time she decompressed whilst breathing oxygen. Latency was 2.5 hours and one hour. She was found to have an 11 mm diameter persistent foramen ovale. It was closed and she returned to work without recurrence of decompression sickness. Review of the literature suggests that shunt mediated decompression sickness is an important o

Respiratory mechanics in Standardbred horses with sub-clinical inflammatory airway disease and poor athletic performance.

The aim of this study was to investigate and quantify respiratory mechanical dysfunctions in Standardbred horses with both poor performance and bronchoalveolar lavage fluid cytology characteristic of inflammatory airway disease (IAD). A control group of healthy Standardbred horses was compared. Respiratory mechanics and breathing pattern were examined at rest and during hyperventilation induced using a rebreathing method. At rest, respiratory mechanics and breathing pattern were superimposable in both groups. In IAD horses, rebreathing increased ventilation, with larger tidal volumes and lower

[Inspiratory pressure support via facial mask in acute respiratory insufficiency].

A patient in acute respiratory failure (Murray score 2.0) caused by pneumonia in the remaining lung 4 days after left-sided pneumonectomy was successfully treated with non-invasive pressure support by face mask. Non-invasive ventilation was chosen to keep the time of ventilating as short as possible, to achieve minimum pressure within the airways, and to retain natural defense and clearance mechanisms. Inspiratory pressure support was added to reduce increased work of breathing, improve ventilatory mechanics and gas exchange. Also, intrinsic PEEP was counterbalanced by external PEEP of 5 cmH2O

[Non-invasive respiratory support in acute respiratory distress].

Conventional oxygen therapy could be delivered via nasal prongs, face mask, venturi mask, or non-rebreather mask. High flow nasal cannula (HFNC) produces a higher flow of FiO2. This higher flow creates a positive pressure on the higher respiratory tract. Collapsed alveoli could be opened due to this positive pressure and CO2 would be eliminated. This improves the oxygenation and reduces the total work of breathing. Continuous positive pressure support (CPAP) realizes a continuous positive expiratory pressure to the respiratory tract, which opens the alveoli and reduces the pre-load by reducing

Physiotherapy intervention for preventing the respiratory muscle deterioration in institutionalized older women with functional impairment.

INTRODUCTION: In elderly seniors (>80 years), respiratory function may be compromised when, in addition to the presence of comorbidity and loss of mobility, there is also reduced respiratory muscle (RM) strength. The literature has shown that RM training could be an effective method to improve RM function and prevent clinical deterioration, particularly in population with RM weakness. The main purpose of this paper was to assess the effectiveness of RM training on the respiratory muscle strength and endurance of institutionalized elderly women with functional impairment. METHOD: Fifty-four re

Inhalation route effects on exposure to 2.0 parts per million sulfur dioxide in normal subjects.

A number of investigations have attributed the control of the nasal to oral/nasal ventilation transition to nasal resistance. To investigate possible changes in nasal resistance due to sulfur dioxide (SO2) exposure, 14 subjects (7 men and 7 women), healthy non-smokers, between the ages of 20 and 46 years, were exposed for 30 minutes to filtered air while free breathing and to 2.0 ppm SO2 with either free breathing, forced oral or forced nasal breathing with continuous exercise at a workload 300 kg.m/min below the workload which initiated cross-over from nasal to oral/nasal breathing in a preli

The respiratory mechanics of the yacare caiman (Caiman yacare).

The structure and function of crocodilian lungs are unique compared with those of other reptiles. We examined the extent to which this and the semi-aquatic lifestyle of crocodilians affect their respiratory mechanics. We measured changes in intratracheal pressure in adult and juvenile caiman (Caiman yacare) during static and dynamic lung volume changes. The respiratory mechanics of juvenile caiman were additionally measured while the animals were floating in water and submerged at 30, 60 and 90 deg to the water's surface. The static compliance of the juvenile pulmonary system (2.89±0.22 ml cmH

The effect of OSAS on sick leave and work disability.

The objective of the present study was to examine the independent contribution of symptoms of obstructive sleep apnoea syndrome (OSAS) to long-term sick leave and permanent work disability. Using a historical cohort design with 4 yrs of follow-up, information on sick leave and disability benefit recipiency were merged with health information from the Hordaland Health Study, carried out in western Norway during 1997-1999. Persons aged 40-45 yrs (n = 7,028) were assessed for self-reported symptoms of OSAS (snoring, breathing cessations and daytime sleepiness), body mass index, somatic conditions

A Novel Approach to the Identification of Compromised Pulmonary Systems in Smokers by Exploiting Tidal Breathing Patterns.

Smoking causes unalterable physiological abnormalities in the pulmonary system. This is emerging as a serious threat worldwide. Unlike spirometry, tidal breathing does not require subjects to undergo forceful breathing maneuvers and is progressing as a new direction towards pulmonary health assessment. The aim of the paper is to evaluate whether tidal breathing signatures can indicate deteriorating adult lung condition in an otherwise healthy person. If successful, such a system can be used as a pre-screening tool for all people before some of them need to undergo a thorough clinical checkup.

Simulation and minimisation of the airway deposition of airborne bacteria.

Respiratory infections represent one of the most important bioaerosol-associated health effects. Bacteria are infectious micro-organisms that may, after inhalation, cause specific respiratory diseases. Although a large number of inhalable pathogenic bacteria have been identified and the related respiratory symptoms are well known, their airway transport and deposition are still not fully explored. The objective of this work was to characterise the deposition of inhaled bacteria in different regions of the lung and to find the optimum breathing modes, which ensure the minimum chance of a bacter
