Establishing surveillance for diabetes in American Indian youth.

OBJECTIVE: To determine prevalence estimates in order to monitor diabetes, particularly type 2 diabetes, in American Indian youth. RESEARCH DESIGN AND METHODS: To explore the feasibility of developing a case definition using information from primary care records, all youth aged <20 years with an outpatient visit or hospitalization for diabetes were identified from the Billings Area Indian Health Service database in Montana and Wyoming from 1997 to 1999, and the medical records were reviewed. Classification for probable type 1 diabetes was based on age < or =5 years, weight per age < or =15th

The Cambrian (Guzhangian; Marjuman) kingstoniid trilobite Kingstonia Walcott, 1924 in Laurentian North America.

Kingstonia Walcott, 1924 is widespread across Laurentian North America in Upper Marjuman (Guzhangian) strata. It is recorded from mixed carbonate-siliciclastic, storm-influenced facies, such as the Nolichucky Formation of Tennessee and the Sullivan Formation of Alberta, as well as shallow subtidal carbonates, including the Du Noir Formation of Wyoming. The genus ranges from the shelf interior in areas such as South Dakota to the Laurentian continental margin in Quebec and Newfoundland. This monograph focuses on the abundant material from shelf margin faunas of the Shallow Bay Formation of west

Epidemiology of professional bare-knuckle fighting injuries.

BACKGROUND: Professional bare-knuckle fighting (BKF) is a variation of boxing which held its first modern legal event in 2018 in Wyoming. Since then, the sport has expanded with state-sanctioned events held in Florida, Missouri, Mississippi, Kansas, and Alabama. The purpose of this study was to evaluate the epidemiology of injuries in bare-knuckle fighting bouts and to discern any trends which may distinguish it from traditional boxing with padded gloves. METHODS: Observational data collection for all state-sanctioned professional bare-knuckle fighting bouts was conducted sequentially over a

DNA vaccination of bison to brucellar antigens elicits elevated antibody and IFN-γ responses.

Brucella abortus remains a threat to the health and well-being of livestock in states bordering the Greater Yellowstone Area. During the past several years, cohabitation of infected wildlife with cattle has jeopardized the brucellosis-free status of Idaho, USA; Wyoming, USA; and Montana, USA. Current livestock B. abortus vaccines have not proven to be efficacious in bison (Bison bison) or elk (Cervus elaphus nelsoni). One problem with the lack of vaccine efficacy may stem from the failure to understand wildlife immune responses to vaccines. In an attempt to understand their immune responses, b

Meeting the needs of regional minority groups: the University of Washington's programs to increase the American Indian and Alaskan native physician workforce.

Minority populations in the United States are growing rapidly, but physician workforce diversity has not kept pace with the needs of underserved communities. Minorities comprised 26.4% of the population in 1995; by 2050, these groups will comprise nearly half. Medical schools must enlist greater numbers of minority physicians and train all physicians to provide culturally responsive care. The University of Washington School of Medicine (UWSOM) is the nation's only medical school that serves a five-state region (Washington, Wyoming, Alaska, Montana, and Idaho). Its mission addresses the need to

[Transmissible spongiform encephalopathies in animals].

Transmissible spongiform encephalopathies in animals are known for centuries. In particular scrapie in sheep and goats occurs worldwide; it spreads as a natural disease and is genetically controlled. Chronic wasting disease (CWD) in the United States (Wyoming and Colorado) also spreads as natural disease among free ranging and captive elk and mule deer. In contrast, transmissible mink encephalopathy (TME) of mink in fur producing farms is caused by contaminated feed; the source of this food contamination is still controversial. The only occurrence of a TSE in an avian species was reported from

A palliative care needs assessment of rural hospitals.

BACKGROUND: Palliative care services are lacking in rural hospitals. Implementing palliative care services in rural and remote areas requires knowledge of available resources, specific barriers, and a commitment from the hospital and community. OBJECTIVE: The purpose of the study was to determine awareness, knowledge, barriers, and resources regarding palliative care services in rural hospitals. METHODS: A descriptive survey design used an investigator-developed needs assessment to survey 374 (40% response rate) health care providers (chief executive officers, chiefs of medical staff, chief

Trends and Disparities in Acute Myocardial Infarction-Related Mortality With Co-Listed Nicotine Dependence in the United States, 1999-2020.

BACKGROUND: Nicotine use is a major modifiable risk factor for acute myocardial infarction (AMI), yet national mortality patterns involving co-listed nicotine dependence remain under-characterized. METHODS: Using CDC WONDER Multiple Cause of Death data from 1999 to 2020, we evaluated AMI-related mortality among US adults aged ≥ 25 years with co-listed death-certificate-coded nicotine dependence, operationally defined using ICD-10 F17.0-F17.9. AMI was identified using ICD-10 I21.0-I22.9. Age-adjusted mortality rates (AAMRs) per 100 000 population were calculated using the 2000 US standard popu

Use of Dog Serologic Data for Improved Understanding of Coccidioidomycosis: A One Health Approach.

BACKGROUND: Coccidioidomycosis (Valley fever) occurs when animals and humans inhale spores of Coccidioides spp, soil-dwelling fungi of the southwestern United States (US). The spatial epidemiology of coccidioidomycosis is poorly understood due to irregular detection of Coccidioides in soil, disease underdiagnosis, and lack of nationwide mandatory reporting. Data on seroreactivity to Coccidioides among dogs-which are highly susceptible to coccidioidomycosis, widespread across the US, and have limited travel-may strengthen our understanding human disease risk. METHODS: We analyzed serologic tes

The 2003 West Nile virus United States epidemic: the America's Blood Centers experience.

BACKGROUND: A detailed assessment of West Nile virus (WNV) yield is needed to evaluate the effectiveness of the WNV nucleic acid amplification technology (NAT) screening implemented in 2003. STUDY DESIGN AND METHODS: WNV NAT screening and donation data were compiled from members of America's Blood Centers, which collect nearly 50 percent of the US blood supply. WNV RNA screening was performed with either the Gen-Probe/Chiron Procleix transcription-mediated amplification assay or the Roche TaqScreen polymerase chain reaction. Results of alternate NAT and WNV immunoglobulin M (IgM) antibody ass

Brucella abortus strain RB51 vaccination in elk. I. Efficacy of reduced dosage.

Bovine brucellosis is a serious zoonotic disease affecting some populations of Rocky Mountain elk (Cervus elaphus nelsoni) and bison (Bison bison) in the Greater Yellowstone Area, USA. The fear that elk and/or bison may spread Brucella abortus to livestock has prompted efforts to reduce or eliminate the disease in wildlife. Brucella abortus strain RB51 (RB51) vaccine has recently been approved for use in cattle. Unlike strain 19 vaccine, RB51 does not cause false positive reactions on standard brucellosis serologic tests. If effective, it may become the vaccine of choice for wildlife. In Febru

Geographic Clustering of Idiopathic Subglottic Stenosis in North America: An Ecological Cohort Study.

BACKGROUND: Idiopathic subglottic stenosis (iSGS) is a recurrent fibroinflammatory narrowing of the subglottis, typically affecting White pre- and perimenopausal women. While its etiology remains uncertain, inflammatory signaling, reflux, hormonal influences, and mycobacterial factors have been proposed. This study evaluated geographic variability in survey-ascertained iSGS prevalence, exploring potential environmental correlates associated with regional disease distribution. METHODS: A retrospective ecological analysis of self-reported demographic and clinical data from members of an interna

Disparities in Access to Gastrointestinal Care in the United States.

Disparities in gastrointestinal (GI) care in the United States represent a major health burden, with GI diseases accounting for approximately $135 billion annually in direct medical expenditures. A pronounced geographic disparity exists, with over two-thirds of the 3,149 U.S. counties lacking any GI specialist. This results in an estimated 49-50 million Americans required to travel more than 25 miles for specialty care. For instance, states like Wyoming and Arizona report over 75% of residents are more than 25 miles from GI care, while states such as Alaska and North Dakota average only 1.8 ga

An inherited lower motor neuron disease of pigs: clinical signs in two litters and pathology of an affected pig.

A chronic progressive neurodegeneration, called hereditary porcine neuronal system degeneration (HPNSD), was recognized in a swine herd in Devon, England. Adult pigs that were presumed carriers of the dominantly inherited trait for HPNSD were transferred from England, where a breeding colony was maintained for 9 years, to the Wyoming State Veterinary Laboratory (WSVL) for study. Two litters of affected piglets were born to 2 carrier sows at the WSVL. Clinical signs of muscular tremors, paresis, or ataxia developed at 12-59 days of age in 4 of 6 liveborn pigs. Three other pigs were stillborn. I
