The dynamics of molecular evolution of emerging avian reoviruses through accumulation of point mutations and genetic re-assortment.

In the last decade, the emergence of variant strains of avian reovirus (ARV) has caused enormous economic impact in the poultry industry across Canada and USA. ARVs are non-enveloped viruses with ten segments of double-stranded RNA genome. So far, only six genotyping cluster groups are identified worldwide based on sequence analysis of the σC protein encoded by the S1 segment. In this study, we performed deep next generation whole-genome sequencing and analysis of twelve purified ARVs isolated from Saskatchewan, Canada. The viruses represent different genotyping cluster. A genome-wide sequence

Closed atraumatic complete rupture of the flexor halluces longus tendon during forward lunge exercise: A case report.

RATIONALE: Acute rupture of the flexor halluces longus (FHL) tendon due to trauma or laceration is a well-known phenomenon. Partial rupture of the FHL tendon caused by tendinitis or stenosing tenosynovitis is common in ballet dancers and athletes. However, atraumatic complete rupture of the FHL is rare: as of 2018, only 7 cases of closed atraumatic complete rupture of the FHL tendon have been reported in the literature. Here, we report on a patient who presented with a closed atraumatic complete rupture of the FHL tendon during a forward lunge exercise. PATIENT CONCERNS: A 35-year-old female

Entrapment of the long head of the biceps tendon: the hourglass biceps--a cause of pain and locking of the shoulder.

We describe an unrecognized mechanical condition affecting the long head of the biceps (LHB) tendon with entrapment of the tendon within the joint and subsequent pain and locking of the shoulder on elevation of the arm. We identified 21 patients with a hypertrophic intraarticular portion of the LHB tendon during open surgery (14 patients) or arthroscopic surgery (7 patients). All cases but one were associated with a rotator cuff rupture. Patients were treated by biceps tenotomy (2 patients) or tenodesis (19 patients) after removal of the hypertrophic intraarticular portion of the tendon and ap

[Long-term results of endoprosthetic joint replacement and synovectomy].

The operations most frequently performed for chronic polyarthritis are synovectomy and artificial joint replacement. The radioisotope synovectomy has in general reduced the need for operative synovectomy. Multicenter studies of the late results of operative synovectomy (over 10 years after the operation) show a lasting improvement in pain, although radiographically the destructive changes that are seen 10 years after the operation are similar to the non-operated side. The synovectomy can therefore buy important time since re-operation, e.g. arthroplasty, is performed mainly on clinical grounds

[Imaging of wrist injuries in athletes].

Wrist imaging is currently used for diagnosis of sport injuries as part of a global strategy of rapid recovery. Standard x-rays are the first step in this procedure. Although arthrography is still the reference for the diagnosis of intrinsic ligament and cartilaginous lesions, MRI can sometimes be sufficient. Ultrasonography is a dynamic process and is accurate in detecting tendon injuries. Wrist sport injuries are frequent and often asymptomatic. Here we review the usual aspects of bone, ligament, and tendon lesions encountered in each sport, while providing advice on the most appropriate ima

Suprafibrous injection with corticosteroid in de Quervain's disease.

Between February 1996 and August 2001, 115 suprafibrous injections with corticosteroid were performed on 103 patients. Initial satisfactory result was found in 105 wrists (91.30%). The average duration of follow-up was 34 months. Seventy-one wrists (61.74%) had no recurrence of the symptom. The average pain free interval in the patient who had recurrence of the symptom after having an initial satisfactory result was five months. The success rate following one to three suprafibrous injections was 77.39 per cent. Suprafibrous injection is technically easier and has less risk of intratendinous in

DE QUERVAIN'S DISEASE: STENOSING TENOVAGINITIS AT THE RADIAL STYLOID PROCESS.

Twelve cases of stenosing tenovaginitis at the radial styloid process (de Quervain's disease), including two bilateral cases, are reviewed. These patients presented the typical symptoms of pain just proximal to the radial styloid process accompanied by limitation of abduction of the thumb. Many previous reports have emphasized the frequency of aberrant tendons in this region and have implied that such anomalies may play a part in the etiology of this disorder. In our cases aberrant tendons were not found. Pronounced thickening of the tendon sheaths was present at operation and accumulations of

Distal clavicle fracture as a complication of arthroscopic distal clavicle resection.

Arthroscopic resection of the distal clavicle has been advocated as a surgical treatment option for acromioclavicular (AC) joint pathology. To our knowledge, iatrogenic fracture of the distal clavicle during distal clavicle resection has never been reported. This report describes distal clavicle fracture as a complication of misidentification of the AC joint and subsequent aggressive burring of the distal clavicle during shoulder arthroscopy. This case is further complicated by the development of symptomatic delayed union and adhesive capsulitis. Ultimately, a revision distal clavicle resectio

Outcome of trigger finger treatment in diabetes.

Trigger finger is an underdiagnosed hand disorder causing disability in longstanding diabetic patients. Sixty diabetic patients [39 insulin-dependent diabetes mellitus (IDDM) and 21 non-insulin-dependent diabetes mellitus (NIDDM)] and 60 nondiabetic patients were examined. All were initially treated by steroid injections: failure to alleviate symptoms was the indication for surgery. The incidence of multiple digit involvement was higher in IDDM patients as compared with the control group (p < 0.001). The diffuse type was 1.45 times more frequent in IDDM and NIDDM than in nondiabetic patients (

Quantifying the Effect of Diabetes on Surgical Hand and Forearm Infections.

PURPOSE: Diabetes has long been established as a risk factor for hand and forearm infections. The purpose of this study was to review the effect of glycemic factors on outcomes among diabetic patients with surgical upper-extremity infections. We hypothesized that diabetic inpatients may benefit from stronger peri-infection glycemic control. METHODS: A prospective cohort study enrolled diabetic and nondiabetic surgical hand and forearm infections over 3 years. Glycemic factors included baseline glycosylated hemoglobin, blood glucose (BG) at presentation, and inpatient BG. Poor baseline control

Work-related musculoskeletal disorders in athletic trainer.

INTRODUCTION Athletic trainers often work for elongated and irregular hours or days, and need to react in emergency situations. These professionals are at risk for work-related musculoskeletal disorders (WMSDs). This study investigated the WMSDs experienced by athletic trainers (ATs), plus related personal and occupational factors. METHODS A customized questionnaire with subject's demographic and WMSDs information was constructed and administered in all accessible ATs in Taiwan. In addition to descriptive analysis of demographics and WMSDs, the relationship between personal/occupational-relate

Occupational hand infections.

Prompt diagnosis and treatment of hand and upper extremity infections is imperative because they have the potential to be life threatening and pose a risk of functional impairment. Serious infections may result in prolonged hospitalization and resultant loss of productivity. The mainstay of treatment continues to be antibiotic therapy, heat, elevation, adequate surgical drainage, and débridement. Prompt specialty consultation should be obtained in cases in which there is a doubt about the diagnosis or when there is failure to improve in the face of seemingly appropriate treatment.

Save that arm: a study of problems in the remaining arm of unilateral upper limb amputees.

A study has been made by questionnaire, personal examination and telephone interview of unilateral upper limb amputees seen at the Prince Henry Hospital, Australia between 1994 and 1997. There were 60 questionnaires posted. Replies were received from 46. Problems were noted in the remaining arm of 23 (50%). The respondents' problems not only consisted of overuse symptoms, but also of an exacerbation of pre-existing arthritis and injury due to trauma to the remaining arm during the accident. Case histories are given in 3 typical cases. Treating professionals are warned about the hazards that on
