Discovery of Biomarkers Related to Colorectal Cancer by Systematic Proteomics Analysis and Experimental Procedures.

BACKGROUND: Total hip arthroplasty (THA) relieves pain and restores function in severe hip arthritis and other painful hip disorders. THA is successful; however, it involves significant perioperative blood loss, which can lead to complications and economic burden. Tranexamic acid (TXA) reduces blood loss. However, its impact on hidden blood loss (HBL) is uncertain. The hemostatic effect of TXA may be enhanced by carbazochrome sodium sulphate (CSS). This study aimed to evaluate the safety and efficacy of combined CSS and TXA in reducing blood loss in THA. METHODS: This systematic review and me

Continuous use of direct oral anticoagulants during and after simple and surgical tooth extractions: a prospective clinical cohort study.

BACKGROUND: No consistent approach to the management of direct oral anticoagulants (DOACs) during and after oral surgery has been established. Thus, DOACs may be unnecessarily discontinued, raising the potential risk of life-threatening thromboembolism. To address the inconsistency in this approach, our study assessed the risk of bleeding and other complications in patients who continue to use DOACs during and after simple and surgical tooth extractions. METHODS: Between May 2016 and December 2023, this prospective study recruited patients aged 18 years or older who were receiving a DOAC or w

Age- dependent injury patterns, management and impact on mortality of severe thoracic trauma in severely injured children: a retrospective study from the TraumaRegister DGU®.

BACKGROUND: Severe thoracic trauma in children is rare and often underestimated. While standardized protocols exist, the rarity of polytrauma in children may lead to uncertainties in their application. Thus, we aim to characterize severely injured children with significant thoracic trauma and identify age-dependent differences in prehospital and early clinical management. METHODS: Patients documented by German hospitals between 2008 and 2023 in the TraumaRegister DGU®, aged ≤ 20 years and sustaining at least serious chest injury (AISThorax ≥ 3) were analyzed. Patients were grouped by age: 0-5

Evaluating Surgeon-influenced Factors for Total Knee Arthroplasty Value-based Reimbursement.

INTRODUCTION: Clinical outcome measures used under value-based reimbursement models require risk stratification of patient demographics and medical history. Only certain perioperative patient factors may be influenced by the surgeon. The study evaluated surgeon-influenced modifiable factors associated with achieving literature-defined KOOS score thresholds to serve as the foundation of the newly established alternative payment models for total knee arthroplasties (TKA). METHODS: We retrospectively reviewed a consecutive cohort of 4,324 patients undergoing TKA. Surgeon-influenced modifiable ri

Multifunctional interaction of CihC/FbpC orthologs of relapsing fever spirochetes with host-derived proteins involved in adhesion, fibrinolysis, and complement evasion.

INTRODUCTION: Relapsing fever (RF) remains a neglected human disease that is caused by a number of diverse pathogenic Borrelia (B.) species. Characterized by high cell densities in human blood, relapsing fever spirochetes have developed plentiful strategies to avoid recognition by the host defense mechanisms. In this scenario, spirochetal lipoproteins exhibiting multifunctional binding properties in the interaction with host-derived molecules are known to play a key role in adhesion, fibrinolysis and complement activation. METHODS: Binding of CihC/FbpC orthologs to different human proteins an

Thromboelastometry-guided haemostatic resuscitation in severely injured patients: a propensity score-matched study.

BACKGROUND: To accelerate the diagnosis and treatment of trauma-induced coagulopathy (TIC), viscoelastic haemostatic assays (VHA) are increasingly used worldwide, although their value is still debated, with a recent randomised trial showing no improvement in outcome. The objective of this retrospective study was to compare 2 cohorts of injured patients in which TIC was managed with either a VHA-based algorithm or a conventional coagulation test (CCT)-based algorithm. METHODS: Data were retrieved from 2 registries and patients were included in the study if they received at least 1 unit of red

Severe Epistaxis Secondary to Dabrafenib and Trametinib Toxicity in Non-small Cell Lung Carcinoma With Small Bowel Metastasis.

BRAF mutations are estimated to be present in 2-4% of non-small cell lung carcinoma (NSCLC) cases. BRAF inhibitor (dabrafenib) and MEK inhibitor (trametinib) are currently approved to treat NSCLC harboring the BRAF V600E mutation. However, the use of this new combined targeted therapy can be associated with severe and life-threatening toxicities. Here, we describe the case of a 77-year-old male with a history of BRAF-positive lung adenocarcinoma with metastasis to the brain, adrenals, and small bowel (jejunum), currently on dual therapy with dabrafenib and trametinib, who presented with refrac

Formative research to design an implementation strategy for a postpartum hemorrhage initial response treatment bundle (E-MOTIVE): study protocol.

BACKGROUND: Postpartum hemorrhage (PPH) is the leading cause of maternal death worldwide. When PPH occurs, early identification of bleeding and prompt management using evidence-based guidelines, can avert most PPH-related severe morbidities and deaths. However, adherence to the World Health Organization recommended practices remains a critical challenge. A potential solution to inefficient and inconsistent implementation of evidence-based practices is the application of a 'clinical care bundle' for PPH management. A clinical care bundle is a set of discrete, evidence-based interventions, admin

Thromboelastometry-guided hemostatic therapy for hemorrhagic shock in the postoperative period of vascular surgery: a case report.

BACKGROUND: Hemorrhagic shock is a medical emergency that often complicates vascular surgery and can lead to death. Hemorrhagic shock is characterized by hypoperfusion and hemodynamic abnormalities leading to the collapse of homeostasis due to massive blood loss. Early diagnosis is critical for a favorable outcome. Thromboelastometry has been considered an effective tool for bleeding management in critically ill patients. Thromboelastometry can guide transfusion therapy quickly, reducing the need for blood products. Therefore, it could be an alternative test to guide hemostatic therapy in comp

Sex-specific pharmacovigilance signals and time-to-onset patterns of drug-related myoclonus and epileptic seizures: a real-world pharmacovigilance analysis.

BACKGROUND: Drug-related myoclonus and epileptic seizures are neurological adverse events that may cause trauma, impaired consciousness, hospitalization, treatment interruption, and life-threatening outcomes. In practice, these events may be difficult to distinguish from neurological diseases, metabolic abnormalities, disease progression, or complications of comorbidities and polypharmacy. Research has focused on individual drugs, case reports, or specific populations, leaving the risk-drug spectrum, sex-related reporting patterns, and onset characteristics insufficiently characterized. Chronic Rupture of the Pectoralis Major: Current Concepts and Various Surgical Repair Techniques - A Mini Review.

INTRODUCTION: Tranexamic acid (TXA) is widely recognized for its hemostatic properties, particularly in reducing blood loss during surgical procedures. Recent studies, however, suggest that TXA may have potential analgesic benefits beyond its role in hemostasis. This study aims to evaluate the efficacy of intra-articular TXA in reducing post-operative pain and improving early recovery outcomes in patients undergoing total knee arthroplasty (TKA). MATERIALS AND METHODS: Fifty-four patients undergoing simultaneous bilateral TKA were randomly assigned to receive intra-articular TXA in one knee,
