Prevalence of Helicobacter pylori in peptic ulcer disease in a Singapore hospital.

INTRODUCTION: High Helicobacter pylori (H. pylori) prevalence is well documented among peptic ulcer patients. However, there have been recent reports of declining H. pylori infection rates in developed countries. Based on previous local data in a different hospital, H. pylori prevalence was 66% in gastric ulcer, 86% in duodenal ulcer and 75% in combined gastric and duodenal ulcers. Our present study aims to review the recent H. pylori prevalence in peptic ulcer patients in a Singapore hospital. H. pylori infection rate in relation to sex, age, non-steroidal anti-inflammatory drug (NSAID) use a

High dose omeprazole plus amoxicillin and azithromycin in eradication of Helicobacter pylori in duodenal ulcers.

BACKGROUND: The aim of our study was to establish whether one-week triple therapy regimen (omeprazole, amoxicillin, azithromycin) with low dose (2 x 20 mg/day) or high dose omeprazole (2 x 40 mg/day) is more effective in curing H. pylori infection in patients with active duodenal ulcer disease. METHODS: One hundred and twenty patients with duodenal ulcer and H. pylori infection were treated with amoxicillin 2 x 1000 mg/day for the first 7 days plus azithromycin 500 mg/day for the first 6 days. Patients were randomly assigned to receive either omeprazole 2 x 20 mg/day for the first 7 days (gro

Will Helicobacter pylori affect short-term rebleeding rate in peptic ulcer bleeding patients after successful endoscopic therapy?

OBJECTIVE: Helicobacter pylori (H. pylori) can augment the pH-increasing effect of omeprazole in patients with peptic ulcer. A high intragastric pH may be helpful in preventing recurrent hemorrhage by stabilizing the blood clot at the ulcer base of bleeding peptic ulcer patients. Therefore, we hypothesized that omeprazole may reduce short-term rebleeding rate in these patients with H. pylori infection after initial hemostasis had been obtained. METHODS: Between July 1996 and December 1998, 65 bleeding peptic ulcer patients (24 gastric ulcer, 41 duodenal ulcer) who had obtained initial hemosta

Detection and typing of the virulence determinants cagA and vacA of Helicobacter pylori directly from biopsy DNA: are in vitro strains representative of in vivo strains?

BACKGROUND: The relationship of Helicobacter pylori genotypes to gastrointestinal disease has relied on cultured isolates. This assumes that cultured strains are representative of in vivo strains. OBJECTIVE: To detect and type the cagA status and the vacA genotypes directly from biopsy DNA without the need for culture, and to further define the relationship between H. pylori genotypes and gastroduodenal pathology. METHODS: Fifty-two Caucasian patients undergoing routine endoscopy for dyspepsia had additional biopsies taken from four gastric sites and one duodenal site for biopsy DNA preparat

Eradication of Helicobacter pylori by a 1-week course of famotidine, amoxicillin and clarithromycin.

OBJECTIVES: The combination of a proton pump inhibitor (PPI) such as omeprazole with amoxicillin and clarithromycin constitutes one of the most effective treatments for the eradication of Helicobacter pylori. Nevertheless, the mechanisms of interaction between these drugs remain unclear. It has been shown that minimal inhibitory concentration values of both antibiotics are considerably lower at neutral pH levels than in an acid environment. Further, omeprazole possesses bacteriostatic activity. To evaluate the significance of these mechanisms we replaced omeprazole with famotidine, a drug whic

T helper 1 effector cells specific for Helicobacter pylori in the gastric antrum of patients with peptic ulcer disease.

Chronic antral gastritis following Helicobacter pylori (Hp) infection is characterized by a cellular inflammatory infiltrate whose cytokines may represent a host-dependent factor influencing the outcome of the infection. The pattern of cytokines produced by the immunologically active cells in the gastric antrum was analyzed at the mRNA level in antral biopsies from five Hp-infected patients with duodenal ulcer and three Hp-negative dyspeptic controls. T cell clones were generated from parallel antral biopsies of the same Hp-infected patients and assessed for reactivity to Hp Ags, cytokine prof

L-ascorbic acid exerts anti-microbial and anti-virulence effects against multi-drug resistant Burkholderia cepacia complex.

Burkholderia cepacia complex (Bcc) encompasses a group of multi-drug resistant, Gram-negative bacilli that poses a significant threat to individuals with cystic fibrosis, and the immunocompromised population. Due to limitations with available anti-microbial therapeutics, alternative strategies using synthetic chemical compounds are widely being explored. Here, we evaluated the anti-microbial, anti-biofilm and anti-virulence effects of L-ascorbic acid against a clinical isolate of the B. cepacia complex (henceforth designated as B. cepacia for the sake of simplicity). Antibiotic susceptibility

Microalgal fertilizers as sustainable alternatives to chemical fertilizers: Enhancing pepper yield and soil health through reduced synthetic input strategies.

Long-term excessive application of chemical fertilizers (CF) has led to a decline in soil fertility and the deterioration of soil health. Consequently, developing eco-friendly microbial fertilizers as alternatives to chemical inputs has become a global consensus for promoting sustainable agriculture. To explore the underexplored potential and soil-modulating mechanisms of microalgae-based fertilizers, this study investigates their feasibility of microalgae-based fertilizers as CF substitutes for enhancing the yield and quality of peppers (Capsicum annuum L.). We compared the application effect

Isolation and characterization of Candida metapsilosis from foci of chronic pododermatitis in captive steppe eagles in Kazakhstan.

Pododermatitis (bumblefoot) is a chronic, debilitating disease of the plantar surface of the foot that affects birds of prey kept in captivity worldwide. Although bacterial pathogens, especially Staphylococcus aureus, are most commonly considered as causative agents, the contribution of opportunistic yeasts to chronic, non-healing footpad lesions remains poorly characterized. Keratinophilic yeasts may sustain the disease process by degrading keratin in superficial tissues, impairing wound healing and, owing to their thermotolerance and minimal nutritional requirements, persisting in the enviro

Right Knee Ureaplasma Septic Arthritis in an Immunocompromised Host, Requiring Right Hip Disarticulation for Definitive Source Control.

INTRODUCTION: Ureaplasma urealyticum is a fastidious, urease-producing bacterium typically responsible for urogenital infections. In immunocompromised hosts, a full-thickness mucosal breach may be complicated by hematogenous spread, leading to septic arthritis in distant joints U. urealyticum is difficult to isolate on routine culture and requires special media or 16S rRNA polymerase chain reaction for detection. CASE REPORT: We report a 26-year-old immunocompromised female with neuromyelitis optica on long-term cyclosporine and Grave's disease, who developed chronic right knee U. urealyticum

Helicobacter pylori infection promotes the formation of the β-catenin/HIF-1α complex, enabling adaptive responses in gastric cancer cells.

Helicobacter pylori is a gastric pathogen associated with the development of gastric cancer. By attaching to the gastric epithelium, it triggers signaling pathways that lead to effects ranging from apoptosis to cell proliferation. H. pylori has been shown to promote nuclear translocation of β-catenin, inducing gene expression related to the cell cycle. However, recent studies indicate it also causes cell cycle arrest by stabilizing hypoxia-inducible factor 1-alpha (HIF-1α). The mechanisms underlying these opposing effects remain unknown. Here, we explored the effects of H. pylori infection on

Development and Evaluation of a Multi-Epitope Vaccine Based on P22 Virus-Like Particles Targeting Helicobacter pylori.

BACKGROUND: The current first-line treatment for Helicobacter pylori (H. pylori) infection is bismuth-based quadruple therapy. However, the widespread use of antibiotics has contributed to the emergence of antibiotic-resistant strains, thereby increasing the likelihood of treatment failure. This study aimed to develop a multi-epitope H. pylori vaccine, designated P22-C3, based on P22 Virus-like Particles, with the objective of reducing infection rates and preventing transmission. MATERIALS AND METHODS: The structural characteristics of P22-C3 were analyzed using transmission electron microsco
