Efficacy of Topical Nepafenac Versus Nepafenac With Perioperative Cyclopentolate for Pain Management After Photorefractive Keratectomy: A Comparative Clinical Evaluation.

PURPOSE: To compare the efficacy of topical nepafenac alone versus nepafenac combined with cyclopentolate for pain management after photorefractive keratectomy (PRK) through a comprehensive evaluation of pain intensity, stinging, tearing, light sensitivity, analgesic use, and sleep disturbance. METHODS: This was a retrospective comparative study using data from a randomized controlled trial registry. A total of 160 patients undergoing PRK were analyzed. Patients were randomized to receive a standard nepafenac regimen either alone or with the addition of perioperative cyclopentolate 1%. Pain,

Crosslinking for post refractive surgery ectasia: application and clinical outcomes.

PURPOSE: Post-laser vision correction ectasia (PLE) is one of the more serious complications that can happen after laser vision correction surgery, manifested as progressive thinning of the cornea and accompanied by increased myopia and irregular astigmatism. We present a case series of PLE managed with pachymetry-guided Epithelium-Off accelerated crosslinking (ACXL) using the M nomogram, in a long term-follow-up evaluation. METHODS: Before-after comparative case series of 13 eyes of 10 patients (3 bilateral treatments) diagnosed with PLE underwent standardized over-the-Flap crosslinking tech

Motorized 0.8-mm micropunch grafting for refractory vitiligo: A retrospective study of 230 cases.

BACKGROUND: Punch grafting for vitiligo is time-consuming and can result in cobblestone-like appearances. We devised a motorized 0.8-mm micropunch grafting procedure to overcome these limitations. OBJECTIVE: To assess the therapeutic effectiveness and adverse events associated with micropunch grafting in refractory vitiligo. METHODS: We retrospectively reviewed 230 cases in 208 patients with stable vitiligo who underwent motorized 0.8-mm micropunch grafting during January 2015-August 2017. Treatment success was defined as ≥75% repigmentation, and factors associated with the outcome were asse

Comparative study between excimer light and topical antioxidant versus excimer light alone for treatment of vitiligo.

BACKGROUND: Vitiligo is an acquired idiopathic cutaneous disease characterized by pearly white patches of variable shapes and sizes. Various medical and surgical therapeutic options have been proposed to achieve repigmentation; phototherapy is one of the most efficient options. Topical therapies have been a mainstay of vitiligo treatment, with or without phototherapy. AIM OF THE WORK: To compare the efficacy of combined topical antioxidant hydrogel and excimer light versus excimer light alone in treating vitiligo. PATIENTS AND METHODS: Thirty patients were included in this comparative, prosp

Endovascular management of multiple arteriovenous fistulae following failed laser-assisted pacemaker lead extraction.

A woman presented for evaluation of new-onset left arm edema after failed laser-assisted pacemaker lead extraction. Initial workup demonstrated a left subclavian artery to vein arteriovenous fistula (AVF). She underwent repair of the AVF with placement of a covered stent in the subclavian artery, however, her symptoms did not completely resolve. Investigation revealed a left common carotid artery to left innominate vein AVF, which was repaired by deploying a covered stent retrograde into the left common carotid artery. Her symptoms subsequently resolved. Multiple iatrogenic AVF can be repaired

Prospective Objective Analysis of Corneal Haze Following Customized Transepithelial PRK Without Mitomycin C Combined With Accelerated Corneal Cross-Linking Versus Corneal Cross-Linking Alone.

PURPOSE: To compare haze and refractive outcomes in patients undergoing combined accelerated corneal cross-linking (A-CXL) and selective wavefront-guided transepithelial photorefractive keratectomy (WG-transPRK) without mitomycin C (MMC) versus those undergoing A-CXL. METHODS: This prospective study analyzed 95 eyes (86 patients) with progressive keratoconus from October 2018 to October 2022. The first group underwent CXL combined with corneal or ocular WG-transPRK (CXL+PRK, n = 52), targeting higher order aberrations (HOAs). The second underwent CXL only (n = 43), both following the same acc

Contemporary management of critical lower limb ischemia in TASC D lesions with subintimal angioplasty in femoro-popliteal lesions, tibial angioplasty and sequential compression biomechanical device for infra-inguinal arterial occlusion. Experience and quality of life outcome learned over 25 years.

AIM: Patients with end-stage critical limb ischemia (CLI) survive on borrowed time and amputation is inevitable if an aggressive management stratagem is not instigated. Our primary aim was to equate effectiveness of subintimal angioplasty (SIA) and tibial balloon angioplasty (TBA) in sustaining clinical improvement and amputation free survival (AFS) in patients with CLI TASD II D. Moreover, patients with severe CLI, who were not suitable for revascularization and who were offered therapy with a sequential compression biomechanical device (SCBD) were scrutinised as part of a comprehensive lower

Current approach to the diagnosis and treatment of femoral-popliteal arterial disease. A systematic review.

Peripheral arterial disease (PAD) is a common manifestation of atherosclerosis affecting 5 million adults in the United States, with an age-adjusted prevalence of 4% to 15% and increasing up to 30% with age and the presence of cardiovascular risk factors. In this article we focus on lower extremity PAD and specifically on the superficial femoral and proximal popliteal artery (SFPA), which are the most common anatomic locations of lower extremity atherosclerosis. We summarize current evidence and perform a systematic review on the diagnostic evaluation as well as the medical, endovascular and s

Impact of graft diameter on corneal power and the regularity of postkeratoplasty astigmatism before and after suture removal.

OBJECTIVE: To assess the impact of graft diameter on corneal curvature before and after removal of a double-running suture after nonmechanical penetrating keratoplasty (PK). DESIGN: Prospective, nonrandomized, comparative (self-controlled) single-center clinical trial. PATIENTS: Four hundred eighty-nine eyes with "two sutures in" and 308 eyes with "all sutures out" (mean age, 52+/-19 years) were included. The diagnoses were keratoconus (48%), Fuchs' and stromal dystrophies (31%), aphakic or pseudophakic bullous keratopathy (11%), and scars (10%). INTERVENTIONS: In all eyes, a central trephi

PURPOSE: To demonstrate a mathematical method for multiscalar decomposition of discrete corneal topography height data into a space-scale space using wavelet analysis techniques, and to demonstrate the clinical applicability of these computations in the postkeratoplasty cornea. METHODS: Fifty patients with either Fuchs' dystrophy (n = 20) or keratoconus (n = 30) were seen preoperatively, at 3 months, at 1 year (before suture removal) and again at 19 +/- 3 months (after suture removal) following nonmechanical trephination with an excimer laser for penetrating keratoplasty. BACKGROUND/AIM: Photorefractive keratectomy (PRK), after laser in situ keratomileusis (LASIK), is commonly performed refractive surgical method worldwide. The aim of this study was to examine the effectiveness and safety of PRK in correction of various strengths of myopia and to assess how much corneal tissue is being removed with one diopter sphere (Dsph) correction by using different optical zones (OZ). METHODS: A prospective study with a follow-up period of 6 months included 55 patients of which 100 myopic eyes were treated by PRIK method (one eye was included in 10 patients). Myopic eyes
