Ultrasound-guided injection of a corticosteroid and hyaluronic acid: a potential new approach to the treatment of trigger finger.

BACKGROUND AND OBJECTIVES: Stenosing tenosynovitis (trigger finger) is one of the most common causes of pain and disability in the hand, which may often require treatment with anti-inflammatory drugs, corticosteroid injection, or open surgery. However, there is still large room for improvement in the treatment of this condition by corticosteroid injection. The mechanical, visco-elastic, and antinociceptive properties of hyaluronic acid may potentially support the use of this molecule in association with corticosteroids for the treatment of trigger finger. This study examines the feasibility an

The enthesis organ concept and its relevance to the spondyloarthropathies.

A characteristic feature of the spondyloarthropathies is inflammation at tendon or ligament attachment sites. This has traditionally been viewed as a focal abnormality, even though the inflammatory reaction intrinsic to enthesitis may be quite extensive. We argue that the diffuse nature of the pathology is best understood in the context of an 'enthesis organ concept'. This highlights the fact that stress concentration at an insertion site involves not only the enthesis itself, but neighbouring tissues as well. The archetypal enthesis organ is that of the Achilles tendon where intermittent cont

Could rheumatoid arthritis result from an abnormal T cell response towards lubricin/superficial zone protein?

The lack of some suppressor T cells (including TCD4+CD25+(high) positively selected first in thymic medulla) specific to a restricted set of autoantigens may be the common link between all patterns of rheumatoid arthritis. In other words, instead of a 'peak' of TCD4+ effector T cells common to all patients with rheumatoid arthritis (which has so far never been demonstrated), a 'hole' in TCD4+CD25+(high) responses towards a limited set of autoantigens responsible for the normal maintenance of tolerance within the joints may be shared by many patients with rheumatoid arthritis. The hallmark of t

[Differential therapy for the rheumatoid shoulder].

Despite frequent involvement, the rheumatoid shoulder is neglected in operative treatment of the upper extremities. The slow course of omarthritis, the compensation mechanism of scapulothoracic motion and neighbouring joints as well as dominating disabilities of the lower extremities and the rheumatoid hand are possible explanations. The pattern of destruction of the rheumatoid shoulder is characterized by progressive joint and soft tissue deterioration. Soft tissue involvement determines the course of the shoulder joint. The subacromial space is a common and early site for rheumatoid involvem

Overload syndromes of the peritalar region.

INTRODUCTION: The purpose of this study was to identify the diagnostic possibilities of the different diagnostic techniques in the evaluation of the pathological conditions affecting the peritalar region. MATERIALS AND METHODS: Between September 1995 and December 1996, 58 patients, with a painful syndrome at peritalar level were submitted to MRI, which was performed using a 'dedicated system' consisting of 0.2 and 0.5 T equipment. Spin-Echo T1-weighted and Gradient-Echo T2-weighted sequences were used on sagittal, coronal and axial planes, with slice thickness of 3-5 mm. Conventional radiogra

Transthyretin Amyloid Cardiomyopathy Risk Evaluation in a Cohort of Patients With Heart Failure.

Introduction Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive, infiltrative form of heart failure (HF). Nevertheless, ATTR-CM is a largely underrecognized and misdiagnosed condition. This study's objective was to develop an efficient model to assess the chance of ATTR-CM in patients with HF. Methods This was an observational study of patients with HF who had a confirmed diagnosis of ATTR-CM and those with HF but without known ATTR-CM between January 1, 2019, and July 1, 2021. Patient characteristics were extracted from administrative and claims electronic databases and compared

Case studies in the diagnosis of upper extremity pain using magnetic resonance imaging.

The diagnosis of upper extremity pain can be a daunting task for the examining physician. The differential diagnosis for pain at each level in the upper extremity is often extensive and diverse. Quite frequently, the clinical signs and symptoms of multiple pathologic conditions are overlapping and nonspecific. The decision to treat conservatively or with surgery often depends on the magnetic resonance imaging (MRI) findings. MRI's ability to produce multiplanar images with excellent soft tissue contrast has made it an indispensable tool for diagnosis and treatment planning in patients with upp

Multiple myeloma presenting with amyloid arthropathy as the first manifestation: Two case reports.

BACKGROUND: Multiple myeloma (MM) can be accompanied by amyloidosis, which occurs in a small number of patients and is characterized by deposition of light chains in the joints, leading to multiple myeloma-associated amyloid arthropathy (MAA). As a rare complication of MM, clinical manifestations of MAA are often similar to those of rheumatoid arthritis, and the two are easily confused. CASE SUMMARY: In recent years, our center treated two patients of MM with amyloid arthropathy as the first manifestation, both of whom presented with polyarthritis. After treatment for MM, both patients achiev

The results of surgical treatment of trigger finger.

A three-part retrospective study was undertaken to review the long-term results of surgical treatment of trigger finger. Seventy-five patients were identified by chart review. Fifty-nine of these were assessed by a telephone survey, with a mean follow-up period of 48 months (range, 6-70 months). Forty-six patients (78%) underwent follow-up physical examination. Surgical treatment was successful in all patients. Ninety-seven percent of patients had complete resolution of triggering, and the rest had significant improvement of symptoms. The recurrence rate was 3%, with only a single patient requ

Abatacept inhibits inflammation and onset of rheumatoid arthritis in individuals at high risk (ARIAA): a randomised, international, multicentre, double-blind, placebo-controlled trial.

BACKGROUND: Individuals with anti-citrullinated protein antibodies (ACPAs) and subclinical inflammatory changes in joints are at high risk of developing rheumatoid arthritis. Treatment strategies to intercept this pre-stage clinical disease remain to be developed. We aimed to assess whether 6-month treatment with abatacept improves inflammation in preclinical rheumatoid arthritis. METHODS: The abatacept reversing subclinical inflammation as measured by MRI in ACPA positive arthralgia (ARIAA) study is a randomised, international, multicentre, double-blind, placebo-controlled trial done in 14 h

Common Fascial Abnormalities in Five Thumb Pain Conditions: A Fifteen-Point Ultrasound-Guided Fascia Hydrorelease Clinical Protocol.

Background: Thumb pain frequently arises from multiple conditions-de Quervain's tenosynovitis, carpometacarpal (CMC) osteoarthritis, carpal tunnel syndrome, intersection syndrome, and myofascial pain syndrome-that often coexist yet are traditionally managed as separate entities. Fascial abnormalities, including hyaluronic acid-related densification and impaired interlayer gliding, have been proposed to constitute a shared pathological substrate underlying these conditions. Ultrasound-guided fascia hydrorelease (US-FHR) targeting fascial pathology has been used clinically for thumb-wrist pain;

The clinical features of rheumatoid arthritis.

Rheumatoid arthritis (RA) is a chronic inflammatory disease characterized by progressive damage of synovial-lined joints and variable extra-articular manifestations. Tendon and bursal involvement are frequent and often clinically dominant in early disease. RA can affect any joint, but it is usually found in metacarpophalangeal, proximal interphalangeal and metatarsophalangeal joints, as well as in the wrists and knee. Articular and periarticular manifestations include joint swelling and tenderness to palpation, with morning stiffness and severe motion impairment in the involved joints. The cli

Prosthetic Joint Infection due to Mycobacterium avium-intracellulare in a Patient with Rheumatoid Arthritis: A Case Report and Review of the Literature.

Nontuberculous mycobacteria (NTM) are a rare cause of prosthetic joint infections (PJI). However, the prevalence of NTM infections may be increasing with the rise of newer immunosuppressive medications such as biologics. In this case report, we describe a rare complication of immunosuppressive therapies and highlight the complexity of diagnosing and treating PJI due to NTM. The patient is a 79-year-old Caucasian male with a history of severe destructive rheumatoid arthritis on several immunosuppressive agents and right hip osteoarthritis s/p total hip arthroplasty 15 years previously with seve

Rilzabrutinib, an oral Bruton tyrosine kinase inhibitor, in patients with warm autoimmune haemolytic anaemia: a prospective, single-arm, multicentre, phase 2b safety and clinical activity study (LUMINA 2).

BACKGROUND: Warm autoimmune haemolytic anaemia is a rare blood disorder characterised by haemolysis, anaemia, elevated thromboembolism risk, fatigue, and impaired quality of life. The selective Bruton tyrosine kinase inhibitor, rilzabrutinib, might increase haemoglobin through multi-immune modulatory mechanisms targeting key drivers of warm autoimmune haemolytic anaemia pathophysiology. The aim of the study was to report clinical safety and activity results for rilzabrutinib in patients with warm autoimmune haemolytic anaemia. METHODS: LUMINA 2, a multicentre, single-arm, open-label, phase 2b

Diagnostic accuracy of musculoskeletal ultrasound in suspected polymyalgia rheumatica: a multicentre, cohort study.

BACKGROUND: Diagnosing polymyalgia rheumatica is challenging, and musculoskeletal ultrasound is primarily used in diagnostically difficult situations. Previous musculoskeletal ultrasound studies in patients with polymyalgia rheumatica used case-control designs or non-representative cohorts, reducing assessment of diagnostic accuracy. This study aimed to assess the diagnostic accuracy of musculoskeletal ultrasound in two independent cohorts of patients with suspected polymyalgia rheumatica. METHODS: This multicentre, diagnostic accuracy study included consecutive, glucocorticoid-naive patients

Whole-body MRI in patients with arthralgia or inflammatory arthritis after exposure to immune checkpoint inhibitors: a single-centre prospective imaging study.

BACKGROUND: Musculoskeletal adverse events due to immune checkpoint inhibitors are common and can present clinically as inflammatory arthritis, polymyalgia rheumatica, or arthralgia. The pathoanatomy of musculoskeletal adverse events related to immune checkpoint inhibitors remains undefined, with a paucity of available imaging data. We aimed to investigate the whole-body imaging phenotype of arthralgia and inflammatory arthritis following exposure to immune checkpoint inhibitors, to fully characterise the pattern of inflammation in these patients and subsequently inform clinical management. Sonography of the musculoskeletal system in dogs and cats.

Sonography of the musculoskeletal system in dogs and cats was undertaken to evaluate the application of this imaging procedure in orthopedics. In most of the patients a 7.5 MHz linear transducer was used because of its flat application surface and its resolving power. The evaluation of bone by sonography is limited, but sonography can provide addition information regarding the bone surface and surrounding soft tissue. Ultrasound is valuable for assessing joint disease. Joint effusion, thickening of the joint capsule and cartilage defects can be identified sonographically. It is also possible t
