The AltaValve system for transcatheter mitral valve replacement: A narrative review of early safety and efficacy data.

Mitral regurgitation (MR) is a prevalent and progressive condition associated with poor outcomes in high-risk patients who are often ineligible for surgery or transcatheter repair. The AltaValve system (4C Medical Technologies, Maple Grove, MN), a novel transcatheter mitral valve replacement (TMVR) device, employs a unique supra-annular, atrial-only fixation mechanism designed to minimize left ventricular outflow tract (LVOT) obstruction and paravalvular leak. This narrative review summarizes current clinical evidence on AltaValve, including case reports, single-center, and multicenter experie

Prevalence of iron deficiency and its influence on six-minute walk test distance in patients eligible for transcatheter aortic valve implantation. A prospective study.

INTRODUCTION: Aortic stenosis (AS) leads to left ventricular outflow tract obstruction and can result in the development of heart failure. Patients with severe AS exhibit similar symptoms to those with iron deficiency (ID), raising the question of whether ID, if diagnosed, could be linked to the lack of physical performance improvement after transcatheter aortic valve implantation (TAVI). AIM: The aim was to assess the prevalence of ID in patients scheduled for TAVI and to evaluate its impact on 6-minute walk test (6MWT) results. MATERIAL AND METHODS: Patients with severe AS, qualified for T

Long-term Surgical Outcomes of Supravalvar Aortic Stenosis: Modified Simple Sliding Aortoplasty.

This study investigated long-term outcomes and factors associated with reoperations in patients who underwent surgical repair of congenital supravalvar aortic stenosis (SVAS). A total of 39 consecutive patients who underwent congenital SVAS repair from 1999 through 2018 were included. Aortic root geometry was evaluated by measuring the ratio of the sinotubular junction diameter to the aortic annulus diameter (STJ/AVA) on echocardiography and proportion of intercommissural distance (ICD) of each sinus on computed tomography. The median age and weight at the time of operation were 4.3 years and

Canadian Cardiovascular Society 2022 Guidelines for Cardiovascular Interventions in Adults With Congenital Heart Disease.

Interventions in adults with congenital heart disease (ACHD) focus on surgical and percutaneous interventions in light of rapidly evolving ACHD clinical practice. To bring rigour to our process and amplify the cumulative nature of evidence ACHD care we used the ADAPTE process; we systematically adjudicated, updated, and adapted existing guidelines by Canadian, American, and European cardiac societies from 2010 to 2020. We applied this to interventions related to right and left ventricular outflow obstruction, tetralogy of Fallot, coarctation, aortopathy associated with bicuspid aortic valve, a

Novel Transcatheter Mitral Prosthesis Designed to Preserve Physiological Ventricular Flow Dynamics.

BACKGROUND: Current mitral bioprostheses are akin to the aortic valve and therefore abolish the left ventricular (LV) physiological vortex. We evaluated the hemodynamic performance and the effects on intraventricular flow dynamics (IFD) of a novel mitral bioprosthesis that presents an innovative design mimicking the native valve. METHODS: A D-shaped self-expandable stent-bovine pericardium monoleaflet valve was designed to provide physiological asymmetric intraventricular flow. Transapical implantation was consecutively performed in 12 juvenile sheep. Postimplant studies using Doppler echocar

Evaluation of subvalvular aortic stenosis in children: a 16-year single-center experience.

Subvalvular aortic stenosis accounts for 1-2 % of all congenital heart disease and for 8-20 % of cases of left-ventricular outflow tract (LVOT) obstruction in children. Recurrence of subaortic stenosis (SAS) is not uncommon after surgical management. This study was performed to investigate the clinical and surgical outcomes and to estimate the predictability of recurrences of SAS. Seventy-nine patients age 3-21 years with SAS between 1994 and 2010 were reviewed. Fifty-one patients had discrete SAS, whereas the remaining 15 patients had fibromuscular ridge-type SAS. Mean follow-up time without

Risk stratification and role of implantable defibrillators for prevention of sudden death in patients with hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is the most common cause of sudden cardiac death (SCD) in young people, including trained athletes. It is now 30 years since the introduction of implantable cardioverter-defibrillators (ICDs) to clinical cardiovascular practice and coronary artery disease, and now device therapy represents the most significant therapeutic innovation and the only definitive strategy for prolonging the life of HCM patients. ICDs have proved effective in preventing SCD in young HCM patients with appropriate intervention rates of 11% for secondary and 4% for primary prevention, de

Recognition and management of accessory mitral tissue causing severe subaortic stenosis.

Failure to recognize the presence of accessory mitral tissue causing subaortic stenosis can lead to not only the performance of inappropriate operations, but the persistence and recurrence of obstruction or even death. Over a 12-month period, we treated 2 children with severe subaortic stenosis caused by accessory mitral tissue. In 1 patient, who was 4 years old, the echocardiogram showed the accessory mitral tissue to be attached to the anterior mitral leaflet and ballooning into the subaortic area. The other patient, as a newborn, underwent simultaneous repair of a complete canal defect and

Flow inefficiencies in non-obstructive HCM revealed by kinetic energy and hemodynamic forces on 4D-flow CMR.

AIMS: Patients with non-obstructive hypertrophic cardiomyopathy (HCM) exhibit myocardial changes which may cause flow inefficiencies not detectable on echocardiogram. We investigated whether left ventricular (LV) kinetic energy (KE) and hemodynamic forces (HDF) on 4D-flow cardiovascular magnetic resonance (CMR) can provide more sensitive measures of flow in non-obstructive HCM. METHODS AND RESULTS: Ninety participants (70 with non-obstructive HCM and 20 healthy controls) underwent 4D-flow CMR. Patients were categorized as phenotype positive (P+) based on maximum wall thickness (MWT) ≥ 15 mm o

Impact of Severe Postoperative Multi-Organ Dysfunction on Long-Term Outcomes After Modified Morrow Surgery.

BACKGROUND: The modified Morrow surgery is the gold standard surgical treatment for hypertrophic obstructive cardiomyopathy (HOCM). While effective, it carries a risk of postoperative severe multi-organ dysfunction and its impact on long-term cardiovascular outcomes remains poorly understood and controversial. METHODS: This observational cohort study included 380 HOCM patients undergoing the modified Morrow surgery between July 2015 and March 2025. Severe multi-organ dysfunction was defined as a maximum Sequential Organ Failure Assessment (SOFA) score ≥ 11. The primary endpoint was the long-t

Mavacamten-Associated Temporal Changes in Left Atrial Function in Obstructive HCM: Insights From the VALOR-HCM Trial.

BACKGROUND: In severely symptomatic patients with obstructive hypertrophic cardiomyopathy (HCM), the VALOR-HCM (A Study to Evaluate Mavacamten in Adults With Symptomatic Obstructive Hypertrophic Cardiomyopathy Who Are Eligible for Septal Reduction Therapy) trial showed that mavacamten reduced the eligibility for septal reduction therapy with sustained improvement in left ventricular outflow tract gradients. Mavacamten also resulted in favorable cardiac remodeling, including improvement in biomarkers (eg, N-terminal pro-B-type natriuretic peptide and troponin T). However, the impact of mavacamt

Cleft-to-commissure conversion in atrioventricular septal defect using artificial chordae: Technical report and case illustration.

BACKGROUND: Primary repair of complete atrioventricular septal defect (cAVSD), typically performed at age 3 to 6 months, is associated with favorable early survival and long-term outcomes. However, a significant incidence of reoperation persists, predominantly for left atrioventricular valve (LAVV) regurgitation (LAVVR) and left ventricular outflow tract obstruction. Optimal management of the LAVV cleft remains controversial. Although routine cleft closure is frequently used to mitigate progressive LAVVR and subsequent reoperation, this approach can be complicated by LAVV stenosis, especially
