Cesarean delivery in the hybrid operating suite: a promising new location for high-risk obstetric procedures.

BACKGROUND: The increasing cesarean delivery rate and attendant placental implantation abnormalities, coupled with increasing general medical complexity in the obstetric population, has driven innovation to optimize the care of high-risk parturients during delivery. Novel and multidisciplinary approaches and locations may enhance the options available for care. METHODS: We reviewed the records of all 11 patients who underwent cesarean delivery in our hybrid operating suite between December 2007 and March 2013 and describe the high-risk cesarean deliveries. RESULTS: The most common indication

Pregnancy outcomes according to increasing maternal age.

OBJECTIVES: To investigate the risks of increasing maternal age on the perinatal and obstetric outcomes. MATERIALS AND METHODS: Information about 29,760 singleton pregnancies delivered between 2005 and 2008 was extracted from our database. Patients were categorized into four groups according to age: 20-29 years, 30-34 years, 35-39 years, and ≥40 years. Multivariable logistic regression analysis was used to evaluate the adjusted odd ratios (AORs) of adverse pregnancy outcomes according to maternal age after adjusting for parity, body mass index, medical history and use of in vitro fertilizatio

Breech presentation: Clinical practice guidelines from the French College of Gynaecologists and Obstetricians (CNGOF).

OBJECTIVE: To determine the optimal management of singleton fetuses in breech presentation. MATERIALS AND METHODS: Consultation of the PubMed database, the Cochrane Library and guidelines issued by the French and foreign obstetrical societies or colleges. RESULTS: In France, 5% of women have breech deliveries (level of evidence [LE] 3). One third of them have a planned vaginal delivery (LE3), and 70% of these give birth vaginally (LE3). External cephalic version (ECV) is associated with lower rates of both breech presentation at birth (LE2) and of cesarean deliveries (LE3) without any increa

Elective use of aortic balloon occlusion in cesarean hysterectomy for placenta previa percreta.

BACKGROUND: Only few reports are available on the use of aortic balloon catheter for cesarean hysterectomy in placenta previa percreta. CASE: A 32-year-old woman with placenta previa percreta underwent cesarean hysterectomy at 34 weeks of gestation. Before starting the surgery, an aortic occlusion balloon catheter (30 mm balloon, 5 Fr) was inserted. For total hysterectomy, the aortic balloon catheter was inflated and there was a sudden and dramatic reduction in blood loss, and the surgery was completed safely. An aortic occlusion was sustained for 80 min, with blood loss estimated at 3,200 ml

Tranexamic acid administered during cesarean delivery in high-risk patients: maternal pharmacokinetics, pharmacodynamics, and coagulation status.

BACKGROUND: Tranexamic acid is frequently administered for postpartum hemorrhage. The World Health Organization recommends 1 g intravenous dosing, repeated once after 30 minutes for ongoing bleeding. Understanding the pharmacokinetics and pharmacodynamics of tranexamic acid in patients at high risk of postpartum hemorrhage may enable dosage tailoring for optimal antifibrinolysis with minimal adverse events, such as thrombosis or renal cortical necrosis. OBJECTIVE: This study aimed to report tranexamic acid pharmacokinetics and pharmacodynamics after 1 g intravenous dosing during cesarean deli

Outcome of cesarean scar ectopic pregnancy continued to viability: data from the international cesarean scar ectopic pregnancy registry.

BACKGROUND: Cesarean scar ectopic pregnancy is a rare but serious complication of a prior cesarean section, in which the gestational sac implants within the previous uterine cesarean scar. Cesarean scar ectopic pregnancy carries a substantial risk of severe maternal morbidity, including life-threatening hemorrhage and hysterectomy. OBJECTIVE: The aim of this study is to describe the course of cesarean scar ectopic pregnancies that were managed expectantly and identify which ultrasound findings were associated with peripartum hysterectomy. STUDY DESIGN: An international registry was establish

Identifying systemic vulnerability phenotypes associated with programmed regimens in frozen embryo transfer: a secondary analysis of a multicentre randomized clinical trial.

BACKGROUND: Programmed endometrial preparation is widely used for frozen embryo transfer (FET) because it allows flexible scheduling. Recent randomized evidence has shown that programmed regimens increase the risk of hypertensive disorders of pregnancy, particularly pre-eclampsia, compared with natural ovulation regimens. However, it remains unclear whether this risk is distributed uniformly across patients or concentrated in women with specific baseline characteristics. OBJECTIVE: This study aimed to evaluate the heterogeneous treatment effects of programmed regimens versus natural ovulation

The epidemiology, clinical burden, and prevention of intrauterine adhesions (IUAs) related to surgically induced endometrial trauma: a systematic literature review and selective meta-analyses.

BACKGROUND: Reproductive-age women with intrauterine adhesions (IUAs) following uterine surgery may be asymptomatic or may experience light or absent menstruation, infertility, preterm delivery, and/or peripartum hemorrhage. Understanding procedure- and technique-specific risks and the available evidence on the impact of surgical adjuvants is essential to the design of future research. OBJECTIVE AND RATIONALE: While many systematic reviews have been published, most deal with singular aspects of the problem. Consequently, a broadly scoped systematic review and selective meta-analyses identifyi

A new technique for uterine incision closure at the time of cesarean section: does it make a difference?

Источник пометил публикацию как отозванную

The purpose of this study was to compare the short-term operative outcomes of three different surgical techniques for uterine incision closure during caesarean section (CS). This trial enrolled 120 patients scheduled for primary caesarean delivery. Patients were randomised into either classical double-layer uterine closure, purse-string double-layer uterine closure (Turan), or our new approach of uterine closure (double layer step up-step down technique). For short-term comparison, transvaginal ultrasonography was planned for all patients 6 weeks after surgery. Compared to group II and Group I

Uterine wound healing after caesarean section: A systematic review.

The rate of caesarean section (CS) is increasing worldwide. Defects in uterine healing have a major gynaecological and obstetric impact (uterine rupture, caesarean scar defect, caesarean scar pregnancy, placenta accreta spectrum). The complex process of cellular uterine healing after surgery, and specifically after CS, remains poorly understood in contrast to skin wound healing. This literature review on uterine wound healing was mainly based on histological observations, particularly after CS. The primary objective of the review was to examine the effects of CS on uterine tissue at the cellul
