Comparative Diagnostic Utility of IS6110 PCR Assay in CSF and Peripheral Blood Samples of Tuberculous Meningitis Patients: A Pilot Study from Central India.

INTRODUCTION: Tuberculous Meningitis (TBM) is the most severe form of Central Nervous System Tuberculosis (CNS-TB) and constitutes about 6% of all Extrapulmonary TB (EPTB) cases. Most guidelines for the diagnosis and management of TBM agree on the use of simple Cerebrospinal Fluid (CSF) analysis using molecular tools like Polymerase Chain Reaction (PCR). However, the sensitivity of PCR varies while using a CSF sample. In the present study, we have compared the diagnostic utility of PCR assay in both peripheral blood and CSF sample collected from TBM cases. AIM: To evaluate the application of

Tuberculosis: a resurgent disease in immunosuppressed patients.

A total of 136 patients, 67 HIV, 69 diabetes mellitus (DM) with or without (+/-) end-stage renal disease (ESRD), were registered for tuberculosis treatment at the National Tuberculosis Center (NTBC) from May to December, 2003. Ages ranged from 21-78 years (median 57.7 years) in TB/DM patients, and 21-62 (mean 37.6 +/- 8.3 years) in TB/HIV patients. TB was significantly found in younger and single HIV patients, but in older and married DM patients (p<0.05). Male patients in both groups were strongly associated with TB, while females more commonly had TB with DM (p<0.05). The majority of these p

Identifying people's energy information needs through Energy Citizen Personas.

UNLABELLED: Renal tuberculosis (TB) is an extrapulmonary form of TB, and is becoming more common and challenging to treat. This is because many anti-tubercular drugs have nephrotoxicity and have poor drug penetration into renal tissues. These problems highlight the urgent need for safer, kidney-targeted treatment regimes, particularly in patients with compromised kidney function. Thus, this study investigates lysozyme, which is a low-molecular-weight protein and has natural kidney-affinity, as a potential carrier for targeted renal drug delivery. Here, a comprehensive in silico approach has be

Utility of cell-free DNA in diagnosing tuberculous pleurisy: A systematic review and meta-analysis protocol.

BACKGROUND: Tuberculous pleurisy is the most common form of extrapulmonary tuberculosis. Diagnosis remains challenging due to the paucibacillary nature of pleural effusions, leading to low sensitivity of conventional microbiological methods and frequent reliance on invasive biopsy. Cell-free DNA (cfDNA), comprising fragmented genetic material released from host cells and pathogens into biofluids, presents a promising minimally-invasive biomarker. This protocol outlines a systematic review and meta-analysis designed to evaluate the overall diagnostic accuracy of cfDNA for tuberculous pleurisy a

Real-world clinical utility of Xpert MTB/RIF Ultra in the assessment of tuberculosis in a low-TB-incidence, high-resource setting.

BACKGROUND: Tuberculosis (TB) diagnosis in the UK is impacted by delay and suboptimal culture-based microbiological confirmation rates due to the high prevalence of paucibacillary disease. We examine the real-world clinical utility of Xpert MTB/RIF Ultra (Xpert-Ultra) as a diagnostic test and biomarker of transmissible infection in a UK TB service. METHODS: Clinical specimens from suspected TB cases triple tested (smear microscopy, mycobacterial culture and Xpert-Ultra) at University Hospitals of Leicester NHS Trust (1 March 2018-28 February 2019) were retrospectively analysed. Diagnostic sen

Risk factors for tuberculosis treatment outcomes: a statistical learning-based exploration using the SINAN database with incomplete observations.

BACKGROUND: Understanding early predictors of treatment outcomes allows better outcome prediction and resource allocation for efficient tuberculosis (TB) management. OBJECTIVES: This study aimed to predict treatment outcomes of TB patients from a real-world population-wide health record dataset with a significant rate of incomplete observations. In addition, potential risk factors associated with death during TB treatment were investigated. METHODS: We exploited the upweighting approach and multiple imputation analysis (MIA) to address the extreme imbalance in responses and missing data. Thr

Tuberculosis treatment without rifampin in kidney/kidney-pancreas transplantation: A case series report.

BACKGROUND: The best approach to tuberculosis (TB) treatment in transplanted patients is still unknown. Current guidelines are based on evidence either extrapolated from other populations or observational. Rifampin-containing regimens have strong pharmacokinetic interactions with immunosuppressive regimens, with high rates of organ dysfunction and ∼20% mortality. This report describes the results obtained using non-rifampin-containing regimens to treat confirmed TB in adult patients with kidney/kidney-pancreas transplantation. METHODS: Retrospective data analysis from confirmed TB cases in ad

BACKGROUND AND AIM: Abdominal tuberculosis (TB) is a common type of extrapulmonary TB with an insidious onset and non-specific symptoms. Adenosine deaminase (ADA) levels increase rapidly in the early stages of abdominal TB. However, it remains unclear whether ADA serves as a diagnostic marker for abdominal TB. METHODS: We performed a systematic literature search for relevant articles published in PubMed, Web of Science, Cochrane Library, and Embase up to April 2022. First, we used the Quality Assessment of Diagnostic Accuracy Studies tool-2 (QUADAS-2), to evaluate the quality of the included

Reviewing a Decade of Outpatient Tropical Medicine in Houston, Texas.

Tropical diseases cause significant morbidity among the world's poorest populations. Although more common in low- and middle-income countries, tropical diseases are also found among underserved populations living in high-income countries such as the United States. The National School of Tropical Medicine at Baylor College of Medicine and the Harris Health System founded a tropical medicine clinic-the Harris Health Tropical Medicine Clinic (HHTMC)-in Houston in 2011 in response to tropical disease-related morbidity in Texas. We conducted a retrospective chart review of a sample of patients olde

Mortality in South African Children and Adolescents Routinely Treated for Tuberculosis.

BACKGROUND: In South Africa, tuberculosis (TB) is a leading cause of death among those <20 years of age. We describe changes in TB mortality among children and adolescents in South Africa over a 13-year period, identify risk factors for mortality, and estimate excess TB-related mortality. METHODS: Retrospective analysis of all patients <20 years of age routinely recorded in the national electronic drug-susceptible TB treatment register (2004-2016). We developed a multivariable Cox regression model for predictors of mortality and used estimates of mortality among the general population to calc

Molecular diagnosis of suspected tuberculosis from archived smear slides from the Balimo region, Papua New Guinea.

BACKGROUND: Tuberculosis (TB) is a serious health problem in Papua New Guinea (PNG) with an estimated 30000 new cases and 3800 deaths each year. In the Balimo region of the Western Province, diagnosis relies on clinical manifestations and on the microscopic detection of acid-fast bacilli (AFB) in sputum smears, a technique with limited sensitivity. METHODS: A molecular diagnosis assay targeting DNA extracted from archived sputum smear slides collected from the Balimo region (2012-2014) was conducted, without the need for a viable culture. The presence of Mycobacterium sp on 1162 slides prepar
