HIV-associated neurocognitive disorders at Moi teaching and referral hospital, Eldoret, Kenya.

BACKGROUND: Human Immunodeficiency Virus (HIV) infection causes a myriad of neurological complications including cognitive deficits referred to as HIV-Associated Neurocognitive Disorders (HAND). With the introduction of combination antiretroviral therapy, there has been an epidemiological shift in cognitive disorders with a decline in the more severe HIV-Associated Dementia (HAD) to an increase in the less severe HAND: Asymptomatic Neurocognitive Impairment (ANI) and HIV-associated Mild Neurocognitive Disorder (MND). Central Nervous System (CNS) involvement in HIV interferes with cognitively d

Evolving clinical phenotypes in HIV-associated neurocognitive disorders.

PURPOSE OF REVIEW: To describe the changes in the presentation of HIV-associated neurocognitive disorders (HAND) comparing the current combination antiretroviral therapy (cART) treatment era to the pre-cART era. RECENT FINDINGS: The frequency of the most severe stage of HAND, HIV-associated dementia (HAD), has decreased, but the frequencies of milder stages of HAND, asymptomatic neurocognitive impairment (ANI) and mild neurocognitive disorder, have increased. In the pre-cART era, HAD was a progressive disorder leading to death within months. With cART, HIV+ individuals with HAND frequently re

Yin and Yang of PDGF-mediated signaling pathway in the context of HIV infection and drug abuse.

The control and eradication of neurological complications associated with AIDS continues to be an important goal in efforts toward improving the well being of HIV-infected patients. Although combined antiretroviral therapies have contributed significantly to increasing the longevity of patients by suppressing the virus burden in the systemic compartments, the prevalence of HIV-associated neurological disorders continues to be on the rise. This in turn, leads to an impaired quality of life of the infected individuals who continue to suffer from mild to moderate cognitive decline and memory loss

Voltage-gated potassium channels in human immunodeficiency virus type-1 (HIV-1)-associated neurocognitive disorders.

Human immunodeficiency virus type-1 (HIV-1)-associated dementia (HAD), a severe form of HIV-associated neurocognitive disorders (HAND), describes the cognitive impairments and behavioral disturbances which afflict many HIV-infected individuals. Although the precise mechanism leading to HAD is incompletely understood, it is commonly accepted its progression involves a critical mass of infected and activated mononuclear phagocytes (brain perivascular macrophages and microglia) releasing immune and viral products in the brain. These cellular and viral products induce neuronal dysfunction and inju

Glutamate release from astrocytes in physiological conditions and in neurodegenerative disorders characterized by neuroinflammation.

Although glial cells have been traditionally viewed as supportive partners of neurons, studies of the last 20 years demonstrate that astrocytes possess functional receptors for neurotransmitters and other signaling molecules and respond to their stimulation via release of chemical transmitters (called gliotransmitters) such as glutamate, ATP, and d-serine. Notably, astrocytes react to synaptically released neurotransmitters with intracellular calcium ([Ca(2+)](i)) elevations, which result in the release of glutamate via regulated exocytosis and possibly other mechanisms. These findings have le

NeuroAIDS, drug abuse, and inflammation: building collaborative research activities.

Neurological complications of human immunodeficiency virus (HIV) infection are a public health problem despite the availability of active antiretroviral therapies. The neuropathogenesis of HIV infection revolves around a complex cascade of events that include viral infection and glial immune activation, monocyte-macrophage brain infiltration, and secretion of a host of viral and cellular inflammatory and neurotoxic molecules. Although there is evidence that HIV-infected drug abusers experience more severe neurological disease, the biological basis for this finding is unknown. A scientific work

A multicenter study of two magnetic resonance spectroscopy techniques in individuals with HIV dementia.

PURPOSE: To evaluate single-voxel proton magnetic resonance spectroscopy (SV-MRS) and magnetic resonance spectroscopic imaging (MRSI) metabolite results in individuals with HIV dementia. MATERIALS AND METHODS: Twenty HIV-positive (HIV+) individuals underwent SV-MRS (TE 35 msec) and MRSI (TE 280 msec). Results were stratified according to serostatus, dementia severity, psychomotor speed performance, and functional impairment. RESULTS: HIV+ individuals with psychomotor slowing had an increased myoinositol/creatine (mI/Cr) ratio (0.63 vs. 0.45) in the frontal white matter using SV-MRS and an in

In vitro methods in the study of viral and prion permeability across the blood-brain barrier.

(1) Infectious agents capable of entering the central nervous system (CNS) produce some of the most dreaded diseases known to man. The infectious agent within the CNS is often protected by the blood-brain barrier (BBB), shielded from endogenous and exogenous anti-infectious agents. (2) The use of in vitro methods offers many advantages to the study of how infectious agents interact with the BBB. Two such agents which negotiate the BBB early in the course of disease before damage to the BBB are the autoimmune deficiency syndrome virus, or human immunodeficiency virus 1, and scrapie prion. Decreased plasma ratio of tryptophan to competing large neutral amino acids in human immunodeficiency virus type 1 infected subjects: possible implications for development of neuro-psychiatric disorders.

Those large neutral amino acids (LNAA) which compete with each other for the carrier mediated transport from plasma into the brain were determined in plasma in human immunodeficiency virus (HIV-1) seropositive subjects and seronegative controls. Previous findings of a decreased concentration of tryptophan were confirmed whereas no difference between HIV-1 seropositive subjects and controls were found in those LNAAs with which tryptophan competes for the transport into the brain. Thus, the ratio in plasma of tryptophan to the total LNAA concentration was decreased in HIV-1 seropositive subjects

Manic syndrome early and late in the course of HIV.

In a chart review at a hospital's infectious disease AIDS clinic, manic syndromes affected 8% of patients who had AIDS. Of the 14 patients with manic episodes, those without a family or personal history of mood disorder presented later in the course of HIV infection and had a higher prevalence of comorbid dementia.

[Non-neoplastic central hearing loss--a review].

Acquired central auditory disorders not due to a tumour are unusual. The well-known conditions such as hypoxic encephalopathy, erythroblastosis, bacterial meningitis and trauma are reviewed. Recent experimental and clinical studies contribute to progress in the understanding of the pathogenesis and etiology of these conditions. The pattern of presbyacusis and cortical deafness is discussed also. AIDS must be considered as a new cause of acquired central auditory dysfunction.

Absolute quantitation in neurological PET: do we need it?

This article addresses the question posed in the title by examining the effects of parameters traditionally associated with improved absolute quantitation, on the analysis of 12 acquired immune deficiency syndrome dementia complex (ADC) patients compared to a normal control group. Results are discussed within the framework of the subprofile scaling model (SSM) for analyzing patterns of regional covariation. It is demonstrated that the ability to extract measures of group discrimination and disease progression are unaffected by (1) limited improvements in image resolution, (2) the use of transm

Headache and acquired immunodeficiency syndrome.

Because the acquired immunodeficiency syndrome (AIDS) virus is neurotropic, physicians will continue to see a rise in the number of neurologic complications of this syndrome. Much of this increase will be accompanied by headache, not only as a primary symptom of HIV infection or opportunistic disease but also as a result of diagnostic tests and therapeutic efforts. Complete understanding of the ramifications of headache in AIDS will be important in the 1990s as we continue to treat a younger population, usually affected by benign vascular and muscle contraction-type headache.

The diseases of Alzheimer and Pick from the viewpoint of prevention.

Alzheimer disease, Pick disease and senile dementia are all characterized by similar morphological changes, both macroscopic and microscopic, and similar psychiatric symptoms. All three should, therefore, be viewed as aspects of one disease, which is termed Alzheimer-type dementia. Nothing contradicts the view that immune complexes take part in the etiology of Alzheimer-type dementia. There are various types of immune complexes and so their points of action in the organism may vary. There might be two main means of preventing Alzheimer-type dementia: the first could be named the serological (b

HIV-associated primary CNS lymorbidity and utility of brain biopsy.

INTRODUCTION: Human immunodeficiency virus (HIV) infection is associated with several central nervous system (CNS) infections and neoplasms. These opportunistic processes generally occur with advanced immunosuppression, but if an accurate diagnosis is made, effective treatment can frequently be initiated. METHODS: In an attempt to assess the safety, diagnostic yield, and utility of stereotactic brain biopsy in the clinical management of suspected HIV-associated primary CNS lymphoma, we retrospectively studied the performance of biopsy in HIV-seropositive patients presenting with focal intracr

Pediatric infection with the human immunodeficiency virus (HIV): head, neck, and oral manifestations.

The increasing incidence of HIV infection in the pediatric population is of concern for the practicing dentist. Long incubation periods of the virus, combined with difficulty in detection, results in many undiagnosed cases of prenatal and natal infections with HIV. As a result, many dentists will unknowingly treat HIV-positive children. This article presents most common features of pediatric HIV infection, placing special emphasis on manifestations which affect the head, neck, and oral tissues.

Common neurologic complications of HIV-1 infection and AIDS.

Complications of human immunodeficiency virus type 1 infection and acquired immunodeficiency syndrome may involve any level of the central or peripheral nervous system. Acute encephalitis, aseptic meningitis and acute demyelinating polyneuropathy may occur early in the course of HIV infection, while dementia, central nervous system-related cancer, opportunistic infections and autonomic neuropathy typically present later. Headache and mental status changes are common early manifestations of central nervous system involvement. Most severe headaches are related to an identifiable cause, including
