The split abdominal wall muscle flap--a simple, mesh-free approach to repair large diaphragmatic hernia.

PURPOSE: The authors present a technique to repair large diaphragmatic hernias that is simple, can be done primarily, and offers a durable closure with living tissue. METHODS: A review of congenital diaphragmatic hernias was performed for the period between January 1991 and August 2000. Repair types included primary repair, synthetic patch, or a split abdominal wall muscle flap. The split abdominal wall muscle flap is performed by downward rotation of the internal oblique and transversalis abdominal wall muscles. This repair requires that the initial subcostal incision be positioned at least

Changes in muscle strength and pain in response to surgical repair of posterior abdominal wall disruption followed by rehabilitation.

BACKGROUND: Posterior abdominal wall deficiency (PAWD) is a tear in the external oblique aponeurosis or the conjoint tendon causing a posterior wall defect at the medial end of the inguinal canal. It is often known as sportsman's hernia and is believed to be caused by repetitive stress. OBJECTIVE: To assess lower limb and abdominal muscle strength of patients with PAWD before intervention compared with matched controls; to evaluate any changes following surgical repair and rehabilitation. METHODS: Sixteen subjects were assessed using a questionnaire, isokinetic testing of the lower limb stre

Immobilzation-Induced Muscle Atrophy Exacerbates Vascular Calcification and Abdominal Aortic Aneurysms Through Irisin Downregulation.

BACKGROUND: Sarcopenia is widely recognized that affects multiple organs, including the cardiovascular system. Clinical studies have consistently shown associations between sarcopenia and vascular aging phenotypes such as vascular calcification, atherosclerosis, and abdominal aortic aneurysms (AAA). However, the molecular mechanisms by which muscle deterioration contributes to vascular degeneration remain poorly understood. METHODS: To elucidate muscle-vascular crosstalk, we established a murine model combining inflammation-associated muscle atrophy induced by hindlimb immobilization with CaP

TCF7L2 promotes abdominal aortic aneurysm through smooth muscle cell-mediated extracellular matrix remodeling.

Abdominal aortic aneurysm (AAA) lacks effective pharmacological therapies. Here, we investigate transcription factor 7-like 2 (TCF7L2), a genetic locus associated with both thoracic and abdominal aortic aneurysms, to elucidate its role in AAA pathogenesis. Integrating summary data-based Mendelian randomization (SMR) with single-cell RNA sequencing of human and mouse aortae, we identify TCF7L2 as a gene enriched in vascular smooth muscle cells (VSMCs) and causally linked to AAA development. Smooth muscle cell-specific TCF7L2 knockout significantly attenuates AAA formation across 3 distinct muri

Correlation between BAC and abdominal muscle alcohol concentration: a promising parachute matrix in cases of severe injury.

Ethanol represents the most abused drug worldwide, and among the most frequently evidenced in toxicological analysis on drug-induced/related deaths. When a blood sample is not available in a death characterized by severe trauma an alternative matrix is needed for an estimation of impairment. Presented is the correlation of postmortem blood and rectus abdominis muscle ethanol concentration. Ethanol concentrations in both blood (BAC) and muscle were determined by head space gas chromatography/mass spectrometry. Twenty-nine forensic cases with positive BAC were selected for the correlation study.

Variation in image-assessed abdominal adiposity and skeletal muscle and their associations with pathological characteristics in renal cell carcinoma.

PURPOSE: Body composition varies by race and ethnicity and influences cancer prognosis, but the associations of body composition with pathological characteristics in renal cell carcinoma (RCC) remain unclear. This study investigated body composition variation and its association with RCC histology, tumor grade, and stage. METHODS: Using preoperative imaging scans, we measured area (cm2) and radiodensity of adipose tissue sub-compartments and skeletal muscle. Area measurements were normalized for height squared (cm2/m2) to calculate intermuscular, visceral, and subcutaneous adipose tissue and

From age to physiological reserve: Frailty and muscle strength in early postoperative outcomes after abdominal surgery.

BACKGROUND: Chronological age is widely used to estimate surgical risk but may inadequately reflect early postoperative recovery. Markers of physiological reserve, including frailty and muscle strength, may better capture vulnerability to surgical stress. METHODS: We conducted a prospective multicenter cohort study of 223 adults undergoing elective abdominal surgery. Frailty was assessed using a phenotype-based model, and handgrip strength (HGS) was measured at admission and discharge, with 90-day follow-up in a subset. Length of stay (LOS) and postoperative complications were analyzed using

Differential Effects of Abdominal Bracing and Hollowing on Scapular Muscle Activation Patterns During the Y-Raise Exercise.

Background: Trunk stability is closely linked to scapular muscle recruitment during upper-limb activities, yet the differential effects of abdominal bracing and hollowing on scapular muscle activation patterns remain unclear. Objective: To compare the effects of abdominal bracing, hollowing, and normal breathing on scapular muscle activation and activation ratios during the Y-raise exercise in individuals with rounded shoulder posture (RSP). Methods: Thirty-six individuals with RSP participated in this within-subject repeated-measures study. Surface EMG was used to record upper trapezius (UT),

BRG1 Ablation in Vascular Smooth Muscle Cells Ameliorates Abdominal Aortic Aneurysm: Mechanism and Translational Potential.

Abdominal aortic aneurysm (AAA) is a major vascular pathology with high morbidity and mortality. Phenotypic changes of vascular smooth muscle cells (VSMC) play a key role in AAA pathogenesis. In the present study, we investigated the effect of VSMC-specific ablation of BRG1, a chromatin remodeling protein, on AAA pathogenesis focusing on transcriptional mechanism and translational potential. BRG1 expression was elevated in VSMCs by pro-AAA stimuli in vitro and in vivo. Compared to the BRG1f/f mice, the VSMC-specific BRG1 knockout mice (BRG1ΔSMC) displayed a less severe AAA phenotype induced by

The Effects of Functional Magnetic Stimulation on Rectus Abdominis Muscle Size and Abdominal Subcutaneous Adipose Tissue Thickness.

Background Functional magnetic stimulation (FMS) is a non-invasive technology that induces supramaximal muscle contractions through rapidly changing magnetic fields. It has been proposed as a method to enhance muscle hypertrophy and reduce localized adiposity, but objective clinical evidence remains limited. This pilot case series implemented a systematic intervention and measurement protocol to evaluate the effects of FMS on rectus abdominis muscle thickness and abdominal subcutaneous adipose tissue. Methods Ten healthy adults (six female, four male; mean age 32.7 years) underwent 10 sessions

Mycotic Abdominal Aortic Aneurysm Secondary to BCG Therapy for Non-muscle Invasive Urothelial Carcinoma of the Bladder.

Bacillus Calmette-Guerin (BCG) therapy for treatment of bladder cancer is a rare cause of Mycobacterium bovis infected aortic aneurysm. Typical presentations have included general malaise, fever, and lower back pain. We present a case with lower back pain and constipation as presenting symptoms, leading to diagnosis of mycotic aneurysm presumed secondary to intravesical BCG therapy. Treatment included open surgical repair with femoral vein grafting and anti-tubercular therapy. This case highlights the importance of a high index of suspicion for less common infectious complications of BCG thera

Genetic and Epigenetic Mechanisms Underlying Vascular Smooth Muscle Cell Phenotypic Modulation in Abdominal Aortic Aneurysm.

Abdominal aortic aneurysm (AAA) refers to the localized dilatation of the infra-renal aorta, in which the diameter exceeds 3.0 cm. Loss of vascular smooth muscle cells, degradation of the extracellular matrix (ECM), vascular inflammation, and oxidative stress are hallmarks of AAA pathogenesis and contribute to the progressive thinning of the media and adventitia of the aortic wall. With increasing AAA diameter, and left untreated, aortic rupture ensues with high mortality. Collective evidence of recent genetic and epigenetic studies has shown that phenotypic modulation of smooth muscle cells (

Pain intensity and abdominal muscle activation during walking in patients with low back pain: The STROBE study.

Nonspecific low back pain (LBP) is a common musculoskeletal problem that is intensified during physical activity. Patients with LBP have been reported to change their abdominal muscle activity during walking; however, the effects of pain intensity, disability level, and fear-avoidance belief on this relationship have not been evaluated. Thus, we compared abdominal muscle activity in patients with LBP and asymptomatic controls, and assessed the impact of pain intensity, disability level, and fear-avoidance belief.Thirty patients with LBP divided into groups reporting low (LLBP) and high-pain in

Split abdominal wall muscle flap repair vs patch repair of large congenital diaphragmatic hernias.

PURPOSE: Large congenital diaphragmatic hernias are commonly repaired with a prosthetic patch. We hypothesized that a split abdominal wall muscle flap would reduce the risk of recurrence. METHODS: A retrospective review of neonates with congenital diaphragmatic hernia in whom primary repair was not possible was performed. Kaplan-Meier analyses and Cox proportional hazards modeling were performed. RESULTS: Of 153 patients, 46 could not have repair with primary closure of the diaphragm. Thirty-three survived to discharge and were subjected to analysis for recurrence. Ten underwent repair with
