Pediatric radiofrequency catheter ablation: results of initial 100 consecutive cases including congenital heart anomalies.

Radiofrequency catheter ablation (RFCA) has recently become a management option for pediatric tachycardia. We reviewed the records of a total of 100 patients (aged 10 months to 19 yr) who had undergone RFCA, from March 2000 to June 2004. Types of arrhythmia (age, acute success rate) were as follows: atrioventricular reentrant tachycardia (AVRT, 9.0+/-3.7 yr, 66/67), atrioventricular nodal reentrant tachycardia (AVNRT, 13+/-2.5 yr, 16/16), ectopic atrial tachycardia (6.4+/-3.3 yr, 5/5), junctional ectopic tachycardia (10 month, 1/1), ventricular tachycardia (12+/-4.9 yr, 6/6), postsurgical intr

Positive QRS complex in limb lead 2 with negative QRS in lead 3 on surface electrocardiogram: A novel predictor for anterior location of right-sided accessory pathways (10-1 o'clock on tricuspid annulus).

BACKGROUND: Localization of atrioventricular accessory pathways (AP) from Electrocardiogram (ECG) is crucial for successful ablation. We analyzed the value of limb lead 2 versus 3 QRS vector discordance on surface ECG among right-sided pathways. METHODS: Data from consecutive patients undergoing successful ablation of manifest AP were analyzed. They were categorized into two groups-Gr I: Endocardial ablation from anterior and anterolateral tricuspid annulus (TA, 10-1 o'clock, right anterolateral [RAL]); Gr II: Ablation outside this region (1-10 o'clock of TA). Inferior lead discordance (ILD)

Radiofrequency catheter ablation of accessory pathway in a patient with Ebstein's anomaly and atrial septal defect--a case report.

The authors report the case of a 57-year-old woman with Ebstein's anomaly and atrial septal defect. She was referred to their hospital for treatment of refractory paroxysmal wide QRS tachycardia. Her 12-leads ECG in sinus rhythm showed ventricular preexcitation of type B Wolff-Parkinson-White syndrome. In a baseline electrophysiological study, a wide QRS tachycardia with right bundle branch block configurations was induced. This tachycardia was orthodromic atrioventricular reciprocating tachycardia with a right inferior accessory pathway. Radiofrequency current was successfully delivered at th

Line of block: A simple pacing maneuver to distinguish AV nodal from accessory pathway ventriculoatrial conduction.

BACKGROUND: Septal accessory pathways (APs), including concealed APs can be difficult to diagnose using standard electrophysiology (EP) maneuvers. We introduce a simple technique using the "line of block" concept that can potentially added to the list of standard maneuvers in cases where the presence of a septal AP is suspected. OBJECTIVE: This study aimed to determine the efficacy of the "line of block" concept among patients with septal APs using a reproducible and simple technique. METHODS: An exploratory catheter is used to pace the basal right ventricle, and the stimulus-to-atrial inter

Comparison of transesophageal and intracardiac electrophysiologic studies in characterization of supraventricular tachycardia in pediatric patients.

OBJECTIVES: This study sought to determine the accuracy of transesophageal electrophysiologic studies in diagnosing and characterizing various mechanisms of supraventricular tachycardia in pediatric patients. BACKGROUND: Transesophageal electrophysiologic studies are a relatively noninvasive means of characterizing supraventricular tachycardia. Although widely used, to our knowledge no data exist that directly compare information obtained from transesophageal electrophysiologic studies with that from intracardiac electrophysiologic studies. METHODS: We reviewed the records of 57 pediatric pa

The effect of NavX on fluoroscopy times in pediatric catheter ablation.

PURPOSE: Catheter ablation is the established curative therapy for pediatric tachyarrhythmias. However, exposure to ionizing radiation from fluoroscopy during the procedure is of concern to both patients and caregivers. We sought to assess the impact of an impedance-based three-dimensional navigation system (NavX(TM), Endocardial Solutions, Inc., St. Paul, MN) on pediatric catheter ablation procedures. METHODS: We retrospectively analyzed procedural data during a 7-year period (2002-2008), which spanned the transition between standard fluoroscopic mapping and adoption of NavX(TM) mapping for

Reliability of retrograde atrial activation patterns during ventricular pacing for localizing accessory pathways.

Definitive localization of accessory pathways is based on atrial activation patterns during orthodromic supraventricular tachycardia when retrograde conduction occurs exclusively through the accessory pathway. In some patients, supraventricular tachycardia cannot be induced or is deleterious. To determine whether accessory pathway sites can be identified accurately during ventricular pacing, retrograde atrial activation was assessed during orthodromic supraventricular tachycardia and ventricular pacing at multiple cycle lengths in 41 patients with a single accessory pathway. To obviate retrogr

Surgery for supraventricular tachyarrhythmias.

Since the first successful surgical intervention for Wolff-Parkinson-White syndrome by W. C. Sealy, a surgical electrophysiological intervention has been developed for every single supraventricular arrhythmia. The surgical rationale is based on the site of the mechanism of the arrhythmia and associated pathology which characterizes the "arrhythmogenic substrate". Wolff-Parkinson-White syndrome is a congenital heart disease characterized by an accessory atrioventricular connection distinct from the AV node-His bundle system. It is associated with AV reentrant tachycardia and/or atrial fibrillat

Facilitation of atrioventricular reentrant tachycardia by iatrogenic right bundle branch block.

The present case report describes the diagnosis of a concealed bypass tract in the right lateral wall revealed by electrophysiologic evaluation performed in a patient with rare palpitations. A iatrogenic right bundle branch block (RBBB) caused the occurrence of an incessant atrioventricular reentrant tachycardia. The disappearance of the RBBB determined a very difficult induction of the tachycardia that, when induced, showed a shorter cycle length and ventriculoatrial interval than those observed during RBBB tachycardia. The presence of a RBBB ipsilateral to the right free wall accessory pathw

Endocardial ablation at the mitral valve vestibule and its surroundings for the treatment of arrhythmias linked to the greater cardiac vasculature.

BACKGROUND: The role of the epicardial vasculature in supraventricular and ventricular arrhythmias was described in clinical studies as well as its treatment by intravascular point ablation or alcohol injection. We report on a case series of patients with different arrhythmias linked to an epicardial site of origin with evidence supporting transmural extensions that were targeted for ablation with successful outcomes. METHODS: The records of patients who has catheter ablation for Supraventricular or Ventricular arrhythmias between 2015 and 2020 was searched for patients with (1) arrhythmias l

Pediatric T-wave memory after accessory pathway ablation in Wolff-Parkinson-White syndrome.

BACKGROUND: Altered ventricular depolarization due to manifest accessory pathway conduction (ie, Wolff-Parkinson-White syndrome) leads to repolarization abnormalities that persist after pathway ablation. The term T-wave memory (TWM) has been applied to these changes, as the postablation T-wave vector "remembers" the pre-excited QRS vector. In adults, these abnormalities can be misinterpreted as ischemia leading to unnecessary interventions. To date, no comprehensive studies have evaluated this phenomenon in the pediatric population. OBJECTIVE: The purpose of this study was to define TWM in th

Accessory pathway ablation in Ebstein anomaly: A challenging substrate.

BACKGROUND: Catheter ablation of accessory pathways (APs) in Ebstein anomaly (EA) has been associated with a high recurrence risk. OBJECTIVE: The purpose of this study was to compare outcomes of AP ablation in EA in an early (1990-2004) vs a recent (2005-2019) era and identify variables associated with recurrence. METHODS: A retrospective review of all catheter ablations for supraventricular tachycardia in EA at our institution was performed. RESULTS: We identified 76 patients with median (25th-75th quartiles) age 9 (2.6-13.3) years. Of these patients, 52 had AP alone, 12 had atrial flutter

Cardiac 'resynchronization' by a left lateral accessory pathway.

We present the case of a 21 year-old woman with unremarkable medical history except for tachycardia episodes. The ECG showed a delta wave pre-excitation by a left lateral pathway. One radiofrequency pulse temporarily terminated the accessory pathway conduction, revealing a nodal conduction with complete left bundle branch block.

[Radiofrequency ablation of accessory pathway from noncoronary cusp of aorta after failed cryoablation].

Successful ablation of atrial or ventricular arrhythmia from aortic sinuses of Valsalva has been well documented with low complication rate. Radiofrequency (RF) ablation of anteroseptal pathway is always challenging for the risk of atrioventricular block. Thus cryoablation is widely accepted approach. We present a 27 year-old patient with WPW syndrome and frequent palpitations referred for cryoablation of anteroseptal pathway. Extensive mapping from high anterseptal region and from noncoronary aortic cusp of aorta revealed a sharp pathway potential but several applications of cryomapping were

Radiofrequency ablation of a left accessory pathway unmasks classic Chagas' disease ECG pattern.

In patients with chronic Chagas' cardiomyopathy, there are forms of the disease that affect the electrical conduction system almost exclusively. The most common disorders include right bundle branch block alone or in association with left anterior fascicular block. We present an unusual case of a patient with Chagas' cardiomyopathy in association with a preexcitation syndrome.
