The posterior deltoid-splitting approach to the shoulder.

Since 1980, the authors have used a posterior approach to the glenohumeral joint in which the posterior deltoid is split caudally in line with its fibers from the posterior acromion to the upper border of the teres minor. The extent of this deltoid splitting approach exceeds that of a similar anterior approach because of the distal emergence of the axillary nerve from the quadrilateral space. This technique gives complete access to the infraspinatus and teres minor muscles and tendons, posterior capsule, and posterior glenoid. Unlike traditional posterior approaches to the shoulder joint that

Quadruple Disruption of the Superior Shoulder Suspensory Complex: A Case Report on Surgical Intervention Following High-Energy Trauma in a Professional Cycling Athlete.

The superior shoulder suspensory complex (SSSC), which consists of several anatomical structures, plays a crucial role in stabilizing the shoulder girdle. While single disruptions can often be managed non-operatively, double disruptions typically require surgical intervention due to increased risks of complications like persisting instability, malunion, and/or non-union. In addition, quadruple disruptions are extremely rare and usually caused by high-energy trauma. Surgical repair is required to optimize functional outcomes. We present the case of a 34-year-old male professional cyclist who su

Scapula position test reliability and comparisons of scapula position and shoulder function among individuals with and without adolescent idiopathic scoliosis.

BACKGROUND: An evaluation of scapula position is important to predict spine curvature and shoulder function in patients with adolescent idiopathic scoliosis (AIS). Therefore, a reliable scapula position is required to assess posture in patients with AIS. OBJECTIVE: This study investigated the reliabilities of scapula position tests and differences in scapula position and shoulder function between individuals with and without AIS. METHODS: The study compared 18 patients with AIS and 18 healthy controls to identify differences in scapula position and shoulder function. The acromion-table dista

[New arthroscopic portal for performing tenotomy/tenodesis procedures on the long head of the biceps brachii tendon].

BACKGROUND: Shoulder arthroscopy is the standard technique for performing procedures involving the intertubercular groove. Current techniques continue to produce excessive soft tissue manipulation and neurovascular injury. MATERIALS AND METHODS: A cross-sectional, observational and descriptive study was conducted on a cohort of 24 shoulders following the standard surgical protocol and using punch dissection. The neurovascular structures with risk of damage by the standard lateral portal were evaluated during the study to establish a secure area for a new arthroscopic portal. Finally, the safe

Quantitative and Qualitative Surgical Anatomy of the Acromioclavicular Joint Capsule and Ligament: A Cadaveric Study.

BACKGROUND: The acromioclavicular (AC) capsule and ligament have been found to play a major role in maintaining horizontal stability. To reconstruct the AC capsule and ligament, precise knowledge of their anatomy is essential. PURPOSE/HYPOTHESIS: The purposes of this study were (1) to determine the angle of the posterosuperior ligament in regard to the axis of the clavicle, (2) to determine the width of the attachment (footprint) of the AC capsule and ligament on the acromion and clavicle, (3) to determine the distance to the AC capsule from the cartilage border of the acromion and clavicle,

Primary Monoblock Inset Reverse Shoulder Arthroplasty Resulted in Decreased Pain and Improved Function.

BACKGROUND: The first-generation, lateral-center-of-rotation reverse shoulder arthroplasty (RSA) modular design has demonstrated durable early-, mid-, and long-term outcomes. The second-generation monoblock implant shares a similar design but eliminates the modular junction and facilitates inset placement within the metaphysis to avoid humeral-sided junctional failures and facilitate metaphyseal press-fit. However, no paper has specifically examined the radiographic findings and improvements in pain and function after the use of this next generation design. QUESTIONS/PURPOSES: (1) After secon

[Enthesopathy of the coraco-acromial ligament and rupture of the rotator cuff].

Ossification of the coracoacromial ligament was looked for routinely on the roentgenograms of 50 patients with arthrographically-documented rotator cuff tear and of 25 controls free of clinical evidence of shoulder disease. Ossification of the coracoacromial ligament was seen in none of the 25 controls versus 12 of the 50 patients. The ossification consistently arose from the acromial attachment site. A few patients developed ossification of the entire ligament bridging the gap between the acromion and coracoid process.

Reduction of missed posterior dislocation of the shoulder. Report of 2 cases, 1 of them bilateral.

We report 2 cases of missed posterior dislocation of the shoulder, 1 of them bilateral. Prominence of the acromion and coracoid, posterior bulging and complete elimination of external rotation of the humerus lead to the diagnosis which was established by an axillary radiographic view. All 3 shoulders were treated surgically according to Mc Laughlin. At 3-4 years follow-up the function was good and there were no signs of arthrosis or avascular necrosis of the head.

Test-retest reliability of a photographic marker-based system for postural examination.

BACKGROUND: The BHOHB system (Bhohb S.r.l., Italy) is a portable non-invasive photographic marker-based device for postural examination. OBJECTIVE: To assess the test-retest reliability of the BHOHB system and compare its reliability with an optoelectronic system (SMART-DX 700, BTS, Italy). METHODS: Thirty volunteers were instructed to stand upright with five markers on the spinous processes of C7, T6, T12, L3 and S1 vertebrae to define the dorsal kyphosis and lumbar lordosis (sagittal plane) angles. Three markers were placed on the great trochanter, apex of iliac crest and lateral condyle o

Internal fixation of symptomatic os acromiale: a series of twenty-six cases.

Twenty-six patients who presented to our shoulder service with a symptomatic meso-os acromiale were reviewed. All had been initially treated for impingement symptoms. Nonoperative treatment had failed in all patients. One patient had also undergone an arthroscopic acromioplasty without benefit. The diagnosis of symptomatic os acromiale was made on the basis of radiographs and point tenderness over the acromion coupled with signs of rotator cuff pathology. We assessed these patients after treatment by internal fixation and bone grafting. Fixation was achieved with either K-wires or screws and t

Suppurative arthritis involving the synchondrosis of an os acromiale.

Os acromiale is an unfused epiphysis of the anterior part of the acromion occurring in approximately 8% of the population. Infection of this joint has not been previously described in the literature. We report such a case in a 59-year-old woman presenting with shoulder pain. A high index of clinical suspicion, with early imaging of the shoulder leading to prompt and definitive diagnosis, can lead to appropriate treatment and produce a favorable outcome.

Study protocol subacromial impingement syndrome: the identification of pathophysiologic mechanisms (SISTIM).

BACKGROUND: The subacromial impingement syndrome (SIS) is the most common diagnosed disorder of the shoulder in primary health care, but its aetiology is unclear. Conservative treatment regimes focus at reduction of subacromial inflammatory reactions or pathologic scapulohumeral motion patterns (intrinsic aetiology). Long-lasting symptoms are often treated with surgery, which is focused at enlarging the subacromial space by resection of the anterior part of the acromion (based on extrinsic aetiology). Despite that acromionplasty is in the top-10 of orthopaedic surgical procedures, there is no

[Degenerative shoulder diseases--their differentiation and surgical therapy].

Degenerative shoulder diseases can be clearly differentiated from each other and from different ailments by careful recording of the case history, through clinical examination, and arthrography. Considerable therapeutic achievements have been recordable from operations on patients who had suffered for years. Eighty anterior acromion repairs had been performed with encouraging results by Juli 1987. Postoperative physiotherapy should be carried out with persistency and may last several months.

The biomechanical effects of acromial fracture angulation in reverse total shoulder arthroplasty.

BACKGROUND: The biomechanical changes and treatment guidelines on acromial fracture after reverse shoulder arthroplasty (RSA) are still not well understood. The purpose of our study was to analyze the biomechanical changes with respect to acromial fracture angulation in RSA. METHODS: RSA was performed on 9 fresh-frozen cadaveric shoulders. An acromial osteotomy was performed on the plane extending from the glenoid surface to simulate an acromion fracture. Four conditions of acromial fracture inferior angulation were evaluated (0°, 10°, 20°, and 30° angulation). The middle deltoid muscle loadi

Clinical and radiological outcomes with an augmented baseplate for superior glenoid wear in reverse shoulder arthroplasty.

AIMS: This study aimed to assess the impact of using the metal-augmented glenoid baseplate (AGB) on improving clinical and radiological outcomes, as well as reducing complications, in patients with superior glenoid wear undergoing reverse shoulder arthroplasty (RSA). METHODS: From January 2016 to June 2021, out of 235 patients who underwent primary RSA, 24 received a superior-AGB after off-axis reaming (Group A). Subsequently, we conducted propensity score matching in a 1:3 ratio, considering sex, age, follow-up duration, and glenoid wear (superior-inclination and retroversion), and selected

The Upper Arm Lymphosome: Watershed of Upper Arm Lymphatic Pathways Evaluated with Indocyanine Green Lymphography.

Understanding lymphatic anatomy of the upper extremity is necessary for treatment of upper extremity lymphedema (UEL). This study aimed to clarify the watershed of upper arm lymphosomes using fluorescent lymphography with multi-site injection of indocyanine green (ICG). Limbs contralateral to breast cancer treatment side of breast cancer survivors were included to evaluate upper arm lymphosomes. 0.1ml of ICG was injected intra-dermally at these 3 points in the mid-lateral upper arm: at the level of one (U1), two (U2), and three quarters (U3) from the lateral edge of the acromion to the lateral

Correlation between anatomy of the scapula and the incidence of rotator cuff tear and glenohumeral osteoarthritis via radiological study.

INTRODUCTION: We conducted a study to elucidate the correlation between the anatomy of the shoulder joint with the development of rotator cuff tear (RCT) and glenohumeral osteoarthritis (GHOA) by using acromioglenoid angle (AGA). MATERIALS AND METHODS: The AGA is a new measured angle formed between the line from midglenoid to lateral end of the acromion with the line parallel to the glenoid surface. The AGA was measured in a group of 85 shoulders with RCT, 49 with GHOA and 103 non-RCT/GHOA control shoulders. The AGA was compared with other radiological parameters, such as, the critical should

The girdles of the oldest fossil turtle, Proterochersis robusta, and the age of the turtle crown.

BACKGROUND: Proterochersis robusta from the Late Triassic (Middle Norian) of Germany is the oldest known fossil turtle (i.e. amniote with a fully formed turtle shell), but little is known about its anatomy. A newly prepared, historic specimen provides novel insights into the morphology of the girdles and vertebral column of this taxon and the opportunity to reassess its phylogenetic position. RESULTS: The anatomy of the pectoral girdle of P. robusta is similar to that of other primitive turtles, including the Late Triassic (Carnian) Proganochelys quenstedti, in having a vertically oriented sc

[The hyperostotic shoulder].

A systematic study of shoulder joints in 30 DISH patients demonstrates the frequency of shoulder involvement in this disease and its characteristic radio-clinical features. Radiological abnormalities observed in DISH shoulders are contrasted with those encountered in rotator cuff tears. However, both disorders can occur together, producing a "complex shoulder" syndrome. The most typical aspect of this is the presence of an osteoarthritic pseudoarthrosis between the greater tuberosity and the acromion which has developed from the subacromial enthesopathy.
