Clinical progress note: Phenobarbital in the treatment of alcohol withdrawal syndrome.

Alcohol withdrawal syndrome (AWS) is a common condition experienced by hospitalized patients. Practice patterns have evolved over time to include the use of phenobarbital, a barbiturate, as an adjunct to benzodiazepines or as an alternative monotherapy. The American Society of Addiction Medicine (ASAM) has recommended the use of phenobarbital in the management of AWS in certain clinical contexts. The current evidence base for the use of phenobarbital in AWS remains limited, though sufficient to demonstrate safety and efficacy as an alternative to benzodiazepines.

[Recommendations for management of misuses and addictions to benzodiazepines].

The management of patients who have become dependent on benzodiazepines and analogues is a problem frequently encountered in both somatic and psychiatric medicine. No pharmacological treatment is currently recognized as effective in the management of these addictions, apart from a gradual reduction of doses. We propose practical strategies for the implementation of gradual dose reduction and choice of molecules while promoting individual adaptation to the withdrawal symptoms presented by the patient.

Opiate-induced molecular and cellular plasticity of ventral tegmental area and locus coeruleus catecholamine neurons.

The study of neuronal adaptations induced by opiate drugs is particularly relevant today given their widespread prescription and nonprescription use. Although much is known about the acute actions of such drugs on the nervous system, a great deal of work remains to fully understand their chronic effects. Here, we focus on longer-lasting adaptations that occur in two catecholaminergic brain regions that mediate distinct behavioral actions of opiates: ventral tegmental area (VTA) dopaminergic neurons, important for drug reward, and locus coeruleus (LC) noradrenergic neurons, important for physic

[Assessment and treatment of cannabis use disorders in primary health care].

In Finland every fourth young adult (25 to 34 years of age) has experience with the use of cannabis and in younger people (15 to 24 years) about 10% have used it during the previous year. The majority (about 90%) neither becomes addicted nor develops significant psychiatric symptoms, but in some individuals regular cannabis-use can cause a variety of problems (health, emotional, behavioural, social, legal). If problems have not yet developed or are still minor, patient's cannabis-use may be addressed by brief intervention in a primary care setting. Also, treatment of uncomplicated cannabis wit

[Drug addiction and culture].

In the course of ages drug always existed. It accompanied man in migrations, conquests and defeats. Formerly bordered upon some countries, and witness of a determined culture, drug has now a planetary dimension. In a way, it is the price of our material progress. Medicine has a limited understanding of the fact. It is worth while when approaching the therapeutic problem of drug addict to study new ways in approaching the problem.

Elective Naloxone-Induced Opioid Withdrawal for Rapid Initiation of Medication-Assisted Treatment of Opioid Use Disorder.

We present a case of elective naloxone-induced opioid withdrawal followed by buprenorphine rescue to initiate opioid use disorder treatment in the emergency department. This strategy may represent a safe alternative to prescribing buprenorphine for outpatient initiation, a method that puts the patient at risk for complications of unmonitored opioid withdrawal, including relapse. After confirmation that the naloxone-induced withdrawal was adequately treated with buprenorphine, the patient was discharged with prescribed buprenorphine to follow up in an addiction medicine clinic, where he was tre

Ivermectin attenuates nicotine-induced reward-like behaviors in mice.

Nicotine addiction poses a significant public health threat, particularly within the realm of emergency medicine, where it is associated with serious complications, including cardiovascular events and respiratory distress. The limited effectiveness of current pharmacological treatments for nicotine dependence underscores the urgent need for innovative and effective therapeutic approaches. Recent studies have shown that ivermectin, an antiparasitic agent, modulates the GABAergic, glutamatergic, and purinergic systems, which are implicated in the pathophysiology of addiction. This study aimed to

The historical shift in the perception of opiates: from medicine to social menace.

An examination of the origins of the laws prohibiting the use of opiates in the United States is provided. The primary focus is on how the development of these laws influenced a marked shift in the perception of opiates. Historically, opium and its derivatives have been perceived as efficacious medicines. However, during the past two centuries this perception has shifted to the point that contemporarily the opiates are commonly thought of as a social menace. This perception now outweighs the efficacious medicine perception to a substantial degree. A historical analysis indicates that this shif

Buprenorphine management in the perioperative period: educational review and recommendations from a multisociety expert panel.

BACKGROUND: The past two decades have witnessed an epidemic of opioid use disorder (OUD) in the USA, resulting in catastrophic loss of life secondary to opioid overdoses. Medication treatment of opioid use disorder (MOUD) is effective, yet barriers to care continue to result in a large proportion of untreated individuals. Optimal analgesia can be obtained in patients with MOUD within the perioperative period. Anesthesiologists and pain physicians can recommend and consider initiating MOUD in patients with suspected OUD at the point of care; this can serve as a bridge to comprehensive treatment

Factors Influencing the Prognostic Assessment of Patients with End-Stage Alcohol-Associated Liver Disease before Liver Transplantation.

UNLABELLED: <p>Introduction: Although alcohol-associated liver disease (ALD) is a leading cause of liver transplantation, most patients with ALD are never evaluated. This study aims to investigate the prognostic assessment process of medical professionals often involved in treating patients with ALD. We wanted to assess the possible effect of disease understanding, stigmatization, experience in transplantation, and addiction medicine on the liver transplantation evaluation process. METHODS: The study was conducted as an anonymous online survey. In total, 407 health professionals were included

Clinical Impact of an Expanded MOUD Access Initiative for Patients Hospitalized With Infections From Intravenous Opioid Use.

BACKGROUND: Despite their efficacy, medications for opioid use disorder (MOUD) remain underutilized in patients with infections from intravenous opioid use (I-IOU). This study evaluates the impact of an Expanded MOUD Access Initiative (EMAI) on MOUD uptake and other clinical outcomes in patients hospitalized for I-IOU at an institution without addiction medicine consultation. METHODS: We performed a retrospective pre-post study of hospital admissions for I-IOU before (January 2019-June 2021) and after (January 2022-December 2023) EMAI introduction. Data was collected via chart review. The EMA

Abdominal Pain and Dysuria Secondary to Chronic Recreational Ketamine Use: A Case Report on K-cramps.

Medical and recreational ketamine use is increasing in the United States; however, little is known regarding the side effects associated with chronic, frequent, and high-dose use. The lack of emergency physician awareness regarding ketamine-induced abdominal pain, nausea, vomiting, and dysuria, collectively and colloquially known as K-cramps, results in delayed recognition, underreporting, and inappropriate diagnostic workup and treatment. A 25-year-old woman with a history of anxiety, asthma, obsessive-compulsive disorder, and chronic high-dose (500-1000 milligrams weekly) ketamine use presen

Cannabinoids in health and disease.

Cannabis sativa L. preparations have been used in medicine for millenia. However, concern over the dangers of abuse led to the banning of the medicinal use of marijuana in most countries in the 1930s. Only recently, marijuana and individual natural and synthetic cannabinoid receptor agonists and antagonists, as well as chemically related compounds, whose mechanism of action is still obscure, have come back to being considered of therapeutic value. However, their use is highly restricted. Despite the mild addiction to cannabis and the possible enhancement of addiction to other substances of abu

Temporal distributions of crime and disorder in casino and non-casino zones.

Research on the relationship between casinos and crime has yielded mixed conclusions. Some authors argue that casinos are crime attractors and provide fertile grounds for disorder in the surrounding communities. Others claim that the impact of casinos on the crime problem has been over-stated and that casinos in themselves are not crime generators. Relying on calls for service and incidents known to the police, this research reveals the specific spatial and temporal characteristics of all criminal and disorderly behaviors around casino venues.

Preclinical target validation for non-addictive therapeutics development for pain.

INTRODUCTION: The Helping to End Addiction Long-termSM Initiative supports a wide range of programs to develop new or improved prevention and opioid addiction treatment strategies. An essential component of this effort is to accelerate development of non-opioid pain therapeutics. In all fields of medicine, therapeutics development is an arduous process and late-stage translational efforts such as clinical trials to validate targets are particularly complex and costly. While there are plentiful novel targets for pain treatment, successful clinical validation is rare. It is therefore crucial to
