Wide-awake local anesthesia for clavicle fracture fixation: A case report.

INTRODUCTION: The wide-awake local anesthesia no tourniquet (WALANT) technique has been used in many orthopedic surgeries. The benefits of this technique are the avoidance of the adverse effects of general anesthesia (GA) and the overall reduction of the cost of surgery. However, a literature search revealed no published report on performing the WALANT technique for clavicular fracture surgery. PRESENTATION OF CASE: We report a case of mid-shaft clavicular fracture that was treated with plate osteosynthesis using the WALANT technique in a patient with uncontrolled atrial fibrillation. During

Safety of Tumescent Liposuction Under Local Anesthesia in 9,002 Consecutive Patients.

BACKGROUND: Liposuction has become one of the most popular aesthetic procedures today. Among the different anesthesia methods, tumescent local anesthesia (TLA) has been shown to be the safest. Liposuction is typically performed as an outpatient procedure under minimal oral sedation and without the need for any intravenous (IV) fluid administration. OBJECTIVE: To record complications in a larger series of patients undergoing liposuction in TLA. MATERIALS AND METHODS: Between 2003 and 2020, 9,002 consecutive patients underwent liposuction in TLA with the same team of surgeons. The occurrence o

Percutaneous Endoscopic Thoracic Decompression for Thoracic Spinal Stenosis Under Local Anesthesia.

BACKGROUND: Thoracic spinal stenosis (TSS), a common vertebral degenerative disease, is generally treated via surgical decompression. Percutaneous endoscopic thoracic decompression (PETD) under local anesthesia is considered a relatively safe, minimally invasive procedure. Few reports detail the success of endoscopic decompression for treatment of TSS caused by ossification of ligamentum flavum, the most common cause of TSS. This study investigated application of PETD for treatment of TSS caused by ossification of ligamentum flavum, ossification of posterior longitudinal ligament, or thoracic

Appropriate Prophylactic Antibiotic Use in Clean Wound Surgery Under Local Anesthesia.

BACKGROUND: Although guidelines to prevent surgical site infections (SSIs) were published more than a decade ago, prophylactic antibiotics are still used subjectively in clinical practice. In this study, we evaluated the safety of single-dose preoperative intravenous antibiotics without postoperative antibiotics in the field of clean wound surgery performed under local anesthesia. We also surveyed the present clinical conditions for prophylactic antibiotic use in the plastic surgery departments of training hospitals in Korea. METHODS: A total of 360 consecutive patients who underwent clean wo

Intense, Instantaneous, and Shooting Pain During Local Anesthesia for Implant Surgery.

Administration of local anesthetics is daily routine for most dental practitioners. Normally, the effect is achieved, and no adverse effects are seen. In this article, the authors describe the complications of immediate, intense and shooting pain, numbness, and marked pallor of the cheek, which occurred during infiltration of a local anesthetic in buccal vestibule infiltration. The patients moved suddenly because of pain and marked pallor of the cheek near the root of the nose and lower eyelid pallor was observed. The pain was very short and the injection was performed again after a few minute

Wrist Arthroscopy Under Portal Site Local Anesthesia Without Tourniquet and Sedation.

We had developed the technique of portal site local anaesthesia (PSLA) for wrist arthroscopy. Two percent lidocaine with 1:200,000 epinephrine is injected through a 25G needle to various portal sites. The radiocarpal joint is then distended with saline injection and portal is created with transverse superficial skin incision followed by dilation with curved hemostat. The patient is neither sedated, nor under general or regional anaesthesia. Tourniquet is not routinely used. PSLA can achieve satisfactory comfort level in 88% of our 111 patients without any complication. In well-selected patient

Surgeon-administered conscious sedation and local anesthesia for ambulatory anorectal surgery.

Anorectal procedures are often performed in an outpatient setting using a variety of anesthetic techniques. One technique that has not been well studied is surgeon-administered conscious sedation along with local anesthetic. The purpose of this study was to evaluate the use of this technique with emphasis on safety, efficacy, and patient satisfaction. Chart review was performed on 133 consecutive patients who had anorectal procedures at an outpatient surgery center. Additionally, 65 patients were enrolled prospectively and completed a satisfaction survey. Inclusively, charts of 198 patients wh

Sympathicotomy under local anesthesia: a simple way to treat primary hyperhidrosis.

General anesthesia has been the standard approach for sympathicotomy to treat primary hyperhidrosis. Sympathicotomy through thoracoscopic guidance is a straightforward procedure with a short operation time. Therefore we propose using local anesthesia for the procedure. The advantages of local anesthesia include avoiding endotracheal intubation and systemic anesthetics, and allowing immediate assessment by means of verbal communication with the patient. We effectively treated primary hyperhidrosis in 3 patients by thoracoscopic sympathicotomy under local anesthesia.

Transient obturator neuropathy due to local anesthesia during transobturator sling placement.

The transobturator tape procedure is a popular minimally invasive procedure to treat stress urinary incontinence. A 42-year-old woman with stress urinary incontinence underwent a transobturator sling under intravenous sedation and local injection with a mixture of 0.5% bupivicaine and 1% lidocaine with 1:200,000 epinephrine. After surgery, she had difficulty with adduction of the left leg, consistent with a left obturator neuropathy, which improved after approximately 12 h and completely resolved by the next day. Although the transobturator tape is an effective, minimally invasive procedure th

[Thoracoscopic pleural biopsy under local anesthesia using a 2 mm laparoscope].

Thoracoscopy is indicated in patients with undiagnosed effusion after conventional methods. It has been usually performed under general anesthesia or using a thoracoscope with a thoracoscope with a diameter over 5 mm. However, it is an invasive diagnostic technique. We evaluated the feasibility of thoracoscopic pleural biopsy under local anesthesia using a 2 mm laparoscope. Six patients with a pleural effusion of unknown etiology after conventional methods, underwent thoracoscopy under local anesthesia. [Electrode applicator for endolaryngeal electromyography of the larynx with local anesthesia].

Electromyography of the internal laryngeal muscles is not yet a routine examination. To improve the technique, we developed a new applicator for easy and safe endolaryngeal placement of electrodes, which is now routinely used. Application of the electrodes is successful in a higher percentage of patients than with applicator forceps used previously. Additionally, the danger of displacement of needles within the pharynx or larynx and of injury of the mucosa has been completely eliminated. This application technique can be recommended for endolaryngeal placement of wire electrodes for laryngeal

[Use of Dormicum (midazolam) injection in oral surgery under local anesthesia].

Authors report on the sedative anxiolytic and anterograde amnesia causing effect of "Dormicum", in connection with 170 dental and/or oral surgery interventions. Initial dose of Dormicum was 0.1 mg/body weight kgs,--necessary repetitions occurred with 0.02-0,03 mg/body weight kgs. In 75% of the patients an acceptable level of sedation developed by using 0.1 mg/body weight kgs. In more than 90% of the cases anterograde amnesia could be observed, which seemed to be the most important element of this kind of treatment.

Tumescent technique for local anesthesia improves safety in large-volume liposuction.

The tumescent technique for local anesthesia improves the safety of large-volume liposuction ( > or = 1500 ml of fat) by virtually eliminating surgical blood loss and by completely eliminating the risks of general anesthesia. Results of two prospective studies of large-volume liposuction using the tumescent technique are reported. In 112 patients, the mean lidocaine dosage was 33.3 mg/kg, the mean volume of aspirated material was 2657 ml, and the mean volume of supernatant fat was 1945 ml. The mean volume of whole blood aspirated by liposuction was 18.5 ml. For each 1000 ml of fat removed, 9.7

[Local anesthesia in CO2 laser treatment of disseminated therapy-resistant condylomata].

In an open prospective study 67 patients with refractory genital warts, irrespective of anatomic localization, received CO2-laser vaporization in local anaesthesia, using lidocaine 2% as the local anaesthetic. Sufficient anaesthesia was obtained in all treated patients, with minor pain as the only side effect observed. A single CO2-laser treatment resulted in disappearance of the lesions in 37 (55%) of the patients. Repeated CO2-laser vaporizations in patients with recurrent disease increased the cure rate to 85%. The response rate seemed unaffected by the localization of the warts. It is conc
