A High Index of Awareness About the Inherent Complications of Thoracic Segmental Spinal Anesthesia: A Case of Mastectomy With Bronchiectasis Under Thoracic Segmental Spinal Anesthesia.

General anesthesia is the gold standard for breast cancer surgeries. Considering the nature of the surgery and its associated pain, various regional techniques are used as an adjunct to general anesthesia. Regional anesthesia as a sole anesthetic technique for breast cancer surgery is an upcoming technique - especially in high-risk patients considering the risk-benefit ratio, various regional blocks like pectoralis major block, pectoralis minor block, and erector spinae block - in which thoracic segmental spinal anesthesia is the recent one. Here we present a 68-year-old patient with bronchiec

Spinal Anesthesia and Digital Anxiolysis (SPIDA) for the treatment of lumbar spinal stenosis - a feasibility study.

Lumbar spinal stenosis (LSS) is a prevalent condition, particularly in elderly patients, characterized by a clinical syndrome that includes buttock or lower extremity pain, commonly associated with sensory and motor dysfunction. The surgical management of these patients is challenging due to higher rates of comorbidities and increased risks of experiencing complications such as postoperative delirium, leading to intensive care and prolonged hospital stays. Awake surgery under spinal anaesthesia (SA) has been associated with advantages concerning the occurrence of complications and the clinical

Clinical trial of a method for confirming the effects of spinal anesthesia in patients with spinal cord injury.

In this case series study, we prospectively examined whether it might be possible to check the effect of spinal anesthesia (SA), based on the disappearance of lower extremity reflexes and spasticity, in patients with spinal cord injury (SCI), in whom the effect cannot be confirmed by the pinprick test or by using the Bromage scale. In 40 patients with chronic, clinically complete cervical SCI who were scheduled to receive SA, pre-anesthetic examination revealed that the Babinski sign, patellar tendon reflex, and spasticity (assessed using the Ashworth scale) were all positive in 31 patients, w

Parental decision-making for infant spinal anesthesia - a qualitative study.

BACKGROUND: Spinal anesthesia is a viable alternative to general anesthesia for infant surgical procedures below the umbilicus. However, choosing spinal anesthesia over general anesthesia is a complex medical decision for parents. Little is known about how parents perceive infant spinal anesthesia as an alternative to general anesthesia and how they make this decision for their children. AIMS: The aim of this research study was to generate a holistic understanding of the parental decision-making process for spinal anesthesia in infants. METHODS: A qualitative research study was conducted at a

Rare Complication of Spinal Anesthesia, Acute Subdural Hemorrhage: Case Report.

Acute subdural hemorrhage, following spinal anesthesia, is one of the rarest complication in obstetrics. Unless detected and treated early, it may lead to permanent disability and even maternal death. Post-dural puncture headache is the commonest cause of headache after spinal anesthesia. Severe and pronged headache, refractory to common analgesia, peripheral neurologic symptoms like weakness, mentation change and speech difficulty suggests subdural hemorrhage. Head imaging with CT or MRI is diagnostic. Conservative management or surgical intervention with burr hole and craniotomy are option o

Preventing spinal anesthesia headache in cesarean section: Randomized clinical trial.

OBJECTIVES: Post-dural puncture headache (PDPH) is a common complication following neuraxial block in cesarean sections, typically occurring 12-72 hours postoperatively and leading to considerable challenges and financial costs. We aimed to compare dexametha-sone and paracetamol for preventing spinal anesthesia headaches in cesarean sections. METHODS: A double-blind randomized clinical trial was conducted from December 2019 to April 2020. This study included 215 singleton pregnant women scheduled for elective cesarean section. To prevent PDPH, the patients were allocated to intravenous dexame

Ultrasound-assisted modified paramedian technique for spinal anesthesia in elderly.

BACKGROUND: At present, there are two techniques which are widely applied clinically; the midline and the paramedian. Both methods are difficult for clinicians when treating the elderly. The aim of this work is to explore the feasibility of an ultrasound-assisted modified paramedian technique for spinal anesthesia in the elderly. This would provide clinicians with a new and easy-to-operate technique. METHODS: A total of 150 elderly patients who were scheduled for urology surgery under spinal anesthesia in our hospital were randomly divided into three groups (n = 50): (i) midline technique gro

Anesthesia is a rapidly evolving medical specialty, centered around the patient's safety, and enhancement of surgical outcomes. In their daily practice, anesthesiologists encounter complex challenges, and always search for, and practice the best evidence-based medicine, which allows them to overcome the challenges, and to obtain the maximum benefits of the intervention, with minimal side effects. Regional anesthesia is considered a favorable modality with significant potential in selected patients and is becoming more popular day by day. In this regard, we present a case report of a 65-year-ol

Varicella-zoster virus meningitis after spinal anesthesia: A case report.

BACKGROUND: Headache is a common complication of regional anesthesia. The treatment of post spinal anesthesia headache varies depending on the cause. Although meningitis is rare, it can cause significant harm to the patient. Post dural puncture headache and septic meningitis are the most commonly suspected causes of post spinal anesthesia headache; however, other causes should also be considered. CASE SUMMARY: A 69-year-old woman was scheduled for varicose vein stripping surgery under spinal anesthesia. The procedure was performed aseptically, and surgery was completed without any complicatio

Use of Spinal Anesthesia in Pediatric Laparoscopic Appendectomies: Case Series.

BACKGROUND: Owing to the widespread use of general anesthesia, administration of spinal anesthesia in pediatric patients is not widely practiced. Yet there is ample positive evidence demonstrating its safety, effectiveness, and success. OBJECTIVE: The objective of this study is to compare postoperative patient comfort, length of hospital stay, and cost-effectiveness of pediatric laparoscopic appendectomies performed under spinal and general anesthesia with the usual standard-of-care procedures employed in the hospital. METHODS: This is a case series of 77 consecutive pediatric laparoscopic a

Transient involuntary movement disorder after spinal anesthesia: A case report.

BACKGROUND: Spinal anesthesia is commonly used for various surgeries. While many complications occur after induction of spinal anesthesia, involuntary movement is an extremely rare complication. CASE SUMMARY: Herein, we report the case of a 54-year-old healthy male patient who experienced involuntary movements after intrathecal injection of local anesthetics. This patient had undergone metal implant removal surgery in both the lower extremities; 7 h after intrathecal hyperbaric bupivacaine administration, involuntary raising of the left leg began to occur every 2 min. When the movement disord

Vocal fold paralysis due to intracranial hypotension following spinal anesthesia.

Cranial nerve palsy is a rare neurological complication of epidural and subarachnoid blocks. Most of these complications are attributed to secondary intracranial hypotension due to cerebrospinal fluid leakage following dural puncture. Vocal fold paralysis (VFP) seems more likely to be overlooked and underreported due to delayed onset of symptoms and lack of attribution of dysphonia to spinal anesthesia. Therefore, VFP can often be missed and described as idiopathic. This article describes a case of VFP due to vagus nerve injury following subarachnoid block.

Neuromonitoring for Spinal Cord Stimulation Lead Placement Under General Anesthesia.

Spinal cord stimulation (SCS) is a common therapeutic technique for treating medically refractory neuropathic back and other limb pain syndromes. SCS has historically been performed using a sedative anesthetic technique where the patient is awakened at various times during a surgical procedure to evaluate the location of the stimulator lead. This technique has potential complications, and thus other methods that allow the use of a general anesthetic have been developed. There are two primary methods for placing leads under general anesthesia, based on 1) compound muscle action potentials and 2

Neurological deficit following combined spinal-epidural anesthesia for knee arthroplasty.

A healthy man developed cauda equina syndrome after uneventful combined spinal and epidural anesthesia. No pre-existing neurologic disorder was recorded. There was no pain or paresthesia during needle placement, drug injection or catheter insertion. The sensory levels were improved within a few days following the deficit but little improvement on motor power but not on sphincter tone. Local anesthesia neurotoxicity was thought to be the leading cause of neurologic deficit in our case.

Spinal anesthesia for diagnostic cardiac catheterization in high-risk infants.

BACKGROUND: The main goals of diagnostic cardiac catheterization (DCC) in infants are to evaluate the anatomy and physiology of congenital and acquired cardiac defects while maintaining normal respiratory and hemodynamic variables. The aims of anesthesia for infants undergoing DCC are to prevent pain and movement during the procedure. General anesthesia (GA) or deep sedation could have undesirable respiratory and hemodynamic effects for conducting such studies. Furthermore, GA is associated with increased risks, especially in high-risk infants. Spinal anesthesia (SA) is a successful alternativ
