We present the case of a 69 years old man who was hit by a car while crossing the road. A CT scan of the skull and brain showed fracture of the left occipital bone, bilateral hemispheric subarachnoid hemorrhage, right frontal-temporal-parietal subdural hematoma with a shift of midline structures of 18 mm and complete obliteration of the third ventricle. He showed signs of anisocoria, absence of mobility of all 4 limbs and was immediately intubated and admitted to intensive care. The neurosurgeon was immediately consulted. He underwent drainage of subdural hematoma and two decompressive craniot

An 8-month-old baby girl, who accompanied her sister with an eye problem, was incidentally noticed to have smaller left eye compared to the right. The mother said that it had been present for 2 months. The child showed no detectable focal neurological deficits. Her vision was age appropriate in both eyes. A slight left-sided ptosis was present. Her eye movements were full in both eyes and there was no evidence of strabismus. Bilateral fundus examination was normal. The child had anisocoria (left pupil being smaller than the right one) and left hemifacial anhidrosis, which prompted the diagnosi

Central pontine myelinolysis (CPM), which was originally considered to be the result of rapid correction of chronic hyponatremia, is not necessarily accompanied by hyponatremia or drastic changes in serum sodium level. Here, we report a case of an anorexic 55-year-old male with a history of pharyngo-laryngo-esophagogastrectomy, initially hospitalized with status epilepticus. Although his consciousness gradually recovered as we were controlling his convulsion, it deteriorated again with new onset of anisocoria, and magnetic resonance imaging (MRI) at this point revealed CPM. Rapid change of ser

A 9-year-old neutered male domestic shorthair cat was presented because of coughing, dysphagia, and prostration. Examination revealed firm bilateral anterocervical swellings extensively adhered to the surrounding tissues. Anisocoria with left-side miosis was also evident. X-ray and ultrasound examinations along with a fine-needle aspiration biopsy were performed. Slides were stained with May-Grünwald Giemsa, and a diagnosis of salivary gland carcinoma was made. At surgery, the tumor was found to involve both submandibular salivary glands as well as adjacent lymph nodes and surrounding tissues.

A 72-year-old woman, previously in good health, suffered for 2 years from attacks of a complex neuropsychiatric syndrome lasting from 2 days to 4 weeks. These episodes, which were followed by intervals of nearly complete recovery, were usually marked by fever, headache, mental confusion, vomiting ataxia, anisocoria, epileptic seizures and stiffness of the neck. Neuroradiological investigation revealed only slight ventricular dilatation. In 13 CSF examinations, there was reduced glucose, increased protein, considerable rise of the IgG index and constant oligoclonal bands. There was also an incr

A 5-year-old male golden retriever was presented after a subacute onset of left-sided Horner syndrome (HS). The dog had anisocoria with left-sided miosis, ptosis of the upper eyelid, and third eyelid protrusion in the left eye. Because of the absence of additional neurological abnormalities, clinical signs were suggestive of left isolated HS, and the lesion was localized at the level of either the preganglionic or postganglionic neuron of the sympathetic chain. Magnetic resonance imaging (MRI) of the head and total body computed tomography (CT) identified marked narrowing and irregularity of t

A young adult female in her 20s presented with gradually progressive exodeviation of the left eyeball from the last 3 years. She had mild ptosis and proptosis in her left eye with 3 mm of anisocoria. On examination, she was found to have 60 prism dioptres exotropia with minimal motility limitation (-1 adduction, elevation and depression) in the left eye. With a provisional diagnosis of acquired third nerve palsy, she underwent MRI brain and orbit with contrast, which showed enhancing nodular lesion along the course of the left oculomotor nerve in the prepontine cistern which was hyperintense o

A young free-ranging black-horned capuchin (Sapajus nigritus) with a history of ataxia, anisocoria, diminished threat and pupil reflexes and increased cranial circumference was referred for evaluation to the Medicine and Wildlife Conservation Section, Federal University of Paraná, Paraná, Brazil. Due to the clinical presentation and radiographic findings of hydrocephalus, euthanasia was performed. Necropsy revealed a dark red mass (1.5 × 1.8 cm) in the region of the third ventricle and marked dilation of the lateral ventricles. Histopathological findings were characterized by proliferation of

Homocysteine is a type of amino acid that isn't genetically encoded by the human body. This amino acid is capable of causing oxidative damage to the endothelial cells, leading to the onset of thrombosis. Moreover, it can also inflict harm to neurons by activating pro-apoptotic factors, causing DNA damage, and inducing oxidative stress, as observed in various animal models and cell cultures. This case report highlights a four-year-old girl who exhibited signs of an ischemic stroke. The neurological examination revealed several symptoms, including anisocoria, decreased tone, decreased power, abs

An 18-month-old European shorthair cat was presented with a two week history of progressive decrease in consciousness, ambulatory tetraparesis, moderate ataxia and generalised decreased-to-absent postural reactions. Bilateral facial and nasal hypalgesia, absent menace response and anisocoria were found, and segmental spinal reflexes were normal. Neurological signs progressed to nonambulatory tetraparesis, tremor and spinal hyperalgesia. Histopathological examination revealed a mild-to-moderate lymphoplasmacytic and histiocytic infiltration, predominantly in the dorsal spinal roots, cranial ner

We report a case of partial oculomotor nerve palsy due to spontaneous arteriovenous fistula. A 62-year-old man noticed double vision on downward gaze and periorbital pain. Clinical examination revealed inability to move the left eye downwards and to the right, and also anisocoria. No conjunctival injection or proptosis was noted. Findings of computed tomographic scan, magnetic resonance imaging and magnetic resonance angiography of the orbits, cavernous sinus region and brainstem were all normal. Carotid angiography demonstrated arteriovenous fistula via the left ophthalmic artery, the dural b

IMPORTANCE: Apraclonidine, a topical α-2 adrenergic agonist, reverses anisocoria in patients with Horner syndrome, a disruption of the sympathetic chain to 1 eye and side of the face. In pediatric patients with anisocoria, apraclonidine is used off-label to exclude Horner syndrome.
OBJECTIVE: To identify and characterize postmarketing case reports reported to the US Food and Drug Administration and reported in the medical literature of systemic toxicity in pediatric patients administered apraclonidine ophthalmic drops.
DESIGN, SETTING, AND PARTICIPANTS: A search of the US Food and Drug Admin

Simultaneous complete oculomotor nerve palsy in herpes zoster ophthalmicus (HZO) is rare. We report a case of a 65-year-old lady who presented with a right drooping eyelid for 2 days associated with rashes over her right forehead. Examination showed crusted vesicular lesions on the right V1 dermatome with right complete ptosis, anisocoria, and limited right extraocular movement on all gazes except full movement on abduction. The anterior chamber showed mild right eye anterior chamber inflammation and bilateral fundus examinations were normal. Other neurological examinations were normal. Neuroi

Diagnosing Horner Syndrome can be difficult in the setting of an incomplete triad. A 27-year-old man presented with unilateral eyelid droop and intermittent ipsilateral headaches, having already seen 7 physicians. Physical examination revealed unilateral ptosis but no pupillary miosis or facial anhidrosis. Inspection of his clinical photographs revealed elevation of the ipsilateral lower eyelid, suggesting sympathetic dysfunction. On further questioning, he admitted to naphazoline dependence. Reexamination after ceasing the naphazoline unveiled the anisocoria. Vascular imaging subsequently rev

We report the case of a 56-year-old man with acute onset of de-novo stabbing, pulsating and diffuse headache with subsequent appearance (within few minutes) of posterior fossa symptoms (vomiting, postural instability, anisocoria, incoordination, dysarthria, retropulsion) lasting 9-12 h. Recurrent hypertensive crises were detected during the acute observation in the Emergency Room, even in the absence of previous history of hypertension. Once subarachnoid hemorrhage and focal lesions (vascular and non-vascular) were excluded, brain computerized tomography-angiography and digital subtraction ang

INTRODUCTION: We report a case of neurogenic pulmonary oedema (NPO) following massive left cerebral infarct, which was initially misdiagnosed as acute myocardial infarction (AMI).
CLINICAL PICTURE: This 52-year-old man presented with acute loss of consciousness with normal brain computed tomography (CT). He was treated as non-ST-elevation AMI complicated with pulmonary oedema based on findings of chest radiograph (bilateral pulmonary oedema), electrocardiogram (marked ST-T changes in leads V3 to V6), and cardiac enzymes [elevated creatinine kinase (CK) and CK-MB]. However, coronary angiogram

In pharmaceutical companies and research institutes, many toxicity tests are performed with laboratory animals. This study was performed to produce reference data for eye toxicity tests and to investigate the ophthalmic diseases of 408 ICR mice and 119 BALB/c mice, which are commonly used as subjects in toxicity tests. The experimental animals without clinical disorders were selected regardless of sex. The ophthalmic diseases were examined by using special ophthalmic instruments: direct ophthalmoscope, indirect ophthalmoscope, slit-lamp biomicroscope and focal illuminator. The most prevalent o
