[Imaging of aneurysms and dissections of the aorta. Part 1--Aneurysms of the abdominal aorta].

The authors review the main imaging findings of aneurysms and dissections of the aorta. Based on the experience and imaging techniques available in their institution (Conventional Radiology, Sonography and Computed Tomography), they try to make a decision tree of radiologic procedures in ambulatory and emergency patients. Nowadays, in the majority of situations, these techniques allow the Radiologist to make the correct diagnosis, detect the complications and evaluate the patients in the pre and post-surgical phases. Other imaging techniques not available in their institution, such as Magnetic

Spontaneous dissecting aneurysm of the common iliac artery.

A 52-year-old man sought medical advice for sudden onset of intermittent claudication of the left lower limb after 50 meters walking. Aortography documented a dissecting aneurysm limited to the left common iliac artery. After resection, a prosthetic graft was inserted. Pathology showed that the cause of the dissection was atheroma. Spontaneous dissecting aneurysm of the common iliac artery is rare. Rupture represents the principal hazard. A high index of suspicion should lead to diagnosis as soon as signs of lower limb ischemia, occasionally transient, appear.

[Aneurysm of the subrenal abdominal aorta spontaneously rupturing into the inferior vena cava].

The authors report a new case of spontaneous rupture of an infrarenal abdominal aortic aneurysm into the inferior vena cava operated with a good haemodynamic result. However, the patient died of multiple postoperative complications and an associated left bronchopulmonary carcinoma. The following features are emphasised: the contribution of complementary investigations to the diagnosis of this condition: abdominal ultrasonography, aortography and above all, especially in future years, of digitised angiography; the clinical importance of ischaemia of the descending colon and acute postoperative

Acute disease of the aorta. Diagnosis by computed tomography and ultrasonography.

CT and ultrasonography, alone or in combination, have partially replaced aortography for the detection of abnormalities of the native and grafted abdominal aorta. In our medical center, ultrasonography is used to detect or confirm the diagnosis of aortic aneurysm and to determine its size, extent, and relationship to the renal arteries. We usually choose CT to assess suspected rupture of the native aorta and complications of the grafted aorta, such as occlusion, leakage, false aneurysm, or perigraft infection. Acute disease of the abdominal aorta can be detected or excluded using these relativ

[Compression of the celiac tripod by the arcuate ligament of the diaphragm].

A case of external compression of the coeliac trunk on the part of the lateral arcuate ligament is reported. A brief account of the questions still posed by Dunbar's syndrome is followed by the assertion that dynamic, not static, aortography must be performed since it is in itself sufficient to clinch the diagnosis. It is suggested that pain is not an expression of reduced arterial flow, but of compression of the coeliac plexus, with the results that surgical management must associate adventitiectomy with section of the arcuate ligament. This will ensure displacement of the plexus, and prevent

A Young Man with Hematemesis.

Primary aortoenteric fistula (AEF) is a rare but fatal cause of upper gastrointestinal bleed. A 27-year-old previously healthy man presented with massive hematemesis, melena, severe anemia and hemodynamic instability. Point-of-care ultrasound and chest radiography suggested thoracic aortic pathology, and CT aortography confirmed a descending thoracic aneurysm with contained rupture and fistulous communication with the esophagus. This case highlights the need to consider rare vascular causes in massive hematemesis, where early imaging can facilitate rapid diagnosis and timely intervention.

Huge renal arteriovenous malformation mimicking a simple para-pelvic cyst.

The presenting symptoms of renal arteriovenous malformation are usually gross hematuria and hypertension. Herein we present an unusual case of a huge renal arteriovenous malformation without these signs, but with an ultrasound picture mimicking a simple para-pelvic cyst. Other imaging tests, including duplex ultrasound, computerized tomography and aortography, demonstrated the vascular lesion. We suggest that duplex ultrasound should accompany routine renal ultrasound in order not to miss such cases, especially when the physical examination suggests an intra-abdominal vascular lesion or bleedi

Left atrial myxoma causing fluttering of the anterior mitral leaflet.

This report describes the M-mode echocardiographic features of a left atrial myxoma, before and after excision of the tumor. Preoperative M-mode echocardiogram demonstrated fine diastolic fluttering of the anterior leaflet of the mitral valve which disappeared after surgery. Preoperative aortography and minutious examination of the aortic valve during operation excluded any associated lesion of the aortic valve. The fluttering of the anterior mitral valve is probably related to turbulent blood flow around the tumor prolapsing between mitral leaflets during diastole.

Outcomes of routine surveillance of grade 1 blunt traumatic aortic injury.

BACKGROUND: Blunt traumatic aortic injury (BTAI) can present with a wide range of severity from mild intimal injuries (grade 1) to rupture (grade 4). Although there is consensus that patients with grade 3 and 4 injuries should undergo immediate thoracic endovascular aortic repair (TEVAR), the optimal treatment strategy for milder injuries is less clear owing to the lack of data regarding the natural history of these injuries. Grade 1 injuries are typically treated with anti-impulse therapy and surveillance imaging. However, the efficacy of this approach remains unclear. Therefore, the aim of t

[Evaluation of regurgitant fraction of aortic regurgitation by means of pulsed Doppler echocardiography as compared with cine angiography].

The regurgitant fraction (RF) of aortic regurgitation (AR) was calculated using pulsed Doppler echocardiography (PDE), and the results were compared with those of aortography. Two-dimensional echocardiography (2DE), PDE and cine angiography were performed for 33 patients, including nine in whom aortic regurgitation (AR) was ruled out by contrast 2DE and 24 in whom AR was confirmed by aortography (AOG). The latter were subdivided into six of Sellers' degree I, seven of degree II, eight of degree III and three of degree IV. Sampling was obtained from the main pulmonary artery, the left ventricul

Incidentally diagnosed Takayasu arteritis on thyroid ultrasonography showing prominent collateral vessels of thyroidal arteries and common carotid artery occlusion.

We report a case of middle-aged woman incidentally diagnosed with Takayasu arteritis during the ultrasonography of a thyroid gland nodule. Prominent collaterals of the thyroidal arteries and a thin common carotid artery with mural thickening and deficient intraluminal flow signals were initially depicted on the ultrasonography with color Doppler. Subsequent magnetic resonance angiography and computed tomography aortography confirmed the diagnosis with the imaging features of a bilateral long segment common carotid artery occlusion and segmental stenosis of the left subclavian artery in additio

[A case of thromboexclusion for thoracic descending aortic pseudoaneurysm with Behçet's disease].

A 59-year-old woman with Behçet's disease was admitted because of hemoptysis. She had undergone resection and patch closure of a thoracic descending aortic aneurysm 6 years before this current admission. The aortography after the first operation had shown a pseudoaneurysm at the operative site. In the current operation, we performed thromboexclusion with a temporary occlusion balloon in the distal thoracic descending aorta above Adamkiewicz. The post operative course was uneventful. Postoperative angiography demonstrated exclusion of the pseudoaneurysm and the patient was discharged without re

Traumatic rupture of the aorta with ductus diverticulum: a case history.

Traumatic rupture of the aorta is a life-threatening injury with an extremely high mortality rate if not promptly diagnosed and surgically repaired. Accurate diagnosis depends on a high index of suspicion and the correct assessment of the angiographic features revealed by aortography. A case is presented that illustrates the angiographic findings in a patient with traumatic rupture of the aorta with an accompanying ductus diverticulum. A ductus diverticulum, which is located just distal to the aortic isthmus, can be confused with an aortic tear angiographically and vice versa. The case discuss

Pseudoaneurysm of the aortic root diagnosed by noninvasive imaging: report of a case.

Pseudoaneurysm of the aortic root is a rare condition typically diagnosed by the use of cardiac catheterization and aortography or by surgical exploration. This case describes a patient who was shown to have a pseudoaneurysm of the aortic root without evidence of any acquired etiologic factors other than atherosclerotic vascular disease. Two new advanced techniques of noninvasive imaging were used in confirming the diagnosis: transesophageal echocardiography and gated magnetic resonance imaging. This case highlights the value of techniques such as these to reliably assess the heart and great v

Quadricuspid aortic valve. Case report and review of the literature.

Quadricuspid aortic valve is a rare congenital anomaly that may be found unexpectedly at surgery or diagnosed preoperatively by means of echocardiography or aortography. The most common hemodynamic abnormality associated with this anomaly is aortic insufficiency. We describe a representative case involving a 62-year-old man with severe aortic insufficiency and mild aortic stenosis. At operation, the aortic valve was found to have 4 cusps. The valve was replaced with a mechanical prosthesis. The patient was released 7 days after operation and was clinically well at 6-month follow-up.

Oesophageal lung with systemic arterial blood supply.

An adolescent girl with congenital bronchopulmonary-foregut malformation (BPFM) is reported. The patient had a hypoplastic, non-functioning right lung with the main bronchus originating from the oesophagus. The pulmonary artery of the "oesophageal lung" was supplied by systemic circulation from the descending aorta, and the venous flow drained through a hypoplastic right pulmonary artery (RPA) into the large left pulmonary artery (LPA). This case was a rare type of left-to-right shunt and demonstrates that oesophagography, pulmonary angiography and aortography are important in demonstrating th

Hemiazygous continuation of a left inferior vena cava: misleading radiographic findings in chest trauma.

A widened mediastinum was found on chest radiography following blunt chest trauma. Aortography alone could not adequately determine the etiology of the radiographic findings; on computed tomography (CT) benign mediastinal widening was confirmed owing to accessory hemiazygous continuation of a left inferior vena cava (IVC), with communication to the left brachiocephalic vein through an enlarged left superior (highest) intercostal vein. This unusual anatomic variant and its radiographic findings are reviewed, as well as the complementary role of CT in the evaluation of the traumatized thoracic a

[Clinical and angiographic aspects of medullary angiomas].

The authors present six personal cases of medullary angiomas: three cases of retromedullary, one of intramedullary and two of intra-retromedullary vascular malformations. One of two last cases was associated with a paravertebral arteriovenous malformation. The neuroradiologic investigations include a water-soluble myelography, with hypocicloidal tomography, and spinal cord selective angiography. The latter present many advantages of the aortography, for feeding arteries visualization, but the site of the shunt inside the intervertebral foramen can be localized only by angiotomography. Emergency Endovascular Repair for the Non-anastomotic Rupture of a 14-Year-Old Reverse Lock-Knitted Dacron Graft.

An 80-year-old man who underwent abdominal aortic replacement 14 years earlier collapsed suddenly and was rushed into our emergency room. In a state of confusion on arrival, he ended up experiencing cardiopulmonary arrest. Cardiopulmonary resuscitation was successful, and a contrast-enhanced computed tomography revealed a massive retroperitoneal hemorrhage around the bifurcated prosthetic graft. Aortography revealed a non-anastomotic rupture of the graft's body. An emergency endovascular repair was successfully performed; however, the patient died due to disseminated intravascular coagulation

An unusual cause of spontaneous pneumothorax.

Bronchopulmonary sequestration is a congenital pulmonary malformation consisting of a non-functioning region of lung parenchyma that receives systemic arterial blood supply. The authors present the case of a 31-year-old male who presented with spontaneous pneumothorax as the initial clinical manifestation of an intralobar bronchopulmonary sequestration. There was a persistent air leak through the chest tube which remained in the pleural cavity for 12 days. CT revealed an area of increased opacity, with focal calcifications and a small eccentric cavity in the posterior basal region of the left
