Models of praxis have posited two major components, production and conceptual. Conceptual praxis disorders may occur in two domains: associative knowledge (tool-action associations such as hammer pound; tool-object associations such as hammer nail) and mechanical knowledge such as knowing the advantage that tools afford. Patients with Alzheimer's disease not only have conceptual apraxia (CA) but can dissociate CA from language deficits and from praxis production deficits (ideomotor apraxia). These findings suggests that knowledge about tools (action semantics) is independent of verbal semantic

The association of aphasia, apraxia and agnosia with cortical but not subcortical dementias, is a widely held belief. The purpose of the present study was to determine the frequency of aphasia, apraxia, and agnosia in groups of cortical and subcortical dementia patients, and to assess the diagnostic utility of these symptoms. Subjects were 64 patients with subcortical dementias (Parkinson's disease and normal pressure hydrocephalus) and 192 patients with cortical dementia (probable Alzheimer's disease) matched for sex, age, and Mini-Mental State Examination score. Each patient was evaluated fo

Competing models of the basis of limb apraxia were tested through analysis of joint coordination deficits in three apraxic subjects with lesions that included the left parietal lobe. Three-dimensional shoulder, elbow, wrist and hand trajectories were recorded for repetitive 'slicing' gestures made in a series of conditions in which contextual cues were introduced in a graded fashion. The apraxic subjects showed marked deficits in joint coordination across context conditions. Even when actually manipulating a tool and object, the apraxic subjects failed to show proper joint synchronization, fai

Slowly progressive apraxia due to Alzheimer's disease was encountered in a 66 year old, right handed man whose initial impairments included coordinated movements of the left hand and some features of the alien hand syndrome. Over four years, the patient developed progressively worsening deficits of memory and language. A biopsy of his right temporal lobe showed numerous plaques and neurofibrillary tangles. Pronounced right parietal lobe hypoperfusion on serial SPECT suggests involvement of this region in contralateral praxis.

Motor performance in 11 patients with ideomotor apraxia, 11 with aphasia without such apraxia, and 11 normal controls was compared. These three groups were matched on age, sex, education, severity of aphasia, intelligence, and size of lesion. Measures of aiming, tapping, line-following, and steadiness developed by Schoppe in 1974 were used. Both apraxic and aphasic groups showed difficulties with motor performance, and the data of the apraxic group were poorer than those of the aphasic group. These results were consistent with Liepmann's theory.

The authors describe a case of vertical congenital ocular motor apraxia (COMA). The pathways of vertical saccades and pursuits are briefly outlined, and a possible cause of vertical COMA is suggested. A neuropathological correlate is needed to confirm the cause of both vertical and horizontal ocular motor apraxia.

A student was diagnosed as having Wilson's disease only after the severe, intermittent inability to open his eyes led him to seek neurologic evaluation. Although lid-opening apraxia is usually a symptom of diffuse extrapyramidal disease, it has not previously been reported in Wilson's disease.

This investigation describes a treatment program for an adult patient with apraxia of speech. The program was composed of a series of physiologic (nonspeech) and phonetic (articulatory) tasks that began with oroneuromotor control activities and progressed to consonant-vowel (CV) syllable, word, and sentence drills. All activities were paced by a metronome. Detailed descriptions are provided about the (a) nature of the specific treatment steps, (b) sequence of steps followed, (c) criteria used for progression within and between steps, (d) actual number of trials and time required to reach crite

A 10-month-old girl presented with apparently unilateral ocular motor apraxia (OMA). Electro-oculography confirmed an inability to generate a saccade for fixation or in response to an optokinetic or vestibular evoked stimulus to the right. Fixation and optokinetic saccades and vestibular evoked nystagmus were present to the left. Magnetic resonance imaging revealed a small cerebellar vermis but no localizing unilateral abnormality or tumour. Subsequent review of video recordings of 16 children with congenital OMA revealed that 5 demonstrated asymmetry in the generation of fixation saccades. Tw

Liepmann postulated that the left hemisphere of right-handed persons contains the "movement formulas" that control purposeful skilled movements of the limbs on both sides of the body. Accordingly, in right-handers apraxia should follow damage to the left hemisphere, whereas right hemisphere damage should not lead to apraxia. Although this is generally true, we recently examined a right-handed man who after a right hemispheric stroke became aphasic and apraxic with his nonparalyzed right hand. Our observations suggest that the right hemisphere of this right-handed man made a critical contributi

We studied a patient who had unilateral (left) limb apraxia associated with a lesion of her right superior (Brodmann areas 5 and 7) and inferior parietal lobes. In monkeys areas 5 and 7 contain "hand-manipulation neurons" that are thought to be important in proprioceptive guidance. Therefore, we tested our patient with her eyes open and closed. Her left hand performance with pantonime, imitation, and use of actual objects dramatically deteriorated in the absence of visual guidance. In addition, although the patient did not have optic ataxia and had normal proprioception, she had difficulty usi

The case of a 4 1/2-year-old boy with congenital vertical ocular motor apraxia who was otherwise developmentally and neurologically normal is reported. The presence of perinatal hypoxia in this patient may have been etiologic. While the presence of a supranuclear vertical ocular motor abnormality usually suggests a serious, acquired neurologic or systemic disease it may rarely occur as an isolated congenital finding, as demonstrated in this case.

Articulatory anticipation of vowel gestures was assessed in an apraxic patient, a dysarthric patient, and three normal speakers. The technique of assessment included perceptual identification of gated speech stimuli. The speech material consisted of /getVte/ utterances with the target vowels /i/, /y/, and /u/. In the case of the apraxic patient the gated vowels were identified at a later instant relative to the preceding plosion burst than in the normal speakers. This result was interpreted as reflecting a delayed onset of coarticulatory gestures, in particular lip rounding. The identification

It is understood that damage to the left cerebral hemisphere in adulthood may result in syndromes of language disturbances called the aphasias. The study of these syndromes sheds light on normal language processes, the relationship between language behavior and the brain, and how best to treat aphasic individuals. Aphasia, for some, is a central communication disorder affecting all symbolic behavior in all modalities (i.e., speech, writing, and gesture). Difficulty producing symbolic gestures on command is called apraxia. Others view aphasia as a manifestation of a motor-sequencing disorder af

Lesion size correlated positively with the severity of apraxia in the acute (one month) and chronic (one year) stage in 177 stroke patients, but accounted for only 25 per cent of the variance of the apraxia scores. Apraxia was measured on a standardized battery and defined in relation to normal controls. Lesions visualized by CT were traced objectively and measured on standard templates. Small lesions (less than one-tenth of the hemispheric volume) producing moderate or severe apraxia were mostly frontal and close to the body of the lateral ventricle. The lesions of the deep parieto- and occip

Spontaneous language samples of eight children diagnosed as presenting developmental verbal apraxia were analyzed in terms of their mean length of utterance, Developmental Sentence Scores, use of 14 grammatical markers, and yes-no and wh-question forms. Although MLUs were greater than the range associated with stage V syntactic development, all children demonstrated difficulties with stage V and beyond grammatical markers and many omitted stage II markers. Developmental Sentence Scores were generally well below chronologic age expectations and revealed notable difficulties with personal pronou

A quantitative test of fine motor function was used to compare the dexterity of the right and left hands of normal children and children with developmental apraxia of speech (DAS). The results suggest that the left cerebral hemisphere of children with DAS is not predominantly impaired in regard to motor control of the upper extremities. In neither female nor male children with DAS was the manual dexterity of the right hand less than that of the left. In female but not male children with DAS, manual dexterity of both hands was impaired when compared to the normal control groups. Therefore, DAS
