Interleukins in diagnosis of perinatal asphyxia: A systematic review.

BACKGROUND: Biochemical markers including interleukins (ILs) has been proposed for early diagnosis of asphyxia. OBJECTIVE: This study has aimed to systematically review the significance of IL measurements in the diagnosis of perinatal asphyxia. MATERIALS AND METHODS: PubMed, Cochrane Library, Web of Science, Embase, and Scopus databases before 2017 were searched for the following keywords: asphyxia, neonatal, interleukin, and diagnosis. A total of 13 out of 300 searched papers were finally selected for evaluation. Interleukins under study were IL6 and interleukin 1 β (IL-1 β ). Interleukins

Troponin-T as a biomarker in neonates with perinatal asphyxia.

BACKGROUND: Troponin-T is a commonly used cardiac biomarker, which could be useful in perinatal asphyxia. We aimed to analyze troponin-T concentrations in asphyxiated neonates and to correlate the concentrations with clinical outcomes. METHODS: Data were collected from electronic medical records of neonates diagnosed with perinatal asphyxia over a period of four years. RESULTS: There were 63 neonates with moderate to severe encephalopathy, in whom serial troponin-T concentrations had been done on days 1, 3, and 7. 53 (84%) asphyxiated infants had troponin-T concentration >100 pg/ml at 2-4 h

Perinatal asphyxia: a review from a metabolomics perspective.

Perinatal asphyxia is defined as an oxygen deprivation that occurs around the time of birth, and may be caused by several perinatal events. This medical condition affects some four million neonates worldwide per year, causing the death of one million subjects. In most cases, infants successfully recover from hypoxia episodes; however, some patients may develop HIE, leading to permanent neurological conditions or impairment of different organs and systems. Given its multifactor dependency, the timing, severity and outcome of this disease, mainly assessed through Sarnat staging, are of difficult

Effects of hypothermia for perinatal asphyxia on childhood outcomes.

BACKGROUND: In the Total Body Hypothermia for Neonatal Encephalopathy Trial (TOBY), newborns with asphyxial encephalopathy who received hypothermic therapy had improved neurologic outcomes at 18 months of age, but it is uncertain whether such therapy results in longer-term neurocognitive benefits. METHODS: We randomly assigned 325 newborns with asphyxial encephalopathy who were born at a gestational age of 36 weeks or more to receive standard care alone (control) or standard care with hypothermia to a rectal temperature of 33 to 34°C for 72 hours within 6 hours after birth. We evaluated the n

Inflammatory mediators as predictors of outcome in perinatal asphyxia.

OBJECTIVES: To identify biomarkers for neuronal injury and outcome in perinatal asphyxia. METHODS: This prospective cohort study was done in authors' level III NICU involving 80 neonates - 40 babies with perinatal asphyxia and 40 weight and gender matched normal neonates. Levels of cytokines IL-6, IL -1β, IL-2 and TNF -α in cord blood of these neonates were estimated and correlated with the severity of asphyxia and developmental outcome at 6 mo using Baroda Developmental Score. RESULTS: The baseline parameters revealed that there was no statistically significant difference between the two gr

Death as a result of violent asphyxia in autopsy reports.

Violent asphyxia can be subdivided into various kinds according to the mechanism, so that the resuscitation techniques are different in each case. The purpose of the present article was to analyze the autopsy reports of the Department of Forensic Medicine of the Medical University in Wroclaw, Poland of 2010, in which the established cause of death was violent asphyxia. We found that among the 890 autopsies performed, there were 164 cases of death due to violent asphyxia caused by drowning, choking on food, gastric fluid, or blood, hanging, manual strangulations, immobilization of the chest (po

Pathological changes in the thyroid gland in crush asphyxia.

To determine whether crush asphyxia may be associated with macro- and microscopic changes in the thyroid gland, four cases of death due to crush asphyxia were evaluated where the decedents (males aged 36, 37, 45, and 65 years respectively) suffered lethal chest compressions. The diagnosis of crush asphyxia in each case was suggested by the death scene description and confirmed by the finding of injuries to the torso, with marked congestion of the face, neck, and upper body associated with petechial and subconjunctival hemorrhages. In addition to other pathological findings, each decedent had i

Magnetic resonance imaging in neonates with total asphyxia.

Magnetic resonance (MR) findings in cases of total asphyxia, whose lesions are mainly in the brainstem and deep nuclei, have not been clarified. In this study, we investigated MR images in neonates with total asphyxia. MR images of six infants (three males and three females; gestational age, 35-39 weeks; birth weights, 1880-3572 g) with total asphyxia were examined. In all subjects, neonatal cortical MR lesions were limited to the hippocampus with highlighting on T1-weighted imaging (T1-WI). The neonatal MR lesions of the cerebral white matter were limited to the white matter between the insul

[Analysis of the evitability of intrapartum asphyxia with a peers review].

OBJECTIVE: To assess the quality of intrapartum care in birth asphyxia cases. METHODS: Prospective analysis of all cases of birth asphyxia in nine maternity units during one year (2010). Birth asphyxia was defined as the combination of at least one clinical factor (Apgar≤7 at 5 minutes, signs of encephalopathy at birth) and at least one biological factor in cord (pH≤7, BD≥12 mmol/L, lactates>10 mmol/L). These cases were analyzed with a peer review from French guidelines 2007. RESULTS: Fifty cases of birth asphyxia were identified. After peer-review, they were defined as 46% non preventable,

[Obturation, aspiration, and inhalation in case of mechanical asphyxia].

This paper aims at updating terminology employed for the characteristic of selected forms of mechanical asphyxia. The medical terms "obturation", "aspiration", and "inhalation" are not infrequently used by forensic medical experts and pathological anatomists engaged in diagnostics of mechanical asphyxia, elucidation of the mechanism of the accompanying injury and causes underlying its fatal outcome. It is argued that the use of these terms for the purpose is sometimes either unjustified or incorrect or both. To begin with, they have different meanings in different situations. Second, the mecha

Infant asphyxia, soft mattresses, and the "trough" effect.

Although unexpected infant death in a cot has traditionally been attributed to sudden infant death syndrome, careful evaluation of death scenes and sleeping environments has increasingly identified deaths due to accidental asphyxia from so-called sleeping accidents. The case of a 5-month-old infant boy who was found facedown and unresponsive in a wooden portable cot with a sagging canvas base is reported to illustrate another potentially lethal situation. Although the autopsy revealed no specific findings, examination of the cot showed a significant depression caused by the sagging canvas base

[Positional asphyxia, a cause of death insufficiently known].

Positional asphyxia (AP) is a fatal condition arising because of the adoption of particular body positions, causing mechanical interference. Consequences are important alveolar hypoventilation and cardiac hyperexcitability due to respiratory acidosis in combination with extensive liberation of catecholamine occurring in attracted individuals sustaining physical restrain. This syndrome can occur in various circumstances and is mostly observed in situations with physical restraint and in combination with excited delirium (ED). The diagnosis is essentially based on three criteria: body position o

Fatal accidental asphyxia in a jack-knife position.

Accidental death from postural or positional asphyxia takes place when the abnormal position of the victim's body compromises the process of respiration. Diagnosis is largely made by circumstantial evidence supported by absence of any other significant pathology or trauma explaining death. This case report is about a 50-year-old male who had been drinking the previous night and was found dead in the morning inside a tire repair shop. His jack-knifed body had been encompassed, buttocks-down, within the hollow core made by 3 big tires stacked on top of each other. The author was called to the sc
