[Disorders of heart rhythm and conduction in dilated and hypertrophic cardiomyopathies].

Idiopathic cardiomyopathies are characterized by diversity of clinical manifestations, among which heart rhythm abnormalities are the most common. The authors carried out qualitative and quantitative evaluations of heart rhythm abnormalities in patients with dilated and hypertrophic cardiomyopathies (DCMP, HCMP) and compared those abnormalities according to the data of daily ECG monitoring. Forty patients with DCMP and 30 with HCMP were examined. In 8 (25%) patients with DCMP and in 2 (6.7%) with HCMP, permanent atrial fibrillation was recorded. Among patients with sinus rhythm, supraventricul

Prevalence, characteristics and significance of ventricular tachycardia (three or more complexes) detected with ambulatory electrocardiographic recording in the late hospital phase of acute myocardial infarction.

A 24 hour electrocardiographic recording was performed before hospital discharge in 430 patients who survived the cardiac care unit phase of acute myocardial infarction. Fifty patients (11.6 percent) had ventricular tachycardia, that is, three or more consecutive ventricular complexes. In 25 (50 percent) of these 50 patients, there was only one episode of ventricular tachycardia and, in 15 patients (30 percent), the longest run of ventricular tachycardia was only three consecutive ventricular premature depolarizations. The average rate of tachycardia was 119/min. Tachycardia rarely started wit

Ventricular ectopy and QTc-interval prolongation associated with dronedarone therapy.

Amiodarone is an effective treatment for atrial and ventricular arrhythmias, but its use is limited by a toxic adverse-effect profile. Although dronedarone has been touted as an antiarrhythmic agent devoid of both solid organ toxicity and proarrhythmic properties, its potential for prolonging ventricular repolarization may augment triggered ectopy. We describe a 66-year-old man who began dronedarone 400 mg twice/day for new-onset paroxysmal atrial fibrillation; he had no left ventricular dysfunction or clinical heart failure. Three months after starting the drug, he complained of malaise, fati

Augmented sympathoadrenal activity during treadmill exercise in patients with Wolff-Parkinson-White syndrome and atrial fibrillation.

It is believed that reciprocating tachycardia and accessory pathways play important roles in atrial fibrillation (AF) in patients with Wolff-Parkinson-White (WPW) syndrome. However, the mechanism by which AF occurs is not yet fully understood. This study was performed to evaluate the contribution of sympathoadrenal activity to the onset of AF in patients with WPW syndrome. Symptom-limited treadmill exercise testing was performed and plasma norepinephrine and epinephrine concentrations were measured simultaneously in 27 patients with WPW syndrome and 20 control subjects. In 13 patients with WPW

Simultaneous anterograde fast-slow atrioventricular nodal pathway conduction after procainamide.

UNLABELLED: Three patients with paroxysmal supraventricular tachycardia underwent electrophysiologic studies that included His bundle recordings, incremental atrial and ventricular pacing and extrastimulation before and after intravenous infusion of 500 mg of procainamide. In all three patients the tachycardia was induced during atrial pacing or premature atrial stimulation, or both. Two of the three patients had discontinuous atrioventricular (A-V) nodal curves with induction of a slow-fast tachycardia during failure in anterograde fast pathway conduction and one patient had a smooth A-V noda

Evaluation of the efficacy and safety of ethacizine in the management of premature atrial and ventricular contractions - a prospective study (ETHAPRO).

BACKGROUND: Premature atrial contractions (PACs) and premature ventricular contractions (PVCs) are among the most frequently observed arrhythmias in clinical practice. They represent early depolarizations originating from the atria or ventricles, respectively, and are often detected in both healthy individuals and patients with cardiovascular disease. PATIENTS AND METHODS: In the ETHAPRO study, 98 patients (58 males, 40 females; mean age 45 years) with either PACs (n = 46) or PVCs (n = 52) were enrolled. All participants had an ectopic burden of >1000 premature beats per 24 h despite prior an

[Usefulness of adenosine to detect advanced paroxysmal atrioventricular block as a cause of syncope].

We present the case of a 66 years old man with recurrent episodes of syncope, up to three times during the last two months without previous symptoms. An ECG after the syncope showed a bilateral block (left anterior fascicle block and right bundle branch block) and first grade atrioventricular block. The exercise test did not demonstrate either AV conduction disorders or tachyarrhythmia episodes. Holter monitoring showed premature ventricular complexes; tilt testing and carotid sinus massage were normal. The electrophysiologic study revealed no alteration in the conduction system. Throughout at

Excessive supraventricular ectopic activity and future onset of atrial fibrillation in patients with cryptogenic stroke.

OBJECTIVES: Although evidence has shown the association of excessive supraventricular ectopic activity (ESVEA) with future development of atrial fibrillation (AF), this relationship is not yet fully understood. This study examines whether ESVEA can predict the future onset of AF, in patients presenting with cryptogenic stroke. MATERIALS AND METHODS: A retrospective cohort of 124 non-AF, consecutive patients, hospitalized for cryptogenic stroke between 2014 and 2015, was retrieved. 24-h inpatient monitoring with Holter was employed to reveal ESVEA, defined as the presence of more than 20 prema

Tachycardia-induced cardiomyopathy in a cat.

A 10-year-old male castrated Domestic Shorthair cat was evaluated for an asymptomatic tachyarrhythmia noted two weeks prior. Electrocardiography revealed a normal sinus rhythm with atrial premature complexes and paroxysms of supraventricular tachycardia with a heart rate between 300 and 400 min-1. Echocardiography was unremarkable, and concentrations of circulating cardiac troponin I, T4, and blood taurine were within reference ranges. The cat was treated with sotalol (2.1 mg/kg q12h, PO) but the arrhythmia was insufficiently controlled as determined during several re-examinations within a two

Frequency, characteristics and significance of supraventricular tachyarrhythmias detected by 24-hour electrocardiographic recording in the late hospital phase of acute myocardial infarction.

The incidence, characteristics and clinical significance of supraventricular tachyarrhythmias occurring in the late hospital phase of acute myocardial infarction (AMI) were assessed in 209 consecutive patients. Arrhythmias were quantified by 24-hour electrocardiographic recording 16 +/- 3 days after AMI, and were classified according to the degree of complexity in 5 classes. Class 0 = less than 5 premature beats/hr; class 1 = between 5 and 100/hr; class 2 = greater than 100/hr or repetitive premature beats; class 3 = atrial-junctional tachycardia; class 4 = atrial flutter-fibrillation. Single-beat global atrial mapping facilitates the treatment of short-lived atrial tachycardias and infrequent premature atrial contractions.

BACKGROUND: Short runs of atrial tachycardias (ATs) and infrequent premature atrial contractions (PACs) are difficult to map and ablate using sequential electrophysiology mapping techniques. The AcQMap mapping system allows for highly accurate mapping of a single atrial activation. OBJECTIVES: We aimed to test the value of a novel dipole charge density-based high-resolution mapping technique (AcQMap) in the treatment of brief episodes of ATs and PACs. METHODS: Data of all patients undergoing catheter ablation (CA) using the AcQMap mapping system were reviewed. RESULTS: Thirty-one out of 219

Importance of superior vena cava isolation in successful ablation of persistent atrial fibrillation in patient with partial anomalous pulmonary vein.

Ectopic foci arising from pulmonary veins (PVs) are the predominant sources for the initiation and maintenance of atrial fibrillation (AF) in a vast majority of cases. However, ectopic foci also exist in the non-PV areas like superior vena cava (SVC) in 10-20% of the cases. We report the significance of SVC isolation in a patient with persistent AF and anomalous pulmonary venous connection of the right superior pulmonary vein into the SVC.

Initiating mechanisms of paroxysmal atrial fibrillation.

BACKGROUND: The understanding of the onset mechanisms of paroxysmal atrial fibrillation (AF) may help to develop preventive therapy. Specific heart rate (HR) patterns and autonomic changes immediately before the onset of paroxysmal AF are not fully investigated. We undertook the present study to assess HR and heart rate variability (HRV) changes before the onset of AF using 24-h Holter electrocardiographic analysis in patients without antiarrhythmic medication. METHODS AND RESULTS: In 27 patients, 48 episodes of AF, lasting more than 30s and preceded by sinus rhythm for more than 1h were anal

Subcostal M-mode echocardiography of the right atrial wall in evaluation of cardiac arrhythmias and pacing.

Subcostal M-mode echocardiography of the right atrial wall in combination with the simultaneously recorded nondiagnostic electrocardiogram is useful in the recognition of various rhythm disturbances. Each arrhythmia has a characteristic right atrial wall motion pattern. Flat and hidden P-waves can be accurately detected throughout the cardiac cycle and supraventricular tachycardias with aberration can be distinguished from ventricular tachycardia with AV dissociation. This echocardiographic technique is of special value in the follow-up of physiological pacemakers because atrial capture and se
