Navigating auditory assessment in autistic individuals: a narrative review.

Abnormal sensory responses have been a diagnostic criterion for autism since 2013, with auditory hypersensitivities representing one of the most frequently reported symptoms. It is not unusual for children with autism, some of whom are as yet undiagnosed, to present clinically to specialists with a history of auditory symptoms in the context of normal hearing. This narrative review paper provides a theoretical background and model to account for autism-associated characteristics that arise within the auditory nervous system. The review highlights the need to assess these symptoms by a multidis

Intestinal Stomas-Postoperative Stoma Care and Peristomal Skin Complications.

Medical interventions to combat serious infection or malignancies carry significant morbidities, including ototoxicity. While these lifesaving drugs are often necessary to preserve life, the impact on quality of life for survivors is increasingly concerning for families and healthcare providers. Of primary importance for medical prescribers are appropriately sensitive ototoxicity grading scales and audiological monitoring protocols for surveillance for hearing loss. The intent of grading scales is to help communicate complicated audiological information to non-audiologist healthcare providers

Epidemiology of chronic suppurative otitis media in Saudi children.

An epidemiological survey was carried out in Riyadh to establish the prevalence of chronic suppurative otitis media in Saudi children. 6421 children aged 2 months to 12 years were examined clinically, their ears were inspected by otoscopic examination by an ENT specialist, and the evaluation of hearing was undertaken by an audiologist using a memory screening audiometer (Madson 84-11). Ninety-four children (1.5%) had chronic ear disease being of the following types: congenital cholesteatoma (n = 1), acquired cholesteatoma and posterosuperior retraction (n = 5), and central perforation (n = 88)

Hearing conservation programs in Massachusetts' vocational/technical schools.

A survey of vocational/technical schools in Massachusetts was performed to evaluate the prevalence of and need for hearing conservation programs. The surveys were sent to the superintendents or principals of all 27 regional schools in Massachusetts. Surveys consisted of questions in the areas of shop noise exposure, use of safety equipment, and hearing conservation education. Results indicate that (1) many teachers and students may be exposed to hazardous noise levels (2) hearing protection is not required in one-third of the schools and is used less frequently than other safety equipment, and

Aminoglycoside ototoxicity.

Aminoglycoside antibiotics are frequently employed in the treatment of serious infections caused by aerobic gram negative bacilli. The use of these potent antibacterial agents is limited by the risks of ototoxicity and nephrotoxicity. Aminoglycosides are excreted by glomerular filtration at a rate proportional to the serum concentration. Impaired renal excretion reduces the rate of clearance from the serum. Utilizing information about aminoglycoside pharmacokinetics, the susceptibility of infecting pathogens to aminoglycosides, and risk factors for ototoxicity, physicians can attempt to optimi

Hearing loss. The physician's responsibility.

Hearing loss is a very important and common problem in the United States, affecting 13 to 14 million people. It is primarily a medical problem, requiring diagnosis and, when indicated, treatment by a physician, preferably in collaboration with an audiologist. History taking is the most important part of the diagnostic process, followed by a physical examination with special attention to the head, neck, and ears. The primary object of the workup is to exclude all possible extrinsic causes of hearing loss. Intrinsic or genetic deafness is a diagnosis of exclusion. After the hearing loss has been

So Now My Patient Has Squamous Cell Cancer: Diagnosis, Staging, and Treatment of Squamous Cell Carcinoma of the Anal Canal and Anal Margin.

Difficulty hearing in the presence of background noise is a common complaint heard by audiologists. This can be accompanied by additional difficulty in classrooms and other difficult listening situations. This was recently exacerbated by the coronavirus disease 2019 (COVID-19) shutdowns and the accompanying mask usage and virtual meetings. This article describes one such patient seen during the recent shutdown. This patient reported difficulty hearing during her college coursework for her music performance major. She was an established otology patient with an extensive middle ear history inclu

Do tissue spears used to clear ear canal pus improve hearing? A case series study of hearing in remote Australian Aboriginal children with chronic suppurative otitis media before and after dry mopping with tissue spears.

OBJECTIVE: To determine whether the use of tissue spears to remove otorrhoea from Aboriginal children's ear canals improves hearing in the affected ear. DESIGN: Case series study with controls. METHODS: The study comprised 61 Aboriginal children from communities in the remote arid zone of South Australia who had otorrhoea obscuring the tympanic membrane in 1 or both ears. Eighty ears were treated with tissue spears, and hearing was assessed before and after treatment. RESULTS: Forty-two children had unilateral and 19 had bilateral active disease. An additional 13 ears without otorrhoea serv

Children with Unilateral Hearing Loss.

In the United States alone, unilateral hearing loss (UHL) affects 0.1 to 5% of school-aged children and 0.04 to 3.4% of infants at birth. Several studies from the 1980s and 1990s raised the specter that UHL in children was not as benign as common wisdom suggested. In children 6 to 12 years old, UHL is associated with significantly poorer oral language skills and vocabulary intelligence quotient scores. Children with UHL are at higher risk of receiving individualized educational plans in school and speech therapy. Quality of life in children also may be negatively affected by UHL, similar to ch

Audiological issues in children with cleft lip and palate in one area of the U.K.

OBJECTIVE: The purpose of this study was to determine the extent of multidisciplinary care and audiological services rendered to children with CL/P who underwent surgical repair of the cleft in 1998 and 1999. This followed proposals to radically reorganize cleft lip and palate services in the U.K. after unsatisfactory findings in a national review. METHOD: Information was obtained from questionnaires sent to clinicians (audiologists; ear, nose, and throat surgeons; and community pediatricians) from audiology teams, and parents were asked to retrieve information from their Personal Child Healt

Multi-dimensional evaluation of user-operated audible contrast sensitivity tests towards efficient hearing healthcare services.

The user-operated audiometry (UAud) project aims at introducing an automated system for user-operated audiometric testing into everyday clinical practice. Here, we focus on the Audible Contrast Threshold (ACT) test, which has been proposed as a language-independent alternative to aided speech-in-noise tests. Five distinct user-operated ACT (U-ACT) test candidates were evaluated in terms of performance, reliability, and usability against two established test benchmarks. The study involved 28 participants with diverse hearing and cognitive abilities. The results demonstrated that the primary fac

[Expert consensus on auditory intervention and language rehabilitation of CHARGE syndrome].

Objective:CHARGE syndrome(CS) is an autosomal dominant congenital malformation involving multiple organs and sensory systems. The incidence of hearing impairment and developmental retardation in CS patients is 87% and 84%, respectively, which is often accompanied by malformations such as abnormal development of temporal bone, venous sinus, and facial nerve that directly affect cochlear implant surgery. Other systemic malformations increase perioperative risks, such as heart defects, micrognathia, laryngeal and trachea malformations. Cochlear implants are significant in promoting hearing and la

Audio-Vestibular Evaluation of Pediatric Pseudo-Conductive Hearing Loss: Third Window Syndromes.

Conductive hearing loss caused by external or middle ear problems prevents the transmission of sound waves from the external auditory canal to the cochlea, and it is a common condition, especially in pediatric patients aged 1-5 years. The most common etiological factors are otitis media and cerumen during childhood. In some patients, external and middle ear functions and structures may be normal bilaterally despite the air-bone gap on the audiogram. This condition, which is often a missed diagnosis in children, is defined as a pseudo-conductive hearing loss (PCHL) caused by third window syndro

Knowledge and Attitudes of Pacific Islander Doctors and Medical Students to Childhood Hearing Loss and Hearing Services: Results of a Structured Questionnaire Survey in SAMOA.

The Pacific Islands have among the highest global rates of childhood hearing loss in the world. Given the scarcity of ENT Specialists and audiologists in the region, the role of the wider health professional community in addressing preventable childhood hearing loss is crucial. A 10-item 5-point Likert scale questionnaire was administered to medical officers and medical students in Samoa. The statements aimed to investigate knowledge and attitudes to childhood hearing loss and hearing services. Overall, 95.8% of participants selected "Disagree/Strongly Disagree" in response to the statement "H

Training for hearing care providers.

The lack of an appropriately trained global hearing-care workforce is recognized as a barrier to developing and implementing services to treat ear and hearing disorders. In this article we examine some of the published literature on the current global workforce for ear and hearing care. We outline the status of both the primary-care workforce, including community health workers, and specialist services, including audiologists, ear, nose and throat specialists, speech and language therapists, and teachers of the deaf. We discuss models of training health workers in ear and hearing care, includi

Recommendations on Collecting and Storing Samples for Genetic Studies in Hearing and Tinnitus Research.

OBJECTIVES: Research on the genetic basis of tinnitus is still in its first steps. A group of scientists dedicated to tinnitus genetics within European Tinnitus Network (TINNET) network recognize that further progress requires multicenter collaborative efforts for defining contributing genes. The purpose of the present work is to provide instructions regarding collection, processing, storage, and shipment of samples intended for genetic studies in auditory research. DESIGN: One part of the recommendations has a general character; another part is of particular importance for auditory healthcar

Identification and management of children with minimal hearing loss.

Throughout the last two decades, we have witnessed a gradual change in the audiological profile of the hearing-impaired child. The number of children with severe to profound hearing losses seems to be declining, while those with minimal losses seems to be increasing. Such losses include unilateral sensorineural, mild bilateral sensorineural, and bilateral conductive hearing loss. Historically, children with minimal hearing loss have received limited attention from physicians, audiologists, or educators. It has been assumed that minimally hearing-impaired children will exhibit few, if any, hand

Heart rate response audiometry: bases, clinical techniques, and limitations.

Although a great deal of information is available concerning cardiovascular responsitivity to sensory stimuli, little is known regarding the pragmatic application of cardiac response measures to the problems of audiological assessment. Some relationships between arousal systems and cardiovascular function have begun to emerge, and the assessment of communicative behavior using HR response measures has become a more realistic goal, but the clinician who desires to utilize these techniques for audiological evaluation faces serious difficulties until further basic research has been conducted.
