To study the mechanism of synergism between Bacteroides fragilis and Escherichia coli, the effect of sublethal dose of E. coli lipopolysaccharide (LPS) (25 micrograms/mouse) was checked on B. fragilis abscess formation. LPS was administered prior or after inoculum injection. No significant difference in the abscess size was observed at necropsy on day 6. However, all the groups receiving LPS showed higher incidence of recovery of additional intestinal bacteria (23.5-45.5%) from the abscess pus. We describe a 53-year-old alcoholic man who presented with hip septic arthritis due to Bacteroides fragilis. This arthritis involved a severe destruction of the femoral head, which was completely devitalized. Recovery was achieved after 4 months of antimicrobial therapy with imipenem/cilastatin plus metronidazole, surgical debridement of the necrotic tissues and four sessions of hyperbaric oxygen.

Natural materials containing tannin were examined to determine whether they possessed any inhibitory activity against collagenase obtained from the culture supernatant of Bacteroides gingivalis. The collagenolytic activity was determined with Collagenokit CLN-100 (Collagen Technological Co., Tokyo) using FITC-conjugated collagen type 1. Among the test samples, only an aqueous extract of cacao (Theobroma cacao) bean husk strongly inhibited the bacterial collagenase. This inhibitory potency was at the same level as that of tetracycline-HCl.

The chemical structure of Bacteroides fragilis NCTC 9343 lipid A was characterized by using conventional chemical procedures, methylation analysis, and laser desorption mass spectrometry. It was found that B. fragilis lipid A consists of a beta-D-glucosaminyl-(1-6)-D-glucosaminyl-1-O-phosphate backbone whose hydroxyl groups in positions 4, 4' and 6' are free, the latter serving as the attachment site for the polysaccharide component in lipopolysaccharide. This backbone molecule carries up to of five molecules of ester- and amide-bound long chain non-hydroxylated and (R)-3-hydroxy fatty acids.

Bacteroides thetaiotaomicron bacteremia remains poorly characterized. We retrospectively analyzed 23 cases of B. thetaiotaomicron bacteremia treated at our hospital. The 30-day mortality rate was 34.8%. Infections were mainly gastrointestinal and associated with immunosuppression. Isolates were largely susceptible to β-lactam/β-lactamase inhibitors, carbapenems, and metronidazole, supporting antimicrobial therapy.
