The Effects of Manual and Automatic Lancets on Neonatal Capillary Heel Blood Sampling Pain: A Prospective Randomized Controlled Trial.

PURPOSE: The aim of this study was to determine the effects of manual lancets and automatic lancets on neonatal capillary heel blood sampling pain. DESIGN AND METHODS: This was a parallel-group, prospective, randomized controlled and observational trial. Participants were randomly assigned (1:1) to the manual lancet (odd days of the month) and automatic lancet groups (even days of the month) for capillary heel blood sampling. The sample consisted of 60 term neonates divided into two groups: Manual lancet (n = 30) and automatic lancet (n = 30). Heart rate, oxygen saturation, duration of blood

Stability of 21 Antihypertensive Drugs in Serum Collected in Standard (Nongel) Serum Tubes Versus Tubes Containing a Gel Separator.

BACKGROUND: Therapeutic drug monitoring of antihypertensive drugs is being increasingly used to optimize treatment and to assess nonadherence. Separator gels are often used in blood collection tubes to facilitate serum or plasma separation from other blood constituents before analyses. Drug adsorption into the separator gel presents a possible pre-analytical cause of falsely low concentrations or false negative results. METHODS: Drug-free blood from blood donors was spiked with therapeutic concentrations of 21 antihypertensive drugs, transferred to serum tubes with and without separator gel (

A safety lancet for neonatal blood spot tests: a design that facilitates pain-free, atraumatic samples.

Safety lancets are used to collect capillary blood samples to test if neonates have rare but serious congenital conditions, such as sickle cell disease, cystic fibrosis, congenital hypothyroidism and inherited metabolic diseases. Blood samples are taken from the heel, but the procedure can cause the neonate pain or discomfort, as well as a risk of local trauma to the nerves and blood vessels, bleeding, infection and scarring. This article explores the need for blood sampling in neonates, discusses the procedure and outlines the types of lancets available. It describes the Neoheel Safety Lancet

A Change in Posture Significantly Affects Plasma Concentrations of Large Molecules Such as IgG Antibodies.

In several studies with monoclonal antibodies, we observed spikes in plasma concentration profiles coinciding with a change of posture of study subjects. We hypothesized that these unexpected changes were due to fluid shifts from plasma to the interstitium. The objective of this study was to investigate size and time course of the change in total immunoglobulin G (IgG) concentration after a change in posture. Thirty-two healthy subjects were enrolled. After entry in the clinic, subjects remained upright for at least 1 hour, followed by IgG sampling. Thereafter, subjects remained supine or seat

Relevance of EDTA carryover during blood collection.

BACKGROUND: The order of draw is regarded as a preanalytical issue to prevent carryover of additives during blood collection. Our objective was to prove the theory of ethylenediaminetetraacetic acid (EDTA) carryover for a closed vacuum system and the influence of EDTA on concentrations of selected biomarkers. METHODS: To test the carryover of EDTA, a blood collection with tripotassium EDTA (K3EDTA) and subsequent non-additive tubes was simulated using distilled water as substitute for blood. EDTA concentrations were measured by tandem mass spectrometry. Then we added increasing concentrations

Towards evidence based emergency medicine: best BETs from the Manchester Royal Infirmary. BET 1: Brachial arterial blood sampling: safe or not?

A shortcut review was carried out to establish whether brachial artery puncture is safe in patients requiring arterial blood sampling. 47 papers were found using the reported searches, of which one presented the best evidence to answer the clinical question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of this paper are tabulated. It is concluded that brachial artery puncture is likely to be safe in the non-shocked patient who is not on concurrent anticoagulant therapy.

An optimized hollow microneedle for minimally invasive blood extraction.

The healthcare system relies widely on biochemical information obtained from blood sample extracted via hypodermic needles, despite the invasiveness and pain associated with this procedure. Therefore, an alternative micro-scale needle for minimally invasive blood sampling is highly desirable. Traditional fabrication techniques to create microneedles do not generate needles with the combined features of a sharp tip, long length, and hollow structure concurrently. Here, we report the fabrication of a microneedle long enough to reach blood vessels and sharp enough to minimize nerve contact for mi

Alternatives to retroorbital blood collection in hispid cotton rats (Sigmodon hispidus).

Cotton rats (Sigmodon hispidus) are a valuable animal model for many human viral diseases, including polio virus, measles virus, respiratory syncytial virus, and herpes simplex virus. Although cotton rats have been used in research since 1939, few publications address handling and sampling techniques for this species, and the retroorbital sinus remains the recommended blood sampling site. Here we assessed blood sampling methods that are currently used in other species and a novel subzygomatic sampling site for their use in S. hispidus. The subzygomatic approach accesses a venous sinus that pos

Liquid chromatography-mass spectrometry measurement of tacrolimus in finger-prick samples compared with venous whole blood samples.

BACKGROUND: Finger-prick sampling is an alternative strategy for monitoring immunosuppressive drug concentrations that could be useful in reducing outpatient visits. We investigated the correlation between venous and finger-prick samples in a group of adult thoracic transplant patients. METHODS: Blood samples (n = 65) for the measurement of whole blood tacrolimus were collected from adult heart (n = 18) and lung transplant (n = 20) recipients receiving tacrolimus-based immunosuppressive therapy and a finger-prick sample was taken at the same time. RESULTS: Between-batch assay imprecision (co

Tetrahydrocannabinol stability in whole blood: plastic versus glass containers.

The stability of tetrahydrocannabinol (THC) in whole blood during storage was investigated. Two types of containers were used: plastic tubes (polystyrene) and glass vials. Freshly drawn blood was spiked with 10 pmol THC/mL and stored for four days at room temperature (20 degrees to 25 degrees C). The material was kept for four weeks at -20 degrees C. Blood samples from 16 persons apprehended by the police on suspicion of cannabis smoking were collected into the two types of containers and stored frozen (-20 degrees C) until analysis. THC was quantified by capillary column gas chromatography/el

[Immunoenzymatic detection of human African trypanosomiasis using dried blood samples].

The application of the ELISA technique to dry blood samples is described with special reference to some parameters: the correspondence between the "confetti" eluate and the serum dilution, and the preservation time of the dry blood samples. A correlation is made between the results obtained with the confetti and those obtained with the sera. The study shows that the dry blood samples are a valuable tool for a large scale diagnosis of human African trypnanosomiasis during sero-epidemiological mass investigations.

Determination of reference intervals and comparison of venous blood gas parameters using a standard and nonstandard collection method in 51 dogs.

The aim of this study was to determine reference intervals (RI) for venous blood parameters determined with the RAPIDPoint 500 (RP500) blood gas analyzer using blood gas syringes (BGS) and to determine whether immediate analysis of venous blood collected into lithium heparin (LH) tubes can replace anaerobic blood sampling into BGS. The null hypothesis was that canine venous blood samples collected in BGS and in LH tubes are comparable. Jugular blood was collected from 51 healthy dogs into a BGS and a LH tube. The BGS was immediately analyzed followed by the LH tube. The RI were calculated from

[Buccal glucose measurements in dogs using safety lancets for blood sampling].

OBJECTIVE: Glucose measurements at the pinna or paws are an important part of the management of diabetic dogs. Despite extensive rubbing or warming of the testing site, an adequate blood sample cannot always be obtained. Therefore, the objective of this study was to evaluate the lip as an alternative sampling site with respect to practicability and tolerance by the dogs and reliability of the measured values. MATERIAL AND METHODS: Nine clinic-owned male-castrated healthy beagles were included in this prospective study. Glucose measurements were performed using a glucometer developed for veter

[Management directed at preventing misidentifications by introduction of the computed identification system and blood sampling by the transfusion unit; aim for good partnership between a transfusion unit and bedsides].

The initial step of blood transfusion therapy is blood type grouping. ABO-mismatch blood transfusion results in serious adverse effects. Several incidents in the process of blood sampling had been experienced in our hospital since 2006 to 2008. Therefore, we have introduced the computed identification system, and the transfusion unit has taken a part of blood sampling. Just after we introduced it in July 2010, only 7% of the doctors and the nurses used the system in blood sampling. Repeated training programs for doctors and nurses on blood sampling procedure improved the utilization to 95%. We

Canine and feline blood donor screening for infectious disease.

Thousands of blood transfusions are performed each year on dogs and cats, and the demand for blood products continues to grow. Risks associated with transfusions include the risk of disease transmission. Appropriate screening of blood donors for bloodborne infectious disease agents should be performed to lessen this risk. Geographic restrictions of disease, breed predilection, and documentation of actual disease transmission by transfusion all are factors that might need to be considered when making a decision on what screening program to use. In addition, factors involving general health care

Blood sample volumes in child health research: review of safe limits.

OBJECTIVE: To determine paediatric blood sample volume limits that are consistent with physiological "minimal risk." METHODS: A literature review was performed to search for evidence concerning the adverse effects of blood sampling in children and for guidelines on sampling volume in paediatric research. The search included Medline, EMBASE, other web-based and non-web-based sources and the bibliographies of the sources identified. Experts were also consulted. FINDINGS: Five studies and nine guidelines were identified. Existing guidelines specify paediatric blood sample volume limits ranging

Donor deferral policies for men who have sex with men: past, present and future.

We review the history and evolution of blood donor criteria for men who have sex with men (MSM). Deferral policies in many jurisdictions, including Australia, New Zealand, Canada, the United States, Brazil and many western European countries are based on a period of abstinence from MSM, often of 12 months duration. Several countries (Italy, Spain and Portugal) defer donors based on sexual behaviours considered to be at high risk, regardless of whether the partner is same sex or opposite sex. Compliance is a key determinant in the efficacy of any deferral policy. We summarize research themes an

Effects of blood collection for transfusion on arterial blood pressure, heart rate, and PCV in cats.

BACKGROUND: Collection of 50 mL of blood (standard unit) in cats is a common procedure. There are several studies on the health status of donors, but to our knowledge there are no reports on the effects of blood collection on the feline donor. HYPOTHESIS: Collection of a standard unit of blood from cats does not significantly change arterial blood pressure (BP), mean arterial pressure (MAP), systolic arterial pressure (SAP), diastolic arterial pressure (DAP), PCV, and heart rate (HR) in healthy blood donor cats. ANIMALS: Twenty-six healthy blood donor cats (6 spayed females and 20 castrated
