Mechanisms by Which Ranolazine Terminates Paroxysmal but Not Persistent Atrial Fibrillation.

BACKGROUND: Ranolazine inhibits Na+ current (INa), but whether it can convert atrial fibrillation (AF) to sinus rhythm remains unclear. We investigated antiarrhythmic mechanisms of ranolazine in sheep models of paroxysmal (PxAF) and persistent AF (PsAF). METHODS: PxAF was maintained during acute stretch (N=8), and PsAF was induced by long-term atrial tachypacing (N=9). Isolated, Langendorff-perfused sheep hearts were optically mapped. RESULTS: In PxAF ranolazine (10 μmol/L) reduced dominant frequency from 8.3±0.4 to 6.2±0.5 Hz (P<0.01) before converting to sinus rhythm, decreased singularity

Hybrid ablation under intraoperative 3D voltage mapping guidance.

Long-standing persistent atrial fibrillation remains a significant therapeutic challenge, often proving to be resistant to treatment with antiarrhythmic medications and transcatheter ablation. Hybrid ablation, which combines a minimally invasive surgical and transcatheter approach, is emerging as a promising treatment option. In this video tutorial, we demonstrate our method of hybrid ablation for long-standing persistent atrial fibrillation using advanced 3D electro-anatomical voltage mapping to guide the ablation process and thereby improve the ultimate effectiveness of the procedure.

Identification of a critical isthmus in complex macroreentrant atrial tachycardia using Ripple mapping in a patient with surgically repaired Ebstein's anomaly.

Ripple mapping is a novel, three-dimensional, electroanatomic mapping tool that displays each electrogram at its corresponding 3-dimensional coordinate as a dynamic moving bar, which changes in length according to the electrogram voltage-time relationship. We present the case of a 43-year-old male patient with surgically repaired Ebstein's anomaly who previously underwent two unsuccessful ablation procedures for right atrial flutter (cavotricuspid isthmus and intercaval lines). Ripple mapping was decisive, enabling the arrhythmia mechanism to be appropriately recognized, and a distinction to b

Absence of rotational activity detected using 2-dimensional phase mapping in the corresponding 3-dimensional phase maps in human persistent atrial fibrillation.

BACKGROUND: Current phase mapping systems for atrial fibrillation create 2-dimensional (2D) maps. This process may affect the accurate detection of rotors. We developed a 3-dimensional (3D) phase mapping technique that uses the 3D locations of basket electrodes to project phase onto patient-specific left atrial 3D surface anatomy. OBJECTIVE: We sought to determine whether rotors detected in 2D phase maps were present at the corresponding time segments and anatomical locations in 3D phase maps. METHODS: One-minute left atrial atrial fibrillation recordings were obtained in 14 patients using t

Role of endo-epicardial dissociation of electrical activity and transmural conduction in the development of persistent atrial fibrillation.

Atrial fibrillation (AF) is an arrhythmia that occurs as a result of numerous pathophysiological processes in the atria. High rate, neurohumoral activation, aging and chronic stretch activate a variety of signaling pathways leading to electrical and structural remodeling. In particular, endomysial fibrosis within the epicardial layer, which also occurs as a result of AF itself, can disrupt electrical connections between muscle bundles. This leads to electrical dissociation not only within the epicardial layer, but also between the epicardial layer and the endocardial bundle network. Although t

Epicardial macroreentrant ventricular tachycardia associated with a left ventricular aneurysm.

A 62-year-old man with severe coronary artery disease and a left ventricular aneurysm underwent catheter ablation of ventricular tachycardia (VT) with right bundle branch block QRS morphology. Endocardial bipolar voltage mapping with standard threshold settings demonstrated no low-voltage areas within the aneurysm. Catheter ablation of the epicardial surface of the aneurysm eliminated the VT. Endocardial bipolar voltage mapping with any other settings could not predict the site of the epicardial arrhythmogenic substrate whereas endocardial unipolar voltage mapping could. Endocardial unipolar v

Clinical predictors of termination and clinical outcome of catheter ablation for persistent atrial fibrillation.

OBJECTIVES: This study evaluated the role of pre-procedural clinical variables to predict procedural and clinical outcomes of catheter ablation in patients with long-lasting persistent atrial fibrillation (AF). BACKGROUND: Catheter ablation of persistent AF remains a challenging task. METHODS: Catheter ablation was performed in 90 patients (76 men, age 57 +/- 11 years) with long-lasting persistent AF. The history of AF, echocardiographic parameters, presence of structural heart disease, and surface electrocardiogram (ECG) AF cycle length (CL) were assessed before ablation and analyzed with r

Validation of coronary sinus activation pattern during left atrial appendage pacing for beat-to-beat assessment of mitral isthmus conduction/block.

INTRODUCTION: Mitral isthmus (MI) ablation for treatment of perimitral flutter is often performed during atrial fibrillation (AF) ablation but is technically challenging. Traditional assessment of MI conduction by left atrial activation mapping while pacing from either side of the line is time-consuming, and cannot be performed during ongoing ablation. Analysis of the coronary sinus (CS) activation pattern during left atrial appendage (LAA) pacing has been proposed as a simpler technique for evaluating MI conduction, enabling beat-to-beat assessment of conduction during ablation procedures and

Remote navigation guided ablation of incessant atrial tachycardia in a patient with restricted venous access.

We report a case of a 45-year-old man with incessant supraventricular tachycardia (SVT) in the setting of intrahepatic inferior vena cava thrombus and a left upper extremity arteriovenous fistula. A diagnostic electrophysiology study (EPS) was performed via the right internal jugular and right subclavian veins. After confirming the mechanism of SVT was atrial tachycardia (AT), mapping, and successful ablation was performed using remote catheter navigation. This case demonstrates that remote navigation can facilitate the therapy of complex arrhythmias in challenging clinical situations.

Electrocardiographic imaging of patients with heart failure with left bundle branch block and response to cardiac resynchronization therapy.

BACKGROUND: Cardiac resynchronization therapy (CRT) for patients with left bundle branch block (LBBB) and left ventricular (LV) dysfunction has variable responses. We hypothesized that conduction delays in intrinsic rhythm and responses to pacing may be influenced by electrophysiologic substrate. To determine this, we used electrocardiographic imaging (ECGI), which maps epicardial ventricular excitation noninvasively. METHODS AND RESULTS: Electrocardiographic imaging was conducted in 8 patients undergoing CRT. During intrinsic conduction with LBBB (6 patients), ventricular activation was hete

Effects of stepwise ablation of chronic atrial fibrillation on atrial electrical and mechanical properties.

OBJECTIVES: This study sought to evaluate the effects of stepwise catheter ablation of chronic atrial fibrillation (AF) on atrial electrical and mechanical properties. BACKGROUND: Although stepwise catheter ablation of chronic AF is associated with acute arrhythmia termination and a favorable clinical outcome, atrial tissue damage following the procedure has not been evaluated. METHODS: Forty patients who had previously undergone catheter ablation of chronic AF were studied. In the index procedure, termination of AF was achieved by catheter ablation alone in 36 of 40 patients (90%). Electroa

Nonfluoroscopic catheter navigation for radiofrequency catheter ablation of supraventricular tachycardia in children.

BACKGROUND: Radiofrequency catheter ablation (RCA) of supraventricular tachycardia (SVT) in children is highly successful but requires exposure to radiation. Nonfluoroscopic mapping systems may significantly reduce fluoroscopy time. METHODS: Forty consecutive pediatric patients who underwent RCA for accessory pathways (AP) or AV nodal reentrant tachycardia (AVNRT) with use of a nonfluoroscopic navigation system (Ensite NavX) (group A) were compared retrospectively to 40 consecutive patients with similar diagnoses who underwent RCA with fluoroscopic guidance only (group B). Common left pulmonary vein: a consistent source of arrhythmogenic atrial ectopy.

INTRODUCTION: The discovery of a consistent relationship between atrial anatomy and electrophysiology would have practical value and may provide insight into arrhythmia mechanism. We previously observed that anatomically common left pulmonary veins were a disproportionate source of atrial ectopy that was associated with the initiation of atrial fibrillation ("arrhythmogenic"). This report details our efforts to characterize and quantify this observation. METHODS AND RESULTS: One hundred subjects with paroxysmal atrial fibrillation underwent computed tomographic scanning to characterize left a

Novel pacing maneuvers for mapping the upstream insertion of unidirectional accessory pathways.

BACKGROUND: Anatomic characterization of unidirectional accessory pathways (APs) is inherently limited to the localization of the downstream insertion site. The inability to define the full anatomic course of unidirectional pathways can limit the safety and effectiveness of ablation in the setting of complex pathways, slanted pathways, unstable catheter positioning at downstream insertions, or insertions near the conduction system. OBJECTIVE: We aimed to develop novel pacing maneuvers to localize upstream insertions of unidirectional APs. METHODS: Two methods were evaluated: localizing the s

Validation of an automated premature ventricular contraction mapping algorithm.

BACKGROUND: Catheter mapping and ablation of premature ventricular contractions (PVCs) requires accurate annotation of earliest local activation time (LAT), but displacement in catheter position between sinus rhythm (SR) and the PVC complicates three-dimensional mapping localization. An automated algorithm to annotate LAT sites at the corresponding sinus rhythm sites would provide an alternative to manual annotation and improve procedural efficiency. METHODS: A retrospective single center study assessed 64 patients undergoing catheter ablation of PVCs. We divided the study patients into two g
