Long-term deterioration of bone-conduction hearing level in patients with labyrinthine fistula.

OBJECTIVE: Although many reports describe the short-term hearing outcomes of surgically managed labyrinthine fistulae, the long-term results remain unknown. We reviewed the long-term postoperative hearing outcomes of 14 ears of patients with cholesteatoma and labyrinthine fistulae. METHODS: Between 1996 and 2010, 84 patients with cholesteatoma and labyrinthine fistula underwent tympanoplasty at Hyogo College of Medicine Hospital. Fistulae were located in the lateral semicircular canal in all patients and in the superior semicircular canal in one. Fourteen patients were followed up for more th

Binaural measurement of bone conduction click evoked otoacoustic emissions in adults and infants.

Transient evoked otoacoustic emissions (TEOAEs) are usually evoked with air conduction (AC) stimuli. Only a few reports exist about OAEs where stimuli have been delivered using bone conduction (BC) by placing a bone conductor on the forehead or the mastoid. The aims of the present study were to improve the test performance of BC-TEOAEs by using a nonlinear stimulation protocol and to find out, whether this technique can be applied in newborn hearing screening. BC-TEOAEs were measured binaurally in ten normal hearing adults and in ten infants. For measurements in infants, miniaturized probes wi

[Effects of chronic suppurative otitis media of bone conduction threshold in old patients].

OBJECTIVE: To research the effects of chronic suppurative otitis media on bone conduction threshold in old patients. METHODS: The files of patients with unilateral chronic otitis media were retrospective analyzed, who were all oder than 60 years, who were inpatient in our department since January 2005 to March 2009. Conventional puretone audiometry test was carried out. Bone conduction thresholds were calculated for frequencies of 0.5, 1, 2, and 4 kHz, with comparison between the ear with chronic otitis media and contralateral ear. Thresholds were examined separately for each frequency. RESU

Myringoplasty. A conventional and extended high-frequency, air- and bone-conduction audiometric study.

Comparison of the pre- and postoperative air- and bone-conduction thresholds in 22 subjects in whom successful myringoplasty was performed has been made in the conventional and extended high-frequency ranges. Air-conduction thresholds improved through 4 kHz, but were elevated postoperatively for the frequencies 6 through 18 kHz. Postoperative bone-conduction thresholds were elevated at 0.25 and 0.5 kHz, were lower by 2-8 dB for 1 through 3 kHz and not significantly altered in the extended high-frequency range of 8 through 16 kHz. The extended high-frequency air-conduction threshold loss follow

Change of bone conduction thresholds by total footplate stapedectomy in relation to age.

INTRODUCTION: The influence of age on threshold changes of bone conduction after stapedectomy has not been thoroughly studied. Improvement of the Carhart notch by stapes surgery might be related to age as well as an increase in cochlear sensitivity to surgical trauma. PATIENTS AND METHODS: A retrospective study on the outcome of stapedectomy was undertaken. The results of surgery performed on 387 ears in 315 patients for otosclerosis between 1962 and 1989 were obtained. Stapedectomy, performed by a single surgeon, consisted of total stapedectomy, seal of the oval window with tragal perichondr

Perception Mechanism of Bone-Conducted Ultrasound and Its Clinical Use.

It is generally believed that ultrasound cannot be heard. However, ultrasound is audible when it is presented through bone conduction. Bone-conducted ultrasound (BCU) has unique characteristics; the most interesting is its perception in patients with profound deafness. Some patients can perceive it and discriminate speech-modulated BCU. Previous reports have suggested that BCU can be used for a hearing aid or tinnitus sound therapy. In this review, the perception of BCU at both the peripheral and central levels was investigated based on previous studies, although some of them remain controvers

Influence of bone-conducted vibration on simulator sickness in virtual reality.

Use of virtual reality (VR) technology is often accompanied by a series of unwanted symptoms, including nausea and headache, which are characterised as 'simulator sickness'. Sensory mismatch has been thought to lie at the heart of the problem and recent studies have shown that reducing cue mismatch in VR can have a therapeutic effect. Specifically, electrical stimulation of vestibular afferent nerves (galvanic vestibular stimulation; GVS) can reduce simulator sickness in VR. However, GVS poses a risk to certain populations and can also result in negative symptoms in normal, healthy individuals

Bone-conducted evoked myogenic potentials from the sternocleidomastoid muscle.

The aim of this study was to show that bone-conducted clicks and short tone bursts (STBs) can evoke myogenic potentials from the sternocleidomastoid muscle (SCM) and that these responses are of vestibular origin. Evoked potential responses to bone-conducted auditory stimuli were recorded from the SCMs of 20 normal volunteers and from 12 patients with well-defined lesions of the middle or inner ear or the VIIIth cranial nerve. The subjects, who had various labyrinthine and retro-labyrinthine pathologies, included five patients with bilateral profound conductive hearing loss, two with bilateral

[Prognostic value of the study of direct bone conduction in patients with otospongiosis].

The actual "early" improvement of hearing thresholds following stapedectomy were measured in 100 patients with otospongiosis by use of direct (intraoperative promontory bone stimulation) and conventional bone conduction audiometry prior to surgery. The direct bone conduction technique was usually more predictive in hearing improvement following stapedectomy than conventional bone conduction audiometry. The direct bone conduction seems to be a superior method for assessing hearing in patients with severe otospongiosis compared with conventional bone conduction which does not reflect actual coch

Performance and Reliability Evaluation of an Automated Bone-Conduction Audiometry Using Machine Learning.

To date, pure-tone audiometry remains the gold standard for clinical auditory testing. However, pure-tone audiometry is time-consuming and only provides a discrete estimate of hearing acuity. Here, we aim to address these two main drawbacks by developing a machine learning (ML)-based approach for fully automated bone-conduction (BC) audiometry tests with forehead vibrator placement. Study 1 examines the occlusion effects when the headphones are positioned on both ears during BC forehead testing. Study 2 describes the ML-based approach for BC audiometry, with automated contralateral masking rul

Improved Bone Conduction Hearing After Middle Ear Surgery: Investigation of the Improvement Mechanism.

OBJECTIVES: When performing middle ear operations, such as ossiculoplasty or stapes surgery, patients and surgeons expect an improvement in air conduction (AC) hearing, but generally not in bone conduction (BC). However, BC improvement has often been observed after surgery, and the present study investigated this phenomenon. METHODS: We reviewed the preoperative and postoperative surgical outcomes of 583 patients who underwent middle ear surgery. BC improvement was defined as a BC threshold decrease of >15 dB at two or more frequencies. Subjects in group A underwent staged ossiculoplasty afte

Planning tools and indications for "virtual surgery" for the Bonebridge bone conduction system.

BACKGROUND: Implantation of the Bonebridge (MED-EL, Innsbruck, Austria), an active semi-implantable transcutaneous bone conduction hearing system, involves the risk of impression or a lesion in intracranial structures, such as the dura or sigmoid sinus. Therefore, determining the optimal implant position requires careful preoperative radiological planning. OBJECTIVE: The aim of this study was to provide an overview of the possibilities for preoperative radiological planning for the Bonebridge implantation and to evaluate their indications and feasibility. MATERIALS AND METHODS: A systematic
