Avoiding complications in the care of fractures of the tibia.

Tibial fractures are the most common long-bone fractures. Orthopaedic surgeons, regardless of their subspecialty, often must treat these injuries, which range from low-energy, minimally displaced fractures to limb-threatening injuries with neurologic and vascular damage and significant damage to the soft-tissue envelope. Tibial shaft fractures are often prone to complications, such as apex-anterior and valgus malalignments after nailing of the fractures in the proximal one third of the tibia, infection after open fractures, and aseptic nonunions. Understanding the common complications will aid

Disease-specific clinical problems associated with the subchondral bone.

The subchondral bone is involved in a variety of diseases affecting both the articular cartilage and bone. Osteochondral defects in distinct locations and of variable sizes are the final results of different etiologies. These include traumatic osteochondral defects, osteochondritis dissecans, osteonecrosis, and osteoarthritis. Traumatic osteochondral defects are caused by osteochondral fractures, separating an osteochondral fragment that includes articular cartilage and both subchondral and trabecular bone from the joint surface. In osteochondritis dissecans, the disease originates in the subc

Intramedullary Nailing for Segmental Tibia Fractures: A Comprehensive Retrospective - Prospective Outcome Analysis in 30 Patients at a Tertiary Care Centre.

INTRODUCTION: Segmental fractures of the tibia (OTA/AO 42-C2) account for 3-13% of all tibial shaft fractures and provide unique complications due to twin fracture lines isolating a vascularly impaired intermediate bone segment. High-energy processes usually cause substantial soft tissue disturbance, increasing the likelihood of non-union, malalignment, infection, and compartment syndrome. The objective of this study is to evaluate the functional and radiological outcome of segmental tibial fractures treated with intramedullary interlocking nailing (IMIL), to assess the rate and time of fractu

The importance of axial rotation of the lower limb : a systematic review of measurement methods.

AIMS: Assessment of lower limb alignment is a cornerstone of orthopaedics. Few studies look at rotational alignment in the axial plane as measured by femoral version (FV) and tibial torsion (TT), both of which have implications for hip, knee, and ankle pathology. This review provides an overview of the axial rotation and evaluates CT-based measurement methods for FV and TT to identify the most reliable and reproducible techniques for use in clinical practice. METHODS: A systematic PRISMA-guided review assessed original CT-based methods, examining inter- and intraobserver reliability (intracla

[Angular and torsional malalignments of the paediatric knee joint].

BACKGROUND: Abnormal angular and torsional alignment of the lower extremities during growth are a common reason for consultation of a paediatrician or orthopaedist. There is often uncertainty about the pathological value of these abnormalities and the need for treatment, even among specialists. In the course of physiological growth and development processes, the axial and torsional alignment of the legs sometimes change considerably and in characteristic patterns. DIAGNOSTICS: The ability to assess whether abnormal angular or torsional alignments of the lower limb in the course of growth shou

The Windswept Foot: Dealing with Metatarsus Adductus and Toe Valgus.

The windswept foot remains a reconstructive challenge. The hallux valgus associated with the medially displaced lesser metatarsal heads is hard to correct. Either the lesser metatarsal heads need to be displaced laterally or the deformity accepted. With the deformity, all the toes tend to be aligned into valgus with the position of the flexor and extensor tendons. Several treatment alternatives exist and may require a combination of open and percutaneous surgery. The authors think that, in severe metatarsus adductus, proximal correction of the first, second, and third metatarsals is required.

Changes in spino-pelvic alignment after surgical treatment of high-grade isthmic spondylolisthesis by a posterior approach: a report of 41 cases.

PURPOSE: To analyze changes in spino-pelvic parameters (SPPs) after surgery of high-grade lumbar isthmic spondylolisthesis (HDIS). METHODS: We analyzed 41 patients affected by HDIS operated upon by attempt of reduction and posterior spinal fusion with pedicle screw systems with or without interbody fusion. Pelvic tilt (PT), lumbar lordosis (LL), pelvic incidence (PI), and sacral slope (SS) were measured, and patients were further divided into balanced and unbalanced pelvis subgroups. RESULTS: SS passed from 46.8° ± 9.8° preoperatively to 50.1° ± 10.1° (p = 0.02). PT passed from 26.7° ± 6.7°

Accuracy of acetabular component position in hip arthroplasty.

BACKGROUND: Acetabular component malposition is linked to higher bearing surface wear and component instability. Outcomes following total hip arthroplasty and surface replacement arthroplasty depend on multiple surgeon and patient-dependent factors. The purpose of this study was to examine the frequency in which acetabular components are placed within a predetermined target range. METHODS: We evaluated postoperative anteroposterior pelvic radiographs for every consecutive primary total hip arthroplasty and surface replacement arthroplasty completed from 2004 to 2009 at a single institution. Neutral mechanical alignment: a requirement for successful TKA: opposes.

There is increasing evidence that for a number of patients, neutral alignment is not normal. Patients with so-called constitutional varus have been in varus alignment since the end of their growth. Restoring neutral alignment in these cases may not be the best option for these patients since it is abnormal to them. We recently performed a number of observational studies to find out how patients develop constitutional varus, how these patients can be recognized, and which factors contribute. A cohort of 800 young patients was analyzed to determine the influence of activity level on growth and t

Recurrent dislocation of the patella accompanying hypotrochlea of the femur and malalignment of the patella.

This case report describes a rare case of recurrent dislocation of the patella which was accompanied with trochlear dysplasia and malalignment of the patella in a 15-year-old girl. She complained of hemoarthrosis and recurrent patellar dislocation in the early knee flexion phase. Plain radiography and computed tomography (CT) showed patellar malalignment (quadriceps angle 20°) and severe dysplasia of the trochlea of the femur (sulcus angle 170°). Surgery was performed, consisting of trochleoplasty in addition to proximal and distal realignment. Trochleoplasty was undertaken using a modified De

Bilateral pes valgus in an Anatolian Sheepdog.

A 15-month-old, male, Anatolian Sheepdog weighing 45 kg, was admitted with uneven stance in both hindlegs, a condition it had had since the age of six months. Radiographs of both hindlimbs revealed lateral deviation of 26 degrees in the distal metaphysis of the left tibia and 32 degrees in the distal metaphysis of the right tibia. Corrective osteotomy was used to treat the deformity. Both tibias were aligned with the closed wedge osteotomy and a suitably curved plate was placed on both bones. In the assessment prior to the removal of both plates, it was determined that a valgus deformation of

Varus foot alignment and hip conditions in older adults.

OBJECTIVE: Mechanical strain on the hip can result from varus malalignment of the foot. This study was undertaken to explore the cross-sectional relationship between varus foot alignment and hip conditions in a population of older adults. METHODS: The Framingham Osteoarthritis Study cohort consists of a population-based sample of older adults. Within this sample, we measured forefoot and rearfoot frontal plane alignment using photographs of a non-weight-bearing view of both feet of 385 men and women (mean age 63.1 years). Each foot segment was categorized according to the distribution of fore

Does axial limb rotation affect the alignment measurements in deformed limbs?

The long-term outcome of total knee arthroplasty and femoral or tibial osteotomy is related to the ability of the surgeon to achieve the desired alignment based on preoperative planning. This study evaluates the effect of axial rotation on measured tibiofemoral angles and the angle formed between the anatomic and mechanical axes of the femur in lower extremities with valgus and varus deformities. A comparison study of the measured tibiofemoral angles indicated a statistically significant effect in models with severe vagus or varus deformity when rotated 10 degrees internally or externally. High Preoperative T1 Slope Is a Marker for Global Sagittal Malalignment.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To develop parameter thresholds obtainable from cervical radiographs that correlate with concomitant thoracolumbar malalignment. SUMMARY OF BACKGROUND DATA: T1 slope (T1S) is typically discussed in the context of cervical deformity and correlated with health-related quality of life outcomes. Prior research suggests that T1S is related to global alignment; however, a definition for "high" T1S has not been established. Most patients undergoing cervical surgery do not undergo full-spine imaging; therefore, obtaining a parameter associated wit

Editorial Commentary: Absent Extreme Bony Malalignment: Medial Patellofemoral Reconstruction Is a Sufficient Primary Surgery for Adolescents With Recurrent Patella Instability.

When planning an adolescent's primary surgery for recurrent patella instability, the surgeon must weigh the failure risk of performing medial patellofemoral reconstruction (MPFLR) versus the potential morbidity of adding a concomitant bony procedure, such as tibial tubercle osteotomy. In this age group, isolated MPFLR is an excellent operation with high clinical success, low failure rates, and favorable complication profile. Patients with high grade J-sign and severe trochlea dysplasia, apprehension into deep flexion, and extremes of bony malalignment, including elevated tibial tubercle-trochl

The Natural History of Lower Extremity Malalignment.

BACKGROUND: Lower extremity malalignment is a common problem presented to pediatric orthopaedists. Risk for early arthritis is often a concern among parents seeking advice and treatment. We seek to review previous research with regard to the natural history of malalignment. METHODS: A search of available literature on PubMed was constructed to capture articles covering the natural history of malalignment, secondary to childhood fracture as well as congenital and acquired pediatric deformity. In order to remain strictly relevant to pediatrics, articles reviewing deformities acquired in adultho

Bone resection for mechanically aligned total knee arthroplasty creates frequent gap modifications and imbalances.

PURPOSE: The objective of this study was to calculate bone resection thicknesses and resulting gap sizes, simulating a measured resection mechanical alignment (MA) technique for total knee arthroplasty (TKA). METHODS: MA bone resections were simulated on 1000 consecutive lower limb CT scans from patients undergoing TKA. Femoral rotation was aligned with either the surgical trans-epicondylar axis (TEA) or with 3° of external rotation to the posterior condyles (PC). Imbalances in the extension space, flexion space, medial compartment and lateral compartment were calculated. RESULTS: Extension

[The relevant axial deviation: Diagnostics and therapy for correction osteotomies of the femur].

Intraoperative assessment of the femoral axis, length and torsion can be difficult. A postoperative torsional discrepancy is a common but rarely diagnosed condition. A clinical evaluation of femoral torsion is limited, especially in the early postoperative period. Conventional radiographs are a reliable diagnostic tool for evaluation of discrepancies of leg length and axis. Computed tomography remains the gold standard for assessment of torsional discrepancies. Because of the wide variability of the physiological femoral torsion, the clinical impact of a torsional discrepancy in individual cas
