Biology of sinus floor augmentation with an autograft versus a bone graft substitute in a preclinical in vivo experimental model.

OBJECTIVES: Compared to autografts, bone graft substitutes are slower to consolidate. If we understood why, this might open strategies to accelerate new bone formation and thus shorten the time to implant placement. In this study, we aimed at comparing autologous bone graft with a bovine bone graft substitute in a preclinical sinus lift model. MATERIALS AND METHODS: The mouse posterior paranasal sinus served as a recipient site for grafting. Autograft from the oral cavity was compared against bone graft substitute using molecular, cellular, and histological analyses conducted on post-grafting

In Vitro Evaluation of Gentamicin or Vancomycin Containing Bone Graft Substitute in the Prevention of Orthopedic Implant-Related Infections.

Antibiotic-loaded bone graft substitutes are attractive clinical options and have been used for years either for prophylaxis or therapy for periprosthetic and fracture-related infections. Calcium sulfate and hydroxyapatite can be combined in an injectable and moldable bone graft substitute that provides dead space management with local release of high concentrations of antibiotics in a one-stage approach. With the aim to test preventive strategies against bone infections, a commercial hydroxyapatite/calcium sulfate bone graft substitute containing either gentamicin or vancomycin was tested aga

Ideal xenograft or a perfect bone substitute?-A retrospective review and analysis of the historical concept of ivory implants in orthopaedics.

PURPOSE: Surgical treatment of fractures has evolved with the development of anaesthesia in 1846. Experiments with different implants both organic and non-organic had led to introduction of sometimes extremely peculiar materials coming from different species like ox bone or elephant's ivory. The aim of this article is to present not widely known concept of ivory use in bone surgery that set its foot in the history of orthopaedics and laid foundations for orthobiologic reconstructions. METHODS: Retrospective analysis of articles and books published between 1846 and 2017 that describe various e

Combining local antibiotic delivery with heparinized nanohydroxyapatite/collagen bone substitute: A novel strategy for osteomyelitis treatment.

Osteomyelitis is a major challenge in bone surgery and conventional treatment is frequently ineffective to control the infection, with an alternative approach being required. In the present work, a heparinized nanohydroxyapatite/collagen biocomposite was produced in granular form, and loaded with vancomycin, to work as a local drug delivery system for osteomyelitis and as a bone substitute. This strategy involves the local delivery of high concentrations of vancomycin, to eradicate the infection. Additionally, these granules work as a scaffold with regenerative properties, to induce bone regen

Successful treatment of chronic osteomyelitis in children with debridement, antibiotic-laden cement spacer and bone graft substitute.

PURPOSE: Chronic osteomyelitis and infected nonunion are relatively rare conditions in pediatric patients and are more frequently seen in developing countries. Although relatively rare, they are medically and surgically challenging. Here we report a novel surgical technique used to manage five patients with chronic osteomyelitis of long bones. METHODS: Five skeletally immature patients with chronic osteomyelitis and infected nonunion of the long bones were treated surgically between 2010 and 2014 by a combination of resection of necrotic infected bone, debridement of surrounding soft tissue,

A new concept of gentamicin loaded HAP/TCP bone substitute for prophylactic action: in vitro release validation.

Infections and their consequences are a considerable problem in orthopaedic surgery. Despite intravenous prophylactic antibiotic administration, infection rates can reach in some occasions more than 1%. Indeed, the concentration in bone tissues is very low with the majority of antibiotics. Because high local dose can be obtained, the local release of gentamicin from acrylic bone cements has been shown to be efficient in preventing infections. However, for surgical procedures other than cemented prostheses no other local antibiotic releasing device is clinically available. The purpose of this s

Evaluation of allogeneic bone graft substitute for treatment of periodontal osseous defects: 6-month clinical results.

This article is an initial clinical evaluation of the use of Puros, a specially processed allogeneic bone graft substitute, for treatment of periodontal osseous defects. Nine patients with chronic periodontitis contributed 1 osseous defect each. Clinical and surgical measurements were taken at the time of the initial grafting and 6 months later at the time of a surgical re-entry. On average, probing depth improved by 3 mm, and there was 2.5 mm bone fill. Resorption of the alveolar crest was minimal. All cases healed satisfactorily, and at the time of re-entry only 1 case required re-treatment.

Resolution of Proximal Humerus Enchondroma in a 53-Year-Old Female with Curettage and Bone Substitution: A Case Report.

INTRODUCTION: Enchondromas are benign cartilaginous tumors commonly affecting the small bones of the hand; however, proximal humeral involvement presenting with persistent pain in older adults is uncommon and may mimic low-grade chondrosarcoma. CASE REPORT: A 53-year-old female presented with right shoulder pain and proximal humeral swelling. Radiographs, magnetic resonance imaging, and computed tomography imaging demonstrated a lobulated intramedullary cartilaginous lesion with cortical thinning and endosteal scalloping. The patient underwent curettage of the lesion, biopsy, and bone graftin

Socket preservation with and without lovastatin in combination with synthetic bone graft substitute: A double-blind clinical trial.

BACKGROUND: The present study examined how bone improvement techniques, particularly the use of lovastatin with ridge preservation strategies, affect bone maintenance after tooth extraction. The goal of these strategies is to potentially decrease bone resorption and enhance the quality of alveolar bone, while also taking into account the associated risks of using high doses of statins. METHODS: Twenty healthy patients undergoing bone grafting after tooth extraction in the anterior region of the maxilla were randomly divided into groups A and B. After non-invasive extraction, group A received
