Reoperation After Primary Lumbar Discectomy with or without Implantation of a Bone-Anchored Annular Closure Device: Surgical Strategies and Clinical Outcomes.

OBJECTIVE: To determine whether presence of a bone-anchored annular closure device (ACD) impacts reoperation strategies and subsequent outcomes. METHODS: Patients with large annular defects after single-level limited lumbar discectomy were randomly allocated to receive an ACD or discectomy alone (controls) and were followed for at least 3 years. RESULTS: Among 550 patients, reoperation risk was lower with ACD (11.0% vs. 19.3%). The types of reoperations and operative time were similar in each group, and the ACD did not interfere with surgical planning or operative technique. Fusion success w

What Are the Complications, Functional Outcomes, and Health-related Quality of Life of Bone-anchored Prostheses in Transfemoral Amputees? A Comparison of Single- and Two-stage Surgery Over Time.

BACKGROUND: The insertion of an osseointegration implant providing direct skeletal attachment to an external prosthesis, creating a bone-anchored prosthesis (BAP), is an alternative for patients who have a lower limb socket-suspended prosthesis with socket-related problems. Historically, the osseointegrated implant was inserted in a two-stage procedure for safety reasons; however, the single-stage procedure is being performed and reported on as well. Because there are no studies comparing these two treatment strategies, we conducted this study to investigate complication rates, functional outc

Comparing Heterotopic Ossification in Two Cervical Disc Prostheses.

STUDY DESIGN: Retrospective analysis using data from randomized clinical trials. OBJECTIVE: To compare the occurrence of heterotopic ossification (HO) between two cervical disc prostheses. Clinical outcome and range of motion (ROM) were also evaluated. SUMMARY OF BACKGROUND DATA: Cervical arthroplasty was reported to be able to maintain the segmental ROM. However, controversy exists since the difference of the occurrence of HO concerning cervical prosthesis is still huge. METHODS: Patients who underwent anterior cervical discectomy with arthroplasty for a cervical radiculopathy due to a her

Extraction of a Fractured, Well-Fixed Femoral Stem Using a Lateral Cortical Window Technique in Revision Hip Arthroplasty: A Case Report.

INTRODUCTION: Fractures of hip prostheses have been well established in medical literature. Various biomechanical studies have shown that the highest concentration of stress occurs over the lateral aspect of the middle third of the femoral stem. Complications at this focal point include stem subsidence, cortical breach, or perforation. The purpose of this report is to describe a modified technique using a lateral cortical window for the extraction of a fractured femoral stem of a hemiarthroplasty. CASE REPORT: We present a case of a 61-year-old female who complained of pain in the right thigh

Changes in lower extremity joint moments one-year following osseointegration in individuals with Transfemoral lower-limb amputation: A case series.

BACKGROUND: Dissatisfaction with socket prostheses has led to the development of bone-anchored prostheses through osseointegration for people with transfemoral amputation, eliminating the need for a prosthetic socket. Gait deviations of transfemoral prosthesis users may be linked to increased risk of osteoarthritis, and it remains unknown if gait biomechanics change following osseointegration. The purpose of this case series was to evaluate the longitudinal changes in joint kinetics one year post-osseointegration in patients with transfemoral amputation during walking. METHODS: Knee, hip, and

Long-term tooth loss in periodontally compromised but treated patients according to the type of prosthodontic treatment. A retrospective study.

If prosthodontic treatment is considered after periodontal therapy, the questions arise i) does prosthodontic treatment affect the treatment outcome of the dentition in general and ii) which type of prosthesis is related to best treatment outcome of abutment teeth? Our goal was to compare long-term tooth loss after comprehensive periodontal therapy in patients with or without prosthodontic treatment. Ninety patients' charts with a total of 1937 teeth who had received comprehensive periodontal treatment 5-17 years ago by the same periodontist were retrospectively evaluated. Sixty-five patients

Femoral fracture risk in transfemoral amputees with bone-anchored prosthetic limbs during activities of daily living.

BACKGROUND: Bone-anchored prostheses (BAPs) address many of the shortcomings of socket prostheses, driving the adoption of the technology for lower limb amputees. However, higher femoral fracture rates have been observed in these patients compared to socket users. It is believed that this increased fracture rate is due to the direct transmission of loads to bone through the bone-anchored implant. While low-impact activities, such as walking, have been extensively studied using finite element (FE) models, other more demanding low-impact activities such as ambulating on stairs and inclines remai

The Role of Concurrent Thigh Lifts in Obese Patients With Transfemoral Osseointegrated Prostheses.

BACKGROUND: Osseointegrated (OI) prostheses are a novel alternative solution for patients with lower-limb amputations. Plastic surgeons are becoming increasingly involved in providing concurrent thigh lifts (CTLs) to reduce soft tissue redundancy around the implant. Because of limited mobility, many patients present with higher body mass indexes (BMIs) and may benefit from CTL. This study aimed to evaluate outcomes in obese patients with OI prostheses who underwent CTL compared to those who did not. Additionally, we aimed to compare outcomes in patients with normal BMIs compared to obese patie

Single-Stage Press-Fit Femoral Osseointegrated Limb Replacement: A Prospective Cohort Study.

BACKGROUND: Press-fit femoral osseointegrated limb replacement (FOLR) allows for a direct transcutaneous skeletal connection between an artificial leg and the residual femur in a single-stage procedure that can be performed open or percutaneously. A skeletally anchored prosthesis can offer enhanced mobility, balance, and proprioception to amputees, as well as eliminate problems associated with socket mounted prostheses, such as skin problems, ulcers, and pain. The purpose of this research is to describe the safety, functional, and patient-reported outcomes for this technique. METHODS: We retr

Simple 3,4-Dihydroxy-L-Phenylalanine Surface Modification Enhances Titanium Implant Osseointegration in Ovariectomized Rats.

Osteoporosis presents a challenge to the long-term success of osseointegration of endosseous implants. The bio-inspired 3,4-dihydroxy-L-phenylalanine (Dopa) coating is widely used as a basic layer to bind osteogenetic molecules that may improve osseointegration. To date, little attention has focused on application of Dopa alone or binding inhibitors of bone resorption in osteoporosis. Local use of a bisphosphonate such as zoledronic acid (ZA), an inhibitor of osteoclast-mediated bone resorption, has been proven to improve implant osseointegration. In this study, ovariectomized rats were divide
