Midterm Results of Renal Transplantation From Controlled Cardiac Death Donors Are Similar to Those From Brain Death Donors.

BACKGROUND: The systematic use of grafts from controlled donors after cardiac death (cDCD) started in our country in 2012 and expanded with the strategic support of National Transplant Organization. We present our experience in kidney transplantation with organs from cDCD donors with a mean follow-up of 3 years. METHODS: Observational prospective study of all transplants performed in our center in 2012-2013 followed to 2016. The immunosuppression protocols were triple therapy for low-risk recipients from a standard brain death donor (DBD), adding basiliximab or thymoglobulin induction for ext

Brain death, states of impaired consciousness, and physician-assisted death for end-of-life organ donation and transplantation.

In 1968, the Harvard criteria equated irreversible coma and apnea (i.e., brain death) with human death and later, the Uniform Determination of Death Act was enacted permitting organ procurement from heart-beating donors. Since then, clinical studies have defined a spectrum of states of impaired consciousness in human beings: coma, akinetic mutism (locked-in syndrome), minimally conscious state, vegetative state and brain death. In this article, we argue against the validity of the Harvard criteria for equating brain death with human death. (1) Brain death does not disrupt somatic integrative u

[Legal Determination of Brain Death in Adults: Apnea Test and Electroencephalogram].

The 2020 Brain Death/Death by Neurologic Criteria is the global consensus for brain death, a state of complete and permanent loss of brain function defined by deep coma, loss of brainstem reflexes, and loss of spontaneous breathing. Risk factors for apnea include hypotension at baseline, A-aDO2 opacification, hypoxemia, acidemia, and age. The test must be performed using positive end-expiratory pressure . In Japan, EEG is mandatory even under conditions that could be considered brain death. A single apnea test and electroencephalogram are reasonable methods for the diagnosis of brain death.

Neuroprognostication after Cardiac Arrest: Who Recovers? Who Progresses to Brain Death?

Approximately 15% of deaths in developed nations are due to sudden cardiac arrest, making it the most common cause of death worldwide. Though high-quality cardiopulmonary resuscitation has improved overall survival rates, the majority of survivors remain comatose after return of spontaneous circulation secondary to hypoxic ischemic injury. Since the advent of targeted temperature management, neurologic recovery has improved substantially, but the majority of patients are left with neurologic deficits ranging from minor cognitive impairment to persistent coma. Of those who survive cardiac arres

Chemical hybridization of sulfasalazine and dihydroartemisinin promotes brain tumor cell death.

Gliomas are primary brain tumors with still poor prognosis for the patients despite a combination of cytoreduction via surgery followed by a radio-chemotherapy. One strategy to find effective treatment is to combine two different compounds in one hybrid molecule via linker to add to or at best potentiate their impact on malignant cells. Here, we report on the effects of a newly synthesized hybrid of sulfasalazine (SAS) and dihydroartemisinin (DHA), called AC254. In previous studies, both SAS and DHA have already proved to have anti-tumor properties themselves and to have sensitizing respective

Current status of organ donation after brain death in the UK.

Organ donation after brain death remains the deceased organ donation pathway of choice. In the UK, the current identification and referral rate for potential donation after brain death donors is 99%, the testing rate for determining death using neurological criteria is 86% and the approach to families for donation is 91%. Increasing donation after brain death donation will primarily require a large increase in the current consent rate of 72% to one matching the consent rate of 80-90% achieved in other European countries. Implementing the use of evidence-based donor optimisation bundles may inc

Brain death interval and relationship to outcomes of pediatric cardiac transplantation.

BACKGROUND: We sought to analyze brain death interval and outcomes of pediatric cardiac transplantation using national registry data. METHODS: We retrospectively evaluated a pediatric cohort from the UNOS registry from 2005 to 2014. We restricted the donor cohort to those with a primary central nervous system event as the cause of hospitalization. Brain death interval (BDI) was defined as the time between hospital admission and organ procurement. Primary outcomes were recipient and graft survival time. Logistical regression modeling was used for multivariable analysis. RESULTS: The donor coh

[New Challenges: Diagnosis of Brain Death in Newborn, Children and Adolescents].

BACKGROUND: According to the current update of the German guideline on brain death (BD), participation of paediatricians is now mandatory for the examination of BD in patients younger than 14 years. The present analysis focuses on the previous practice and highlights the challenges that arise from the current update. METHODS: Retrospective evaluation of the patient registry of the German organ procurement organisation (north-eastern bureau) between January, 2001 and December, 2010 with specified paediatric age groups according to the 4th update of the German guideline on BD from the 1st of Ju

Pregnancy and Brain Death: Lack of Guidance in U.S. Hospital Policies.

Objective The death of Marlise Muñoz, a pregnant woman who suffered an anoxic brain injury in November 2013, highlights the social, ethical, legal, and medical controversies associated with brain death in pregnancy. We sought to evaluate whether institutions in the United States have policies in place for situations in which a pregnant woman is declared brain dead. Study Design Institutional brain-death protocols from hospitals in the United States were obtained in cooperation with local and regional organ procurement agencies. Each protocol was reviewed to determine if and how it addressed br

Oligodendrocyte birth and death following traumatic brain injury in adult mice.

Oligodendrocytes are responsible for producing and maintaining myelin throughout the CNS. One of the pathological features observed following traumatic brain injury (TBI) is the progressive demyelination and degeneration of axons within white matter tracts. While the effect of TBI on axonal health has been well documented, there is limited information regarding the response of oligodendrocytes within these areas. The aim of this study was to characterize the response of both mature oligodendrocytes and immature proliferative oligodendrocyte lineage cells across a 3 month timecourse following T

Ischemic stroke brain sends indirect cell death signals to the heart.

BACKGROUND AND PURPOSE: Ischemic stroke is a leading cause of mortality and morbidity in the world and may be associated with cardiac myocyte vulnerability. However, it remains uncertain how an ischemic brain contributes to cardiac alternations. Here, we used experimental stroke models to reveal the pathological effects of the ischemic brain on the heart. METHODS: For the in vitro study, primary rat neuronal cells were subjected to 90-minute oxygen-glucose deprivation (OGD). Two hours after OGD, the supernatant was collected and cryopreserved until further biological assays. Primary rat cardi

Organ donation following brain stem death after ventricular assist device implantation.

The availability of donor organs is the biggest limitation for lung transplantation, and a significant proportion of patients die on the waiting list. We describe a case of a 44-year-old lady who developed subarachnoid hemorrhage and cerebral edema on second postoperative day after left ventricular assist device implantation. She was declared brain stem dead 2 days later, and her organs were transplanted to suitable recipients on the waiting list for lung, liver and kidney transplantation.

Donor brain death time and impact on outcomes in heart transplantation.

INTRODUCTION: Although the early effects of brain death on the organ donor are well described, there is a paucity of information on the impact of donor brain death on the transplanted recipient's heart in both the short and long term. The few studies published report conflicting results. The aim of this article was to undertake a retrospective exploratory study examining the impact of brain death in the donor on recipient outcomes with respect to the cardiac allograft, specifically looking at primary graft failure (PGF), postoperative rejection, and survival. METHODS: A total of 215 consecuti

Brain weight-body weight ratio in sudden infant death syndrome revisited.

AIMS: To determine whether the brain-body weight ratio is increased in sudden infant death syndrome (SIDS). METHODS: Review of autopsy files from Forensic Science SA, South Australia was undertaken over an eight-year period from 1999 to 2006, with classification of cases according to the San Diego definition. Sudden and/or unexpected deaths in previously healthy infants due to asphyxia or infection were selected as controls. RESULTS: There were 42 SIDS cases and 25 controls. The SIDS cases were aged from 1 to 42 weeks (mean: 16.26 ± 1.5 weeks) with a male to female ratio of 26:16. The contro

Dipyrone inhibits neuronal cell death and diminishes hypoxic/ischemic brain injury.

BACKGROUND: Dipyrone is an analgesic and antipyretic drug usually prescribed for patients with inflammatory conditions. We recently identified dipyrone as an antiapoptotic agent by screening a library of 1040 compounds for their ability to inhibit cytochrome c release from isolated mitochondria. OBJECTIVE: We investigated the potential neuroprotective properties of dipyrone in cerebral ischemia. METHODS: We evaluated the protective effects of dipyrone in experimental models of neuronal hypoxia/ischemia, including an oxygen/glucose deprivation model in primary cerebrocortical neurons and a fo

Guillain-Barré syndrome mimicking brain death pattern: a poorly reversible condition.

A 73-year-old man developed a fulminant form of Guillain-Barrd syndrome with abolition of brainstem reflexes. Antibodies to GQ1b were positive (1:180). The clinical findings mimicked a "brain death" pattern for a period of 12 days. In contrast, the EEG showed remaining cerebral electrical activity. Brainstem auditory evoked potentials, long-latency auditory potentials and flash-evoked visual potentials were normal. However, no peripheral and cortical somatosensory evoked potentials could be elicited. Accordingly, nerve conduction studies were indicative of motor and sensory axonal neuropathy.

Variability in pediatric brain death determination and documentation in southern California.

OBJECTIVES: Because the concept of brain death is difficult to define and to apply, we hypothesized that significant variability exists in pediatric brain death determination and documentation. METHODS: Children (0-18 years of age) for whom death was determined with neurologic criteria between January 2000 and December 2004, in southern California, were included. Medical charts were reviewed for documented performance of 14 specific elements derived from the 1987 brain death guidelines and confirmatory testing. RESULTS: A total of 51.2% of children (142 of 277 children) referred to OneLegacy

Myocardial dysfunction associated with brain death: clinical, echocardiographic, and pathologic features.

BACKGROUND: The sequelae of severe brain injury include myocardial dysfunction. We sought to describe the prevalence and characteristics of myocardial dysfunction seen in the context of brain-injury-related brain death and to compare these abnormalities with myocardial pathologic changes. METHODS: We examined the clinical course, electrocardiograms, head computed tomography scans, and echocardiographic data of 66 consecutive patients with brain death who were evaluated as heart donors. In a sub-group of patients, we compared echocardiographic findings with pathologic findings. RESULTS: Echoc
