Management of first branchial cleft anomalies: report of two cases.

Embryological anomalies of the first branchial apparatus result in rare forms of developmental abnormality of the head and neck. Their presentation may be similar to other conditions and they may easily be overlooked by the unwary when considering the differential diagnosis of a parotid swelling or a neck sinus. Consequently, they may be mismanaged on one or more occasions. We encountered two patients in whom such problems arose and conclude that appropriate investigation and management by a team skill ed in surgery of this region is necessary if a satisfactory outcome is to be achieved.

Imaging of congenital anomalies of the branchial apparatus.

One of the most challenging topics for radiologists is congenital anomalies arising from abnormal embryogenesis of the branchial apparatus. Defects of the branchial apparatus result in a spectrum of anomalies that includes fistulas, sinuses, cysts, temporal bone anomalies, craniofacial malformations, and systemic disorders. A better understanding of the various radiologic abnormalities is aided by an understanding of their embryonic origins. This article reviews the spectrum of abnormalities that are secondary to anomalous embryogenesis of the branchial arches.

Family with branchial arch anomalies, hearing loss, ear and commissural lip pits, and rib anomalies. A new autosomal recessive condition: branchio-oto-costal syndrome?

We report on a family in which 3 sibs were affected with conductive deafness, bilateral preauricular and commissural lip pits, monolateral branchial fistula, and rib anomalies. On the basis of parental consanguinity, lack of clinical variability and affected subjects of both sexes, this condition seems to be inherited as an autosomal recessive trait. We suggest that these findings comprise a new autosomal recessive entity of branchial, auricular and costal anomalies, for which we suggest the acronym BOC (branchio-oto-costal) syndrome.

Branchio-oto-renal syndrome.

Branchio-oto-renal (BOR) syndrome is a rare, autosomal dominant genetic disorder involving branchial cleft and renal anomalies, hearing loss, and other otologic manifestations. We report a case of a family with three generations of branchial cleft anomalies and otologic anomalies with hearing loss. A review of the literature, classic clinical presentations, associated findings, and the differential diagnosis of BOR syndrome is presented. Due to BOR syndrome's variability of both penetrance and expression and a high incidence of renal the practicing otolaryngologist should consider BOR syndrome

Branchial cleft anomalies--a study of 20 cases.

Preauricular sinus, branchial cyst, branchial sinus and branchial fistula are the result of incomplete obliteration of branchial clefts. Classically these lesions have particular sites and nature. The aim of this three years retrospective study was to see the prevalence of these conditions as well as the outcome of the conventional surgery. Twenty cases of branchial cleft anomalies were reported, of which 60% had with preauricular sinus. One case with preauricular sinus had recurrence during postoperative follow-up.

Thyroglossal duct cysts and branchial cleft anomalies.

Thyroglossal duct cysts and branchial cleft anomalies are important in the differential diagnosis of cervical lesions. Both lesions are common and are seen more commonly in the child than in the adult. Considerable discomfort and morbidity may be associated with severe infections occurring in both of these lesions. Of importance is the observation that once these congenital cysts become infected, subsequent surgical removal is very difficult, and recurrence may result. For these reasons it is essential that the clinician become familiar with the embryology of these lesions, which is vital to t

A lethal syndrome resembling branchio-oculo-facial syndrome.

Branchio-oculo-facial syndrome, a recently delineated autosomal dominant condition, is characterized by branchial cleft sinuses, ocular anomalies, and unusual facial appearance. A patient with branchial cleft fistulae, microphthalmia, nasomaxillary dysplasia, in addition to cardiac and CNS malformation (holoprosencephaly and meningo-encephalocele), is described. Although many features of this lethal malformation complex resemble those seen in the branchio-oculo-facial syndrome, the complex may represent a new multiple malformation syndrome.

[The first- and second-arch syndrome. A treatment analysis and plan].

There is a very great lesion staging in these syndromes, and a great number of procedures have been described, according to the age and severity of the lesions. The initial assessment is therefore important, since it allows classifying these malformations according to their severity in key sectors. Scheduling the treatment in time must take account of: growth, the necessity to perform successive operations in one region, since no operation must hinder a subsequent one. It is essential to determine the optimum time for bone reconstruction. An early treatment with a conventional bone graft, with

Branchial cleft and pouch anomalies.

We present a retrospective study of 106 patients with branchial cleft and pouch anomalies who presented to the Hospital for Sick Children between 1948 and 1990. The relevant embryology of the branchial apparatus is summarized and a theoretical description of individual anomalies given. Second branchial cleft sinuses were the most common anomalies, and the majority were managed simply, with adequate excision and a low recurrence rate. Five cases of first branchial cleft anomalies are presented, emphasizing the delay in diagnosis, the need for complete excision to prevent recurrence, and for a p

[Anomalies of the 1st branchial arch. Apropos of 3 cases].

The authors report three cases of first branchial cleft anomaly in three patients treated in O.R.L. Service of H. Thameur Hospital of Tunis. After an embryologic and a classification discussion, they expose the etiologic data and insist on the extreme rarity of these malformations (1-8% of the branchial abnormalities). To establish a positive diagnosis, some specific clinic data have to be known, so much the chirurgical management can be began. The operative difficulty is consecutive to the connection with the VII nerve

[Facial malformations and asymmetries caused by the first and second branchial arch syndrome].

The first and second branchial arch syndrome is a congenital affection, among whom one can range as well hereditary as non-hereditary entities. They always cause a badly mutilating facial asymmetry. In this article, the first and second branchial arch syndrome were divided in hemifacial and bilateral dysostoses. We pointed out the hemifacial microsomia, the most occurring entity in these series. From two case reports, we attempt to give a treatment synopsis of this syndrome, which is always done by a multi-disciplinary team, including dentists, orthodontists, maxillofacial surgeons and plastic

Branchio-oto-renal syndrome: reduced penetrance and variable expressivity in four generations of a large kindred.

The family on which this report is based is of interest because it contains individuals with the branchio-oto-renal (BOR) syndrome who have renal hypoplasia or malformations of the kidney or collecting system including duplication; only branchial and ear anomalies; and apparent nonpenetrance of the syndrome. This report provides evidence to support the hypothesis that in some families variable expressivity includes duplication of the urinary collecting system in individuals with other manifestations of the BOR syndrome as well as individuals with branchial and ear anomalies who have apparently

Cervical Thymic Cyst with parathyroid tissue - a diagnostic conundrum.

Cervical thymic cysts are relatively rare benign cystic lesions that tend to be diagnosed clinically as branchial cysts, which usually present as painless, enlarging neck masses. They can occur anywhere along the normal path of descent of thymic primordia from the angle of the mandible to the sternal notch, with mediastinal extension observed in approximately 50% of cases. They are usually seen in the first decade of life on the left side with a male predominance. Here we report a case of a 15-year-old boy who presented to the hospital with left-sided neck swelling for about 2 months. The neck

[Acute thyroiditis caused by Eikenella corrodens and abnormality of the left pyriform sinus].

A case of suppurative thyroiditis is reported. The initial course was insidious and mimicked De Quervain subacute thyroiditis. The abscess was surgically drained. It contained numerous Eikenella corrodens bacilli. Fibroscopy of pharyngo-laryngeal region showed that the left pyriform sinus was abnormal. Surgical removal of a fistula of the fourth branchial pouch was performed in order to prevent recurrence of the thyroiditis.
