EUS assisted transmural cholecystogastrostomy fistula creation as a bridge for endoscopic internal gallbladder therapy using a novel fully covered metal stent.

BACKGROUND: Laparoscopic cholecystectomy (LC) has become the "gold standard" for treating symptomatic gallstones. Innovative methods, such as a scarless therapeutic procedure through a natural orifice are being introduced, and include transgastric or transcolonic endoscopic cholecystectomy. However, before clinical implementation, instruments still need modification, and a more convenient treatment is still needed. The aim of this study was to evaluate the feasibility of endoscopic internal gallbladder therapy such as cholecystolithotomy in an animal survival model. METHODS: Four pigs underwe

Clinical potential for infected wound care: synergistic photothermal-photodynamic therapy using a conjugation-bridge modulated D-A-D porous organic polymer.

Porphyrin-based photosensitizers (PSs) often suffer from aggregation-caused quenching (ACQ) due to strong π-π stacking, which severely compromises their therapeutic activity. To address this, we leverage the inherently rigid and distorted frameworks of porous organic polymers (POPs) to physically impede such detrimental stacking, thereby preserving the photoactivity of the embedded porphyrin units. By employing a conjugation-bridge engineering strategy, we synthesized a series of porphyrin-based conjugated microporous polymers (CMPs). Among them, the optimal material, TPA-POR, is constructed f

Splenectomy revisited in Wiskott-Aldrich syndrome in the era of HSCT and gene therapy: a bridge, a rescue, or simply obsolete?

Wiskott-Aldrich syndrome (WAS) is a rare X-linked primary immunodeficiency caused by mutations in the WAS gene, classically presenting with microthrombocytopenia, eczema, and recurrent infections, with an incidence of approximately 1 per 100, 000 live births. At the mechanistic level, deficiency of the WAS protein (WASp) disrupts actin cytoskeleton remodeling across hematopoietic cells, impairing immune synapse formation, T-cell receptor signaling, natural killer cell cytotoxicity, B-cell activation, and dendritic cell migration. Concurrently, these cytoskeletal defects compromise megakaryocyt

A Successful Bridge Therapy Combining Hypomethylating Agents with Venetoclax for Adult Patients with Newly Diagnosed or Relapsed/Refractory Acute Myeloid Leukemia.

Recently, the combination of VEN-HMA has been shown to achieve durable responses in patients with both newly diagnosed (ND) and R/R-AML. We retrospectively evaluated the post-allo-HCT outcomes of 50 patients who received VEN-HMA therapy. In total, 10 were ND and 40 were R/R and, at the time of HCT, the median age was 53 years. In the ND- and R/R-AML groups, the percentage of patients who achieved CR/CRi or MLFS was 90% and 92.5%, respectively. In all, after a median follow-up of 13.7 months, the probabilities of overall survival (OS), relapse-free survival (RFS), cumulative incidence of relaps

Improved survival and decreasing incidence of adverse events with the HeartMate II left ventricular assist device as bridge-to-transplant therapy.

BACKGROUND: Pulsatile left ventricular assist devices (LVADs) are effective as bridge-to-transplant therapy, but they are limited by their large size and lack of durability. Smaller, more durable, continuous flow devices such as the HeartMate II LVAD are increasingly being used. The aim of this study is to report our single-center experience with this device as bridge-to-transplant therapy. METHODS: Overall, 47 patients received HeartMate II LVADs at our center from June 2005 to July 2007; 32 as bridge to transplant, 7 as destination therapy, and 8 as exchange therapy for a failed HeartMate X

Ag-Anchored Ti3C2Tx-Polyacrylamide Hydrogels with Interlayer Electron Bridges for Stretchable Bioelectronics and Photothermal-Assisted Therapy.

Stretchable bioelectronic hydrogels must reconcile extreme mechanical compliance with stable, high-fidelity charge transport and therapeutic functionality. We introduce a Ag-anchored Ti3C2Tx-polyacrylamide (PAM) hydrogel, in which silver nanoparticles bridge adjacent MXene layers, thereby lowering interlayer resistance and forming a continuous, strain-adaptive conductive network. Concurrent metal-polymer co-ordination with 2D MXene reinforcement yields a hierarchical dissipation architecture featuring ultra-stretchability (∼2036%), high toughness (∼2350 kJ m-3), and rapid self-recovery under c

[Refractory DLBCL to a polatuzumab-containing regimen: efficacy of CAR-T therapy in second line with radiotherapy bridging].

DLBCL represents the prototype of aggressive lymphomas; despite therapeutic advances, a substantial proportion of patients remain refractory to first-line treatment. Anti-CD19 CAR-T therapy has revolutionized the second-line setting for patients with early relapse or refractory disease; however, real-world data in patients refractory to modern polatuzumab-containing regimens remain limited. We report the case of a 73-year-old woman with stage IVB GCB-type DLBCL, refractory to Pola-R-CHP and subsequently unresponsive to R-GEMOX. She was considered eligible for second-line axicabtagene ciloleuce

Implementation Gaps and Limitations of Statin Therapy in Young Adults: Bridging Guideline Recommendations and Real-World Practice.

Atherosclerosis is a progressive inflammatory process involving the arteries of the body that is driven by cumulative exposure to low-density lipoprotein cholesterol and other lipoprotein particles. Although clinical manifestations typically occur later in life, the atherosclerotic process can begin decades earlier if lipid levels are not well controlled. Despite this, current prevention strategies do not adequately account for young adults at elevated lifetime cardiovascular risk. This review examined multiple studies evaluating statin use in young adults and found that many high-risk individ

Biomimetic Synthesis of Multinuclear Metal-Oxo Bridges Under Mild Aqueous Conditions for Catalytic Cancer Therapy.

In biological systems, enzymes achieve efficient catalysis by precisely assembling multinuclear metal-oxo bridges, such as Fe─O─Fe motifs, under physiological conditions. However, constructing such structures in synthetic systems, particularly under mild aqueous conditions, remains challenging. Here, we report a self-assembled helical polymer that creates a protein-like microenvironment and enables the biomimetic construction of Fe─O─Fe structures in a synthetic polymer system under mild, neutral aqueous conditions. This strategy increases the stability constant of iron coordination by two ord

Poly-ε-caprolactone (PCL) nanoparticles in breast cancer therapy: bridging bench research and clinical potential.

Breast cancer continues to challenge global health, with rising incidence and limited success from conventional treatments such as chemotherapy, surgery, and radiotherapy. These approaches often lack precision, leading to systemic toxicity, resistance, and relapse. Nanotechnology offers a transformative path by creating drug delivery systems that concentrate therapeutics at the tumor site while protecting healthy tissue. Among polymeric carriers, poly-ε-caprolactone (PCL) has emerged as a leading material due to its biodegradability, biocompatibility, and tuneable release profile. This review

Bridging the gut and mind: a narrative review of depression in Crohn's disease and emerging gut-brain axis therapies.

Depression is a highly prevalent and clinically significant comorbidity in Crohn's disease (CD), a form of inflammatory bowel disease (IBD), and is associated with increased disease activity, hospitalization, and a reduced quality of life (QoL). While traditionally attributed to the psychosocial burden of chronic illness, emerging evidence indicates that depressive symptoms in CD arise from dysregulation of the gut-brain axis (GBA), a bidirectional network linking immune, microbial, and central nervous system (CNS) processes. Chronic intestinal inflammation, characterized by elevated cytokines

Leveraging chimeric antigen receptor-T cell therapy for the treatment of multiple sclerosis: Bridging oncology and neurology.

Multiple sclerosis (MS) is driven by complex interactions among B cells, autoreactive T cells, and compartmentalized inflammation behind the blood-brain barrier (BBB). Although treatment with B cell-targeting monoclonal antibodies (mAbs) has transformed MS management, their efficacy remains limited by inadequate penetration into the central nervous system (CNS), incomplete depletion of long-lived plasma cells, and development of treatment resistance in a significant number of patients. In recent years, chimeric antigen receptor (CAR)-T cell therapy has emerged as a new therapeutic approach to
