PURPOSE: To describe nonpathological myopia-related characteristics of the human eye.
METHODS: Based on histomorphometric and clinical studies, qualitative and quantitative findings associated with myopic axial elongation are presented.
RESULTS: In axial myopia, the eye changes from a spherical shape to a prolate ellipsoid, photoreceptor, and retinal pigment epithelium cell density and total retinal thickness decrease, most marked in the retroequatorial region, followed by the equator. The choroid and sclera are thin, most markedly at the posterior pole and least markedly at the ora serrata.

Epithelial cells of multiple types produce and interact with the extracellular matrix to maintain structural integrity and promote healthy function within diverse endogenous tissues. Collagen is a critical component of the matrix, and challenges to collagen's stability in aging, disease, and injury influence survival of adherent epithelial cells. The retinal pigment epithelium (RPE) is important for maintaining proper function of the light-sensitive photoreceptors in the neural retina, in part through synergy with the collagen-rich Bruch's membrane that promotes RPE adherence. Degradation of B

Age-related macular degeneration (AMD) is a progressive neurodegenerative disease affecting the central area (macula lutea) of the retina. Research on the pathogenic mechanism of AMD showed complex cellular contribution governed by such risk factors as aging, genetic predisposition, diet, and lifestyle. Recent studies suggested that microbiota is a transducer and a modifier of risk factors for neurodegenerative diseases, and mitochondria may be one of the intracellular targets of microbial signaling molecules. This review explores studies supporting a new concept on the contribution of microbi

A prospective study was conducted on 33 eyes of 33 patients with open-angle glaucoma who underwent trabeculectomy to investigate hemodynamic changes in the temporal optic nerve head (ONH) and peripapillary atrophy (PPA) after trabeculectomy. Laser speckle flowgraphy of ONH and PPA was performed at baseline and at 1, 3, and 6 months postoperatively. The waveforms of the mean blur rate in the tissue area (MT) in the temporal ONH, βPPA (with Bruch's membrane), and γPPA (without Bruch's membrane) were evaluated. Mean intra-ocular pressure (IOP) decreased from 19.1 ± 0.8 to 8.5-9.6 ± 0.7 mmHg at po

Doyne honeycomb retinal dystrophy (DHRD), also termed malattia leventinese (MLVT), is a dominantly inherited ocular disease characterized by the progressive accumulation of macular and peripapillary drusenoid material beneath the retinal pigment epithelium in the Bruch membrane. In all affected individuals genetically characterized to date, DHRD/MLVT is caused by a single heterozygous p.Arg345Trp missense variant in the EGF-containing fibulin-like extracellular matrix protein 1, EFEMP1. Recently, pathogenic variants in the EFEMP1 gene have also been demonstrated in several families with juveni

INTRODUCTION: Angioid streaks are breaks in Bruch's membrane that may be associated, among others, with pseudoxanthoma elasticum. Its most common complication is the development of subretinal neovascular membranes (SRNVM) and the decreased vision this entails.
CASE REPORT: A 28 year old woman with angioid streaks and SRNVM in the left eye, who received 3 injections of intravitreal bevacizumab, with rapid improvement in vision and stability during 11 months follow up. The finding of angioid streaks led to the diagnosis of pseudoxanthoma elasticum.
CONCLUSION: Intravitreal bevacizumab should b

Chorioretinal neovascularisation was noted in the midperipheral fundus of a young black girl with sickle cell anaemia. Rapid-sequence fluorescein angiography showed choroidally fed neovascularisation in the centre of a large black sunburst (chorioretinal scar). We suspect that a massive midperipheral retinal haemorrhage from the sickle cell disease caused the sunburst and altered Bruch's membrane, thus allowing the ingrowth of choroidally fed vessels.

Retinochoroidal coloboma results from an incomplete closure of the ocular embryonic fissure. Uni or bilateral, the coloboma can be associated with cataract, microphthalmia, retinal detachment... but rarely with neovascularization. This association is comparable with subretinal neovascularization in high myopia, angioid streaks, choroiditis and choroidal ruptures. The disruption of the normal anatomy of the Bruch's membrane is a possible entry site of choroidal blood vessels. But the reason why only rare cases are reported is unknown.

Recognizing the characteristic vertically oriented atrophy, pseudodrusen distribution, and retinal pigment epithelium-Bruch's membrane separation is critical for distinguishing extensive macular atrophy with a pseudodrusen-like appearance (EMAP) from age-related macular degeneration (AMD). Early identification of neovascular complications and prompt anti-vascular endothelial growth factor therapy can stabilize macular neovascularization (MNV) and help preserve residual vision in this rare retinal disorder. To the best of our knowledge, this is the first reported case of EMAP with MNV in an Asi

Age-related macular degeneration (AMD), a leading cause of vision loss among individuals over 60, is characterized by progressive retinal degeneration in a critical area for vision, the macula, and neovascular complications. Pigment epithelium-derived factor (PEDF) has the potential to address both of these pathologic processes because it has both neuroprotective and anti-angiogenic activities. In this study, we used a polymeric nonviral gene transfer platform to express PEDF in the eyes of mice and rats. In mice, there was high expression of PEDF in the retina and eyecup two weeks after subre

Myopia is significantly growing eye disease which accompanies various ocular pathologies including glaucoma. Understanding visual field (VF) and retinal nerve fiber layer (RNFL) damage observed in myopic eyes is important. Therefore, we evaluated optic disc margin anatomy using spectral-domain (SD) optical coherence tomography (OCT). We enrolled 40 healthy myopic patients and 64 myopic patients with RNFL defects in the superonasal region of the optic disc on red-free photographs and OCT. Optic disc stereophotographs were co-localized to SD-OCT images, and we analyzed the frequency with which t

Lewis rats were immunised with retinal S-antigen to induce experimental autoimmune uveitis and experimental autoimmune pinealitis. The blood-retinal and blood-pineal 'barrier sites' were examined by transmission electron microscopy. Inflammatory cell movement through Bruch's membrane involved separation of its constituent layers, cell migration through pores in the membrane, and between retinal pigment epithelial cells without causing significant displacement of the retinal pigment epithelial cell layer. The endothelial cells of retinal and pineal vessels with perivascular infiltrates showed m

To describe pathological changes in the anatomy of highly myopic (axially elongated) eyes, enucleated globes were examined by light microscopy and ocular structures were measured histomorphometrically. These studies revealed that highly axially myopic eyes show continuous thinning of the sclera starting at or behind the equator with a maximal thinning at the posterior pole; a profound thinning of the choroid decreasing from ∼ 250 to <10 μm in extreme axial myopia, secondary macular defects in the Bruch's membrane associated with a complete loss of retinal pigment epithelium and choriocapillari
