Assessing inhalation injury in the emergency room.

Respiratory tract injuries caused by inhalation of smoke or chemical products are related to significant morbidity and mortality. While many strategies have been built up to manage cutaneous burn injuries, few logical diagnostic strategies for patients with inhalation injuries exist and almost all treatment is supportive. The goals of initial management are to ensure that the airway allows adequate oxygenation and ventilation and to avoid ventilator-induced lung injury and substances that may complicate subsequent care. Intubation should be considered if any of the following signs exist: respi

Risk Factors for In-Hospital Mortality in Smoke Inhalation-Associated Acute Lung Injury: Data From 68 United States Hospitals.

BACKGROUND: Mortality after smoke inhalation-associated acute lung injury (SI-ALI) remains substantial. Age and burn surface area are risk factors of mortality, whereas the impact of patient- and center-level variables and treatments on survival are unknown. METHODS: We performed a retrospective cohort study of burn and non-burn centers at 68 US academic medical centers between 2011 and 2014. Adult inpatients with SI-ALI were identified using an algorithm based on a billing code for respiratory conditions from smoke inhalation who were mechanically ventilated by hospital day 4, with either a

Burns During Pregnancy: Implications for Maternal-Perinatal Providers and Guidelines for Practice.

IMPORTANCE: When a major burn is suffered during pregnancy, the obstetric provider is challenged to respond on an evidence basis because personal experience usually is lacking. Currently, there is a paucity of publications to inform the obstetrician, guide practice, and impact early critical decision making. OBJECTIVE: The aims of this study were to summarize the available information on early management of burns during pregnancy and to identify components of best practices for optimal outcome. EVIDENCE ACQUISITION: The PubMed database was searched for relevant titles and abstracts involving

[National experts consensus on tracheotomy and intubation for burn patients (2018 version)].

Airway edema, stenosis, obstruction and even asphyxia are easy to occur in patients with extensive burn, deep burn of head, face, and neck area, inhalation injuries, etc., which threaten life. Timely tracheotomy and intubation is an important treatment measure, but lack of knowledge and improper handling in some hospitals resulted in airway obstruction. The technique of percutaneous tracheotomy and intubation provides convenience for emergency treatment of critical burns and mass burn. The Chinese Geriatrics Society organized some experts in China to discuss the indications, timing, methods, e

Electrocardiographic changes during inhalational oleander toxicity.

Inhalational oleander toxicity was considered in a family of 4 by history of exposure to smoke from burning oleander twigs. Electrocardiography revealed first- and second-degree atrioventricular block with digoxin-like ST-T-wave changes, suggestive of oleander toxicity in the absence of exposure to digoxin or other herbal medicines, and without systemic illness. Complete blood count, biometabolic profile, chest x-ray, and echocardiography did not reveal any abnormalities. Electrocardiographies normalized within 4 days when kept away from offending agents. Because oleander plant materials are u

[The use of reamberin in antishock therapy in severely burned patients].

A 51-year-old severely burned woman had hospitalized at the Clinic of Thermal Injuries of the S.M. Kirov Military Medical Academy with a diagnosis: flame burn in a surface area of 40% (11%)/II-III b degrees of head, neck, trunk, limbs. Inhalation injury of moderate severity. The infusion drug of the combined action reamberin, which has a volemic and antihypoxic effect, had added to the complex antishock therapy. The presented clinical observation demonstrates the favorable course of burn shock: stopping of burn shock 28 hours after injury.

Investigating risk factors and antibiotic resistance of bloodstream infections causing Klebsiella pneumoniae in burn patients.

This study aimed to investigate the risk factors associated with bloodstream infections (BSI) caused by Klebsiella pneumoniae (KP) and the antibiotic resistance of KP in patients with severe burns. This retrospective study included severe burn patients admitted to the Author's Hospital. KP-induced BSI was confirmed by blood culture. The patients with confirmed KP-infection were placed in the KP-infection group and the remaining patients were included in the noninfection group. A total of 113 patients were included in the study, with 65 males. Among them, 23 patients were classified into the KP

Preexisting Conditions Influence Length of Stay in Burn Patients: Population-Based Retrospective Review.

BackgroundAge, total body surface area (TBSA), burn location, and inhalation injury are predictors of outcomes in burn patients. Preexisting conditions (PECs) impact the care and length of stay of burn patients. Limited knowledge is documented on how PECs impact length of stay beyond the traditional severity metrics in burn patients at the national level. Our purpose is to examine the relationship between PECs' status and length of stay (LOS) in the hospital (H) and intensive care unit (ICU) in this population, while controlling for traditional severity metrics.MethodsThe National Burn Databas

Burns in motor vehicle accidents.

Burns received as a result of motor vehicle accidents (MVA's) create special problems in their care, as they are frequently severe and are often associated with other injuries. One hundred seventy-eight consecutive patients with burns sustained in an MVA were studied. The mean TBSA burn was 33.9%. The mortality was 24.7%, but the mean burn size in this fatal group was almost doubled at 63.9%. The injury most commonly associated with death was inhalation injury (in 36.3%). Thirty-six per cent of the patients sustained other injuries in addition to their burn, the most frequent of which was to t

The 1942 fire at Boston's Cocoanut Grove nightclub.

The year 1992 marked the 50th anniversary of one of the worst civilian disasters in American history. On November 28, 1942, fire destroyed Boston's Cocoanut Grove nightclub, killing 491 people and sending hundreds more to area hospitals. The disaster came at a unique time in the history of burn care and resulted in a number of important advances in burn treatment, including the first comprehensive descriptions of inhalation injury, improvements in topical treatment of burn wounds, resuscitation of shock, use of antibiotics, and understanding of the metabolic response to injury. In addition, th

CLOSED-LOOP VENTILATION AND OXYGENATION WITH DECISION SUPPORT FLUID RESUSCITATION TO TREAT MAJOR BURN INJURY WITH SMOKE-INDUCED ACUTE RESPIRATORY DISTRESS SYNDROME.

Introduction: The understanding of the interaction of closed-loop control of ventilation and oxygenation, specifically fraction of inspired oxygen (FiO 2 ) and positive end-expiratory pressure (PEEP), and fluid resuscitation after burn injury and acute lung injury from smoke inhalation is limited. We compared the effectiveness of FiO 2 , PEEP, and ventilation adjusted automatically using adaptive support ventilation (ASV) and decision support fluid resuscitation based on urine output in a clinically relevant conscious ovine model of lung injury secondary to combined smoke inhalation and major

In vivo assessment of airway wall compliance during inhalation injury response using anatomical optical coherence elastography.

SIGNIFICANCE: Inhalation injury, a critical complication in patients with severe burns, contributes substantially to morbidity and mortality. Current diagnostic practices suffer from subjectivity and lack quantitative metrics. Enhanced diagnostic accuracy is imperative for improving treatment outcomes. AIM: Our objective was to develop normalized cross-sectional compliance (nCsC) of the airway wall, as measured by anatomical optical coherence tomography (aOCT), to reflect the severity of trachea inhalation injury. APPROACH: We employed a customized aOCT system that incorporates an intralumin
