Noninvasive measurement of cardiac output: two methods compared in patients with mitral regurgitation.

In search for the origin of the less reliable cardiac output (CO) estimations by means of electrical impedance cardiography (EIC), the authors hypothesized that cardiac valve pathology might be one of the reasons. Twenty-six patients were examined by means of echo Doppler (ED) and EIC. The cardiac valve status was obtained by means of echocardiography and color Doppler flow, while CO was obtained by means of both methods. Seventeen patients had no valve pathology (nVP) while nine patients had mild to moderate mitral regurgitation (MVR). The overall correlation between the calculation of CO by

Cardiac output variations in supine resting subjects during head-out cold water immersion.

Five men, aged 31.2 years (SD 2.3), under semi-nude conditions and resting in a dorsal reclining position, were exposed to thermoneutral air for 30 min, followed immediately by a cold water (15 degrees C) immersion for 60 min. Cardiac output was measured using a dual-beam Doppler flow meter. During immersion in cold water, cardiac frequency (fc) showed an initial bradycardia. The lowest values were reached at about 10 min after immersion, 58.3 (SD 2.5) to 48.3 (SD 7.8) beats min-1 (P < 0.05). By the 20th min of exposure, fc had gradually risen to 70.0 beats min-1 (SD 6.6, P < 0.05). This chang

[Noninvasive determination of cardiac output by continuous wave Doppler technique using apical approach].

The accuracy of a new method for measuring cardiac output with continuous wave Doppler technique from the cardiac apex (CWa) was studied in comparison with two conventional methods, pulsed Doppler technique using apical approach (PWa) and continuous wave Doppler technique using suprasternal approach (CWs). Study population consisted of 25 patients whose cardiac output had been determined by thermodilution method. None of the patients had aortic valve disease, significant tricuspid regurgitation or intracardiac shunt. Aortic flow velocity curves were recorded with the above three Doppler techni

Echocardiographic prediction of postoperative low cardiac output syndrome in patients with mitral stenosis.

Thirty-eight patients were operated on for mitral stenosis between March 1979 and September 1981. Thirty-six of them were examined as to their age, symptom duration, chest roentgenograms, electrocardiograms and echocardiograms to obtain various indices of left ventricular function. The usefulness of these indices as preoperative risk factors for predicting postoperative low cardiac output syndrome (LOS) was investigated. Cases which had values of ejection fraction, cardiac index, percent fiber shortening or mean velocity of circumferential fiber shortening less than 0.45, 2.0 l/min/m2, 25% and

Morphine effects on cardiac output and regional blood flow distribution in conscious dogs.

This study documents the changes in selected regional hemodynamics occurring in response to large doses of intravenous morphine. It was performed in normal conscious dogs. Their cardiovascular systems was unaltered by the presence of other drugs or recent surgery. The animals had been surgically prepared previously by implantation of chronic indwelling electromagnetic and Doppler ultrasonic flow probes on the aorta and the cranial mesenteric, renal and iliac arteries and placement of aortic catheters. Morphine, 1 mg/kg, produced mesenteric vascular dilation (30 per cent maximum decrease in res

Cardiac output, GFR, and renal excretion rates during maintained volume load in rats.

The correlation among cardiac output (CO), glomerular filtration rate (GFR), fractional tubular sodium rejection (TFRNa), and renal excretion rates of water and salt was investigated during ischemic blood volume expansion in rats. Initially circulating blood volume was equilibrated isovolemically with a reservoir volume of 6% albumin solution equal to one-third the estimated blood volume. Later the equilibrated reservoir contents were infused intravenously. CO was measured by thermodilution, GFR by inulin clearance. Significant linear correlations existed between GFR and the rates of urine flo

Immediate effect on cardiac output of reversion to sinus rhythm from rapid arrhythmias.

Cardiac output was estimated immediately before and after conversion to sinus rhythm in nine patients with rapid arrhythmias. Conversion was by synchronized direct-current shock in eight patients, and by direct atrial wall stimulation in the other. In seven patients there was an immediate increase in cardiac output after restoration of sinus rhythm. The percentage increase in output was directly proportional to the rate of the arrhythmia immediately before conversion (r=0.91, P<0.01). The critical heart rate, above which an immediate increase in cardiac output might be expected on conversion t

A performance comparison of the most commonly used minimally invasive monitors of cardiac output.

BACKGROUND: Shock is common in critically ill and injured patients. Survival during shock is highly dependent on rapid restoration of tissue oxygenation with therapeutic goals based on cardiac output (CO) optimization. Despite the clinical availability of numerous minimally invasive monitors of CO, limited supporting performance data are available. METHODS: Following approval of the University of Saskatchewan Animal Research Ethics Board, we assessed the performance and trending ability of PiCCOplus™, FloTrac™, and CardioQ-ODM™ across a range of CO states in pigs. In addition, we assessed the

[Chorangioma of the Placenta - A Rare Placental Cause of Fetal High Output Cardiac Failure].

Chorangiomas of the placenta, benign tumors of chorionic tissue, are a rare placental cause of adverse fetal and maternal outcomes. We describe the case of a large placental chorangioma leading to polyhydramnios as well as consecutive preterm birth and high output cardiac failure of the newborn. Derived from a literature review, we suggest instructions for diagnosis and optimal prenatal care in case of a a suspected placental chorangioma.

Cardiac output monitoring during Cesarean delivery in a patient with palliated tetralogy of Fallot.

PURPOSE: Tetralogy of Fallot (TOF) is one of the most common causes of cyanotic congenital heart disease. The anesthetic management of parturients with uncorrected TOF is challenging and controversial, especially for Cesarean delivery (CD). We describe the use of noninvasive cardiac output (CO) monitoring to assist the management of CD for a woman with palliated TOF under general anesthesia. CLINICAL FEATURES: A 34-yr-old woman presented for elective CD at 38 weeks gestation. Having been born with TOF, she underwent a modified Blalock-Taussig shunt at six years of age, followed nine years lat

Influence of the renal nerves on sodium excretion during progressive reductions in cardiac output.

The purpose of this study was to elucidate the role of the renal nerves in promoting sodium retention during chronic reductions in cardiac output. In five dogs, the left kidney was denervated and the urinary bladder was surgically divided to allow separate 24-h urine collection from the innervated and denervated kidneys. Additionally, progressive reductions in cardiac output were achieved by employing an externally adjustable occluder around the pulmonary artery and by servo-controlling right atrial pressure (control = 0.9 +/- 0.2 mmHg) at 4.7 +/- 0.1, 7.5 +/- 0.1, and 9.8 +/- 0.2 mmHg for 3 d

Validity of cardiac output measurement by the thermodilution method in the presence of acute tricuspid regurgitation.

Evaluation of patients with acute tricuspid insufficiency may include assessment of cardiac output by the thermodilution method. The accuracy of estimates of thermodilution-derived cardiac output in the presence of tricuspid insufficiency has been questioned. This study was designed to determine the validity of the thermodilution technique in a canine model of acute reversible tricuspid insufficiency. Cardiac output as measured by thermodilution and electromagnetic flowmeter was compared at two grades of regurgitation. The relationship between these two methods (thermodilution/electromagnetic)

Single- versus dual-chamber sensor-driven pacing: comparison of cardiac outputs.

Previous studies have shown that single-chamber sensor-driven pacing improves exercise tolerance for patients with chronotropic incompetence. However, long-term single-chamber pacing has a number of inherent problems that limit its usefulness. Although sensor-driven dual-chamber pacing largely obviates the problems inherent with single-chamber sensor-driven pacing, the physiologic benefit of dual-chamber sensor-driven pacing has not yet been demonstrated. Accordingly, the purpose of this study was to compare exercise-induced cardiac output for patients with chronotropic incompetence, after pro

Ventilation and cardiac output during the onset of exercise, and during voluntary hyperventilation, in humans.

Three normal subjects performed rest--exercise transitions on a cycle ergometer, from rest to unloaded pedalling (0 W), 50, 100 and 150 W. Each experiment was performed in triplicate, with randomized work load order, in two sessions. Ventilation was obtained breath-to-breath by integration of a pneumotachygraph signal, and cardiac output beat-to-beat by a new development of the Doppler technique. Results were bin-averaged in 4 s bins over the first 20 s, and compared to resting values. Both ventilation and cardiac output increased significantly in the first 2 s. This initial rise in ventilatio

The effect of halothane on the distribution of cardiac output and organ blood flow in the hypovolemic dog.

The circulatory effect of halothane anesthesia given to moderately hypovolemic dogs (blood loss 25 ml/kg b.w.) was investigated using the radioactive microsphere technique. The effect of the anesthetic was reflected by a decrease in the arterial mean blood pressure and a reduced blood flow to the heart, whereas the blood flows to other organs did not change significantly but were well preserved in vital organs (brain, kidneys, liver and lungs). The effect on the circulation of reinfusing the withdrawn blood was also studied.
